Behavioral sequelae of closed head injury. A quantitative study.

We determined the profile of behavioral disturbance in relation to closed head injury of graded severity. Patients with severe injuries, as defined by duration of coma and the presence of neurological deficit, were differentiated from a group of mildly injured patients by behavioral ratings that reflected cognitive disorganization, emotional withdrawal, and motor retardation. Neurologic measurements of injury related to the severity of behavioral disturbance included hemiparesis, aphasia, and abnormalities on computerized axial tomography. Agitation during the acute phase of injury was also pr

Wrestling-related concussions and closed head injuries predominantly occur in high school age athletes.

BACKGROUND: Sports-related concussions (SRC) and closed head injuries (CHI) have recently garnered national attention given mounting concern for long-term neurological sequelae resulting from repetitive head trauma. Despite historically dangerous techniques in wrestling that involve impacts to the head, there is a paucity of epidemiologic data in regard to wrestling-related concussions (WRCs) in the United States (USA). METHODS: The National Electronic Injury Surveillance System (NEISS) database was queried (2000-2018) to report national estimates and demographic characteristics of patients 6

The therapeutic use of computers in occupational therapy with patients with closed head injuries.

Occupational therapists in various settings were surveyed to obtain information on their use of computers with closed head injured patients. The majority of therapists use micro-computers for cognitive rehabilitation followed by vocational rehabilitation. Respondents indicated that an efficient means of distributing information does not yet exist resulting in a high incidence of trial and error learning on the part of the therapist.

The use of clinical CCT images in the forensic examination of closed head injuries.

The forensic evaluation of clinical cranial computed tomographies (CCT) frequently is the only reliable source of morphological evidence in head injuries when the injured individual survives or when death is delayed and autopsy findings are characterized by secondary changes. We have reviewed 21 cases where clinical CCT examinations were used to establish a medico-legal diagnosis. In 18 cases falls (n = 13) could be distinguished from blows (n = 5) due to the presence and/or absence of coup and contrecoup lesions and linear or depressed skull fractures. In two cases the striking object could b

The effect of a femoral fracture on concomitant closed head injury in patients with multiple injuries.

OBJECTIVE: We sought to determine the effect of a femoral shaft fracture, and its treatment by early intramedullary nailing, on the neurologic outcome of patients with multiple injuries with a concomitant head injury. DESIGN: Retrospective, case-control design using a prospectively gathered trauma data base. MATERIALS AND METHODS: We identified 46 patients with multiple injuries (mean Injury Severity Score [ISS] = 33.2) with closed head injuries (mean Glasgow Coma Scale [GCS] score = 7.8) and femur fractures, and matched as controls 99 patients with multiple injuries with head injuries but w

Clinical studies on diffuse axonal injury in patients with severe closed head injury.

OBJECTIVE: To discuss the clinical criteria for diagnosing diffuse axonal injury (DAI). METHODS: Clinical and computed tomographic features of 117 patients with severe closed head injury were analyzed. The authors preliminarily put forward CT diagnostic criteria of DAI, that is, 1) single or multiple small intraparenchymal hemorrhages in the cerebral hemispheres (< 2 cm in diameter); 2) intraventricular hemorrhage; 3) hemorrhage in the corpus callosum; 4) small focal areas of hemorrhage adjacent to the third ventricle (< 2 cm in diameter); and 5) brain stem hemorrhage. All patients were divid

[Headache as the consequence of brain concussion and contusion in closed head injuries in children].

The most frequent type of head injury in children is closed head trauma with brain concussion or contusion, and headache is the dominant complaint of early and late postinjury period. Because of scant number of studies on the problem of occurrence, characteristics and persistence of posttraumatic headache this study was undertaken in a group of 100 children (29 girls and 71 boys), aged 3-14 years old, 90 after brain concussion and 10 after contusion. Children with a history of injuries, central nervous system infections, with headaches before injury and chronic diseases were excluded. In 9 cas

[Injuries of cranial nerves II-VII in 350 patients hospitalized due to closed head trauma].

Ophthalmological examination was performed in 350 consecutive inpatients with closed head injury, treated in the Neurosurgical Department, Białystok over a period of two years. The group comprised 266 men and 84 women, aged between 16 and 90 years. The purpose of the study was to screen for the symptoms of damage to cranial nerves II-VII. In 12% of cases internal, external or total III nerve palsy was identified. In 2% of cases there were symptoms of damage to the VII nerve, in 1.4% of damage to the V nerve and in 1.1% to the II nerve. In 0.6% of cases damage was observed to the VI nerve. Ther

Behavioral and Structural Effects of Single and Repeat Closed-Head Injury.

BACKGROUND AND PURPOSE: The effects of multiple head impacts, even without detectable primary injury, on subsequent behavioral impairment and structural abnormality is yet well explored. Our aim was to uncover the dynamic changes and long-term effects of single and repetitive head injury without focal contusion on tissue microstructure and macrostructure. MATERIALS AND METHODS: We introduced a repetitive closed-head injury rodent model (n = 70) without parenchymal lesions. We performed a longitudinal MR imaging study during a 50-day study period (T2-weighted imaging, susceptibility-weighted i

Novel model of frontal impact closed head injury in the rat.

Frontal impact, closed head trauma is a frequent cause of traumatic brain injury (TBI) in motor vehicle and sports accidents. Diffuse axonal injury (DAI) is common in humans and experimental animals, and results from shearing forces that develop within the anisotropic brain. Because the specific anisotropic properties of the brain are axis-dependent, the anatomical site where force is applied as well as the resultant acceleration, be it linear, rotational, or some combination, are important determinants of the resulting pattern of brain injury. Available rodent models of closed head injury do

Cerebral venous congestion following closed head injury in a child.

In pediatric patients with head injury, progressive hemispheric low density (HLD) on CT is a specific phenomenon although the underlying mechanism remains controversial. We used CT, MRI, diffusion weighted imaging (DWI), and perfusion weighted imaging (PWI) studies to identify pathophysiological patterns in a child. A 6-year-old boy sustained a left temporal head injury in a car accident. Although the initial CT was normal, serial CT demonstrated progressive left HLD; an extensive heterogeneous high-intensity lesion was noted on fluid-attenuated imaging recovery (FLAIR) and DWI studies. On PWI

Brief report: anosmia and remote outcome in closed head injury.

The value of posttraumatic anosmia as a predictor of late social outcomes was examined in a sample of closed head injury (CHI) patients. Unemployment rates were equally high in both the anosmic and nonanosmic closed head injury patients. The groups also did not differ in psychiatric or neuropsychological status. Anosmic patients had longer initial hospital stays and deeper initial comatose/confusional states. Anosmia does not appear to add incrementally to disability status and it does not automatically imply the presence of basal-frontal damage.

Subclinical status epilepticus in a child after closed head injury.

A 14-year-old girl with closed head injury and a normal computerized tomographic scan underwent an electroencephalographic tracing that surprisingly revealed typical status epilepticus electrical activity. No episodes of motor clinical convulsions were observed from the moment of trauma throughout the admission period. Treatment with phenytoin caused a dramatic clinical improvement. Repeated electroencephalogram (EEG) 4 days later was within normal limits. Posttraumatic seizures are reported after head injury, yet, the issue of "invisible" or "subclinical" seizures associated with trauma is no

Relative risk of deterioration after mild closed head injury.

In this prospective study, a series of 1812 consecutive mild head injured adult patients who visited the hospital emergency department were assessed. Twenty-eight patients (1.5%) deteriorated after head injury; 23 of these (1.3% of the series) required surgical intervention. Five patients (0.3%) deteriorated due to non-surgical causes [post-traumatic seizure 2, syndrome of inappropriate secretion of antidiuretic hormone (SIADH) 3]. Most of the deterioration occurred within the first 24 hours (57%). Post-traumatic headache was found in 280 patients (15.5%) and 84 patients (4.6%) suffered post-t

Treating a case of surface dyslexia after closed head injury.

We describe an original treatment program for surface dyslexia in a patient with a closed head injury. Addressing the question of whether the deficient lexical reading routes could be improved in this type of case. we designed three tasks and administered them over ten consecutive treatment sessions. Results indicate improved accuracy of responses as a function of presentation times or response times, or both, on all treatment tasks. Comparisons of standardized reading comprehension and oral reading tests before and after therapy disclosed increases in reading rate, with the patient's original
