This paper was undertaken to distill a century of thought on the larynx, specifically in relation to the posterior glottis, an area which has only recently been recognized as a distinct part of the larynx. The arytenoidopexy operation has been performed in 15 cases of bilateral abductor paralysis of different causes. A tube on n. 6 for women and a tube of n. 6.5 for men has been used for the intratracheal general anesthesia without a tracheotomy. Initial clinical experiences of a new method for the treatment of glottic synechiae developed in animal experiments are reported. Adhesions were divided by a microsurgical technique without opening the larynx. A plastic keel was fixed by stitching it from inside out between the separated cicatrial membranes. The keel was removed after 5 to 6 weeks. The adhesions were successfully relieved.
