A 75-year-old man had been monitoring his glucose using a blood glucose monitoring system at the same body site for at least 20 years (>7300 needlesticks). The asymptomatic skin lesion had been present for many years. He used the same site because it hurt less than the fingers and bled well. His medical history was remarkable for diabetes mellitus, hypertension, coronary artery disease, and a pacemaker. His medications included glipizide, metformin, carvedilol, furosemide, lisinopril, amlodipine, clopidogrel, and aspirin. Physical examination revealed a brown, slightly raised, rough plaque wit…

Concentrative and facilitative glucose transporters are responsible for the movement of glucose across the plasma membrane of human cells. Defects in concentrative glucose transporters cause renal glycosuria and glucose-galactose malabsorption. Alterations in facilitative glucose transporters explain the newly discovered syndrome of low CNS glucose in the presence of normal blood sugar, causing seizures and developmental delay. Defects in other facilitate glucose transporters also help explain Fanconi-Bickel syndrome, glycogen storage disease type, Id, and non-insulin-dependent diabetes mellit…

The advent of dry chemistry techniques has made inexpensive glucose testing equipment available to physicians and their patients. The ability to measure blood glucose concentration quickly, in almost any setting, has eliminated urinary glucose monitoring as the parameter of choice for managing diabetic patients. Blood glucose monitoring has allowed improved control of diabetic patients who avail themselves of one of the techniques. The physician choosing to offer blood glucose testing in the office laboratory has a variety of test equipment from which to choose. Most physicians will want to ow…

Home monitoring of blood glucose is an important new tool in the management of patients with diabetes mellitus. Current monitoring techniques are safe, reliable and reproducible, and they enable diabetics to participate in their own care. There are no contraindications to home glucose monitoring, but some consideration must be given to cost and maintenance. The physician can evaluate the effectiveness of home monitoring by determining the glycosylated hemoglobin level.

BACKGROUND: Sweat contains glucose that can accurately reflect blood glucose. However, skin surface glucose can confound these measurements.
METHODS: A perfusion method was used to rapidly harvest sweat from forearm sites on human subjects. The sweat samples were analyzed for glucose by high-performance liquid chromatography methods and compared with the results obtained with a blood glucose meter.
RESULTS: The results of 23 different studies of seven individual subjects with diabetes show a strong correlation between sweat glucose and blood glucose.
CONCLUSION: Sweat glucose, when properly…

Sodium fluoride was inadvertently added as a preservative to the urine of an eight-year-old boy with diabetes mellitus before urinary glucose was measured. On preliminary screening of the urine, the test by glucose oxidase paper reagent strip gave a negative reading for glucose, whereas quantitative urinary glucose assay by the coupled enzyme reaction (hexokinase-glucose 6-phosphate dehydrogenase) gave a glucose concentration of 81.5 g/liter. Inadvertent use of sodium fluoride as a urine preservative may cause a falsely negative result with the glucose tests involving oxidase.

An electrocatalytic glucose sensor for in vivo application has been developed. The sensor is a flow-through cell with three electrodes and can be integrated into a blood vessel. The principle of measurement is based on the direct electrochemical oxidation of glucose at a membrane-covered noble-metal electrode. To test the potential long-term in vivo function of the sensor, it was implanted in the carotid artery of a sheep. Thus, the sensor performance was verified over a period of 71 days. During this time, a nearly constant blood flow through the cell was achieved, which indicates good blood …

Background Neonatal hypoglycemia is a common metabolic disturbance in neonates. Glucose monitoring is recommended for certain groups of neonates, including those whose mothers have pre-gestational or gestational diabetes. Little is known about the relevance of glucose monitoring in neonates whose mothers have an elevated screening glucose challenge test (GCT) but a normal oral glucose tolerance test (OGTT). Objectives The objectives of this study were to determine if neonates of mothers with an elevated GCT but a normal OGTT had hypoglycemia and to establish if there was an association between…
