[Reticular elastolytic giant cell granuloma. A variant of the annular elastolytic giant cell granuloma].

The case report presented describes a new variant of annular elastolytic giant cell granuloma. Over a period of 10 years, a 40-year-old woman had developed reticular, brown to livid, partly atrophic skin lesions on the shoulders and decolletée. The patient had no subjective symptoms. Histology revealed a patchy lymphohistiocytic infiltrate in the dermis, with many giant cells. Elastic fibres were less frequently found in these infiltrates; in a few of the giant cells and macrophages fragments of elastic fibres were demonstrated within the cytoplasm. There were no deposits of mucin. For this ne…

Light chain renal amyloidosis with prominent giant cells.

Clinical diagnosis of amyloidosis may be very challenging because signs, symptoms, and laboratory study results can be highly variable and m … chain (AL) κ amyloidosis with an unusual inflammatory infiltrate including prominent multinucleated giant cells in the interstitium and at the glomerular hilus. Amyloid was apparent within giant cells on Congo Red staining, as well as on ultrastructural evaluation. Together with prior studies of tumoral nonrenal amyloid and renal amyloid A, we suggest that th …

Small intestinal carcinoma with osteoclast-like giant cells.

An elderly woman underwent jejunectomy for a large, stenosing neoplasm. Histologically, we found an undifferentiated carcinoma with scattered mononuclear and multinuclear macrophages in the intestine and lymph node metastases. The multinuclear cells, being acid phosphatase-positive and CD68-immunoreactive, are referred to as osteoclast-like giant cells. Hepatic secondaries were discovered 6 months postoperatively. The patient succumbed to a chemotherapy-related septic event. We suggest that intratumoral infiltration by mononuclear and multinuclear macrophages expresses one of the body's defens…

Multinucleated epidermal giant cells in inflammatory skin diseases.

The occurrence of multinucleated epidermal giant cells (MEGCs) with more than three clumping nuclei has been considered an episodic and exceptional phenomenon in inflammatory skin diseases. However, we have found them in 97 histologic specimens obtained from inflammatory lesions. They appeared in 88 of 196 biopsy specimens from a group of patients with chronic eczema or prurigo. In pruriginous lesions alone, they were found in 67 (74%) of 90 specimens. On the other hand, MEGCs were found rarely in lichen amyloidosus (2/11), dermatitis herpetiformis (1/6), erythema multiforme (1/11), pustular p…

Pleural malignant mesothelioma with osteoclast-like giant cells.

Reported herein is the first case of malignant mesothelioma with osteoclast-like giant cells (OGC) in a 55-year-old man, a building contractor. Open thoracoscopic biopsy from the left parietal pleura showed a pleomorphic tumor composed of round to spindle cells mixed with numerous OGC; focal hemorrhage and necrosis were observed, but neither bone nor osteoid tissue was seen. The tumor cells had eosinophilic cytoplasm and round to oval nuclei with varying degrees of atypia. OGC had multiple small uniform nuclei without atypia and large eosinophilic cytoplasm, similar to giant cells of the so-ca…

Giant lymphadenopathy infiltrated by gaucher cells mimicking lymphoma.

Gaucher disease (GD) is a lysosomal storage disease characterized by deficiency of beta-glucocerebrosidase, which results in accumulation of glucocerebroside in reticuloendothelial system, bone marrow infiltration, progressive hepatosplenomegaly, and skeletal complications. Herein we report a 5-year-old female with GD receiving enzyme replacement therapy who had giant mesenteric lymphadenopathies.

Multinucleated foreign body giant cells in placental membrane.

We report the pathologic finding of a multinucleated foreign body giant cell reaction to squames and fetal hair in the placental membranes in a 37-week 1-day intrauterine gestation. This reaction appeared to have developed in association with repeated intrauterine procedures performed in the third trimester, including cordocentesis for fetal blood sampling, intrauterine blood transfusion, and amnioreduction for polyhydramnios. This type of reaction most likely was directed to prolonged amniotic fluid leakage that occurred spontaneously or after intrauterine procedures in the second half of the

Multinucleated stromal giant cells of the uterine cervix.

We have studied two cases in which numerous, multinucleated giant cells were present as an incidental finding within the superficial stroma of the cervix. The cells exhibited varying degrees of nuclear atypia and resembled some of the cells found within fibroepithelial polyps of the lower female genital tract. Although the pathogenesis of the lesion is unknown, we believe that the lesion is nonneoplastic and probably represents an exaggeration of the nuclear atypia occurring within the superficial stromal cells of the lower female genital tract. These cells may be mistaken for malignant cells …

[Diagnosis of giant cell arteritis].

DIAGNOSIS OF GIANT CELL ARTERITIS. The diagnosis of giant cell arteritis (GCA) must be made promptly in order to initiate appropriate treatment aimed at relieving symptoms and avoiding ischemic complications, particularly visual ones. The diagnosis of GCA is based on the occurrence, in a patient over 50, of clinical signs of GCA, primarily recent headaches, or polymyalgia rheumatica, as «evidence» of large-vessel vasculitis, which is provided by histological analysis of an arterial fragment, usually the temporal artery, or by imaging of the cephalic arteries, the aorta and/ or its main branche

[An update on giant cell arteritis].

Giant cell arteritis (GCA) is a primary granulomatous systemic vasculitis involving the aorta and its main branches that affects people aged over 50 years with a genetic predisposition. Its main phenotypes are cranial and extracranial involvement, with or without symptoms of polymyalgia rheumatica. These phenotypes can overlap. The extracranial form can be oligosymptomatic and must be sought directly. The main complications of the disease are ischemia of essential territories such as the optic nerve or cerebral circulation, and aneurysmal dilations of the aorta and its large branches. Clinicia

Treatment of giant cell arteritis.

Giant cell arteritis (GCA) is the most common form of vasculitis in adults. Cranial manifestations are typical clinical features of this vasculitis. Sometimes the presenting symptoms are nonspecific and, in some cases, large-vessel involvement may prevail. Polymyalgia rheumatica is a frequent manifestation that in some cases may be the presenting symptom of GCA. Visual complications, in particular the risk of blindness, constitute the most feared manifestations of GCA. Prompt recognition of this vasculitis is required to avoid irreversible complications. Prednisone/prednisolone at a dose of 40

[Update Giant Cell Arteriitis].

Giant cell arteriitis (GCA) is the most common systemic vasculitis in patients ≥  50 years. GCA belongs to the group of large vessel vasculitides. Clinical hallmarks of GCA are persistent localized headache, jaw claudication, limb claudication, symptoms of polymyalgia rheumatica, constitutional symptoms and acute visual symptoms. A fast track diagnostic workup and subsequent immediate treatment are crucial to avoid persistent ischemic lesions, particularly blindness. This article reviews the EULAR recommendations for the use of imaging in large vessel vasculitis in clinical practice and the 20

Tocilizumab in Giant Cell Arteritis.

Giant cell arteritis is a granulomatous immune-mediated vasculitis of medium and large vessels. It most commonly affects white females over the age of 50 and is the most common primary vasculitis in the United States. Treatment of this disease has classically been with high-dose corticosteroids, but this therapy has been associated with severe morbidity and mortality. Tocilizumab, a humanized monoclonal antibody targeting the interleukin-6 receptor, has been used with great efficacy and safety in rheumatoid arthritis and systemic-onset juvenile idiopathic arthritis. As interleukin-6 has been s
