A pelvic cake kidney with hydronephrosis and renal failure with the single draining ureter was reported. Frequent urination related to compression of the bladder was the most important symptom. The kidney was craniolaterally displaced, the malfunctioning ureter was replaced with the Boari-like tubularization of the bladder, and the neoureter was anastomosed with the calyx. The bladder was enlarged by intestinal augmentation.

A 90-year-old male developed acute onset of abdominal and lumbar pain due to the rupture of an 11-cm abdominal aortic aneurysm. A congenital fused pelvic kidney perfused by three renal arteries arising from iliac axes was detected. In an emergent setting, an aorto-uni-iliac endograft was deployed through right femoral surgical access with occlusion of the upper right renal artery. An occluder device was placed in the common iliac artery above the renal artery through left femoral access. A femorofemoral crossover bypass completed the procedure. The patient developed acute renal failure, with n

Fused supernumerary kidney (fused SNK) is an extremely rare congenital malformation of the urinary system. This article reports a 59-year-old male patient who was admitted to the emergency department because of severe right flank pain and gross hematuria 4 days after a fall. Computed tomography angiography (CTA) revealed a complex combination of variations: the absence of the left kidney and left renal vessels and 3 partially fused supernumerary kidneys (upper pole, middle, and lower pole units) in the right renal fossa with 3 independent collection systems. The vascular and ureteral anatomy w

Cadaveric donors with congenital renal anomalies are often refused because of technical difficulties. As the waiting periods for kidney transplants continue to increase, transplant centers need to consider them also as potential donors. This paper describes transplantation of a crossed fused disc type ectopic kidney to two patients with end-stage renal disease, after parenchymal transection. The transplants were successful with minimal complications, and the recipients had immediate graft function. It is now one year from transplantation, with excellent renal functions.

BACKGROUND Crossed fused renal ectopia (CFRE) is an uncommon congenital renal anomaly in which 1 kidney crosses the midline and fuses with the contralateral kidney. Although CFRE is often asymptomatic, its rare association with renal malignancy poses substantial diagnostic and surgical challenges due to abnormal anatomy and vascular supply. This report describes clear cell renal cell carcinoma (RCC) arising in CFRE with direct invasion into the contralateral, normally positioned kidney. CASE REPORT A 60-year-old man presented with a 2-month history of mild left loin pain without urinary or sys

Crossed renal ectopia is a rare congenital anomaly, with non-fused variants being exceptionally uncommon. The management of nephrolithiasis in such kidneys is challenging due to the abnormal anatomy, malrotation, and altered ureteral course. We report a case of a non-fused left crossed ectopic kidney with a 21 × 14.5 mm with density of 1250 HU renal calculus successfully managed with retrograde intrarenal surgery (RIRS), achieving complete stone clearance (SC). This case highlights the feasibility of RIRS, even in complex renal anomalies with a significant stone burden. A review of the existin

Complete fused renal ectopia, also known as pancake kidney, is a rare congenital renal fusion anomaly characterized by complete fusion of both kidneys within the pelvis. Most patients remain asymptomatic and diagnosis is often incidental. I report a 49-year-old woman from a low-resource setting who presented with lower urinary tract symptoms and dyspareunia. Initial management was directed toward recurrent cystitis; however, further evaluation with computed tomography and intravenous urography revealed a pancake kidney located in the pelvis with two distinct collecting systems and normally dra

Horseshoe kidneys are a common, yet enigmatic, renal malformation. This review critically appraised the literature surrounding the embryology, etiology and clinical anatomy of horseshoe kidneys. The systematic literature search produced 104 articles, and 56 primary and further secondary references. There were several etiological theories regarding horseshoe kidneys. The established view was that during ascent, the kidneys come into close apposition as they pass through an arterial fork. Another possible mechanism related to lateral flexion of the trunk or rotation of the caudal embryo; the ass

Horseshoe kidney (HSK) is a common renal malformation with unique and complex characteristics. A systematic literature search was conducted using PubMed and ScienceDirect databases. Several theories have been proposed regarding HSK formation, such as the close apposition of the kidneys during ascent through an arterial fork, lateral flexion of the trunk, and caudal embryonic rotation. Emerging evidence from animal models implicates notochord signaling and the sonic hedgehog pathway in HSK formation. The isthmus, a defining feature of HSK, is hypothesized to arise from ectopic mesenchymal tissu

INTRODUCTION: Horseshoe kidney is the most common congenital renal fusion anomaly, with an estimated incidence of 1.7 to 2.5 cases per 1000 live births. In these patients, nephron-sparing surgical management of renal tumors may be complicated by abnormal renal location, aberrant vasculature, and the presence of a renal isthmus. We present the largest known series of patients with renal malignancy in horseshoe kidneys managed by partial or hemi-nephrectomy with associated outcomes.
MATERIALS AND METHODS: A retrospective review of our institution's electronic medical record was conducted to ide

Renal fusion anomalies are common congenital anomalies of the urogenital tract and have their genesis in the early embryonic period. They are classified into partial fusion anomalies (e.g., crossed fused ectopia, and horseshoe kidney) and complete fusion anomalies (e.g., fused pelvic kidney). Horseshoe kidney is the most common renal fusion anomaly and is characterized by the presence of two distinct functioning kidneys on either side of the vertebral column, with fusion occurring at the inferior poles in majority of the cases. Crossed fused ectopia is characterized by the presence of an ectop

We present a 20-year-old man with a chronic discharging sinus in the left loin arising from a horseshoe kidney with a large pelvicalyceal calculus on its left side. The left moiety was non-functional, so a left hemi-nephrectomy, together with excision of the whole fistula tract, was carried out. Histopathology was suggestive of chronic interstitial nephritis. Patients with horseshoe kidney are prone to development of renal stones and their associated complications. The unfamiliar anatomy of horseshoe kidneys may increase the complexity of any surgical procedure. Radiological investigations are

OBJECTIVE: Horseshoe kidney is the most common renal fusion anomaly. Its peculiar anatomical characteristics increase the risk of lithiasis formation and always entails a surgical challenge for its treatment.
MATERIAL AND METHODS: We present the cases of men with horseshoe kidneys diagnosed by CT one case of alarge pyelic lithiasis with others of a smaller size in the calyces, and the other of a big solitary lithiasis in renal pelvis. RESULTS: We performed a laparoscopic pyelolitectomy to remove the pyelic lithiasis and we used a flexible cystoscope through a trocar to extract the calyceal st

BACKGROUND: Crossed fused renal ectopia (CFRE) is a common congenital anomaly where one kidney is positioned abnormally on the opposite side of the midline, often fused with the other kidney. However, single ureter draining crossed fused renal ectopia is a rare occurrence.
CASE REPORT: Here, we report a case of crossed fused renal ectopia with a single ureter in a 46-year-old Nepali male who presented with history of lithuria. Computed tomography revealed that the left kidney was situated on the right side and fused with the right kidney. The renal pelvises of both kidneys were fused, and a s

BACKGROUND Horseshoe kidney is a rare congenital anomaly, occurring in approximately 1 in 400 to 1 in 1800 individuals. When considering transplantation, horseshoe kidneys present unique challenges due to vascular and urinary tract anomalies. They can be transplanted either as a whole, en bloc unit or as 2 separate, split fragments. The complexities in vascular and urinary reconstruction require careful consideration to ensure successful transplantation. CASE REPORT We present a case involving the transplantation of a horseshoe kidney from a deceased 36-year-old male donor with confirmed brain

Nephrolithiasis is one of the most common indications for surgery in patients with a horseshoe kidney. Robotic-assisted surgery has become a staple in urologic practice, yet its application in stone management is largely undefined. We present a patient with a horseshoe kidney, who underwent a robotic-assisted laparoscopic pyelolithotomy (RPL) to treat a 3 cm stone burden. This procedure allowed for safe access that could not be obtained with percutaneous nephrolithotomy (PCNL) and stone removal without fragmentation, which would have been challenging with traditional laparoscopy. We advocate f
