Callus distraction in the hand skeleton.

From 1992 to 1996, 27 patients with traumatic amputations or malformations underwent lengthening of thumb and fingers. A total of 36 procedures were carried out. In several cases, deepening of the web space or bone transplantations proved to be necessary to improve general function or to compensate for missing bone structure. Complications included pin-track infections, necrosis of bone, non-union and scarring. Overall the results indicate that distraction of the thumb and fingers is a feasible operative technique leading to a promising improvement of function.

Roles and functions of occupational therapy in hand rehabilitation.

Occupational therapy makes a major contribution to the practice and continued development of hand rehabilitation. The unique combination of training in physical, functional, psychological, social, and vocational aspects of physical dysfunction enables the occupational therapist to provide comprehensive treatment necessary to return the patient with hand dysfunction to a productive life style.

Current state of treatment of Dupuytren's disease.

On overview is present which briefly summarizes the incidence, pathology and natural progress of Dupuytren's Disease. The importance of recognizing the Dupuytren diathesis in planning the surgical treatment, the importance of conservatism in initiating surgery, and the principles of surgical management including dermofasciectomy are presented, and it is once again stressed that a distinction exists between recurrence and extension of Dupuytren's Disease post-operatively.

Multiple grade II deformities in a child: tragic effect of leprosy.

Stigma is almost synonymous with visible deformity and this is what makes leprosy a dreaded disease for the common man. Deformities in children are more distressing, due to the limitation of their physical, academic and social life. An 11-year-old female child was presented with trophic ulceration of right foot and triple nerve paralysis (i.e. total claw hand with wrist drop) of the left hand of 2 years duration. In addition, she had multiple hypopigmented hypoaesthetic patches on the extremities and buttocks. She was diagnosed as a case of borderline tuberculoid leprosy and was started on WHO

Swan-Neck and Boutonniere Deformity in Rheumatoid Hand.

Rheumatoid arthritis (RA) is an inflammatory disease for which the exact cause is unknown. It is an autoimmune disorder that primarily affects such joints which allows free movement in different directions, but also has many other severe complications related to mortality. On the other hand, this disease is responsible for digital deformities, arising out of a combination of synovitis, intrinsic muscles, and extrinsic tendons impairment, ligament rupture, joint dislocation, and final ankylosis. Articular swelling due to synovial proliferation is the first manifestation, but later deformity, su

Volar Tenodesis for the Treatment of Swan-neck Deformity; A Systematic Review.

BACKGROUND: Swan-neck deformity is a common problem particularly in patients with Rheumatoid arthritis. Mobile swan-neck deformities (Nalebuff types I,II) can be treated non-operatively and operatively. In this paper we report on a systematic review of the treatment of swan-neck deformities with volar tenodesis. METHODS: We performed a literature search and analysed the literature according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Only eight papers were eligible. None was of high quality. The data reporting was very variable. Therefore, no

Boutonniere deformity in Dupuytren's disease.

The boutonnière deformity in Dupuytren's disease has been found to be due to fibrous contraction of the transverse retinacular ligament which anteriorly displaces the fibres of the extensor apparatus. Complete ablation of this ligament and liberation of the extensor apparatus allows easy extension of the finger when operating on severe cases of Dupuytren's contracture.

Osteotomy for digital deformity.

Osteotomy may be indicated in digital phalanges to correct angular or rotational deformity resulting from malunited fracture, epiphyseal arrest, or congenital or developmental causes. The use of fine power tools facilitates precise correction. lateral angulation is corrected by closing wedge osteotomy, dorsal angulation by opening wedge and bone graft, rotational deformity by transverse osteotomy, and derotation through the involved phalanx. Smooth Kirschner wires stabilize the osteotomies to permit early joint motion. These procedures can safely be performed in children.

The conservative treatment of the Swan-Neck deformity in the rheumatoid hand.

A technique is described to correct rheumatoid Swan-Neck deformity in those patients where there is limited proximal interphalangeal joint flexion in all positions. It involves the manipulation of these joints and their temporary immobilisation with Kirschner wires followed by mobilisation using an elastic band method.
