[Cervicogenic headache].

In 2004 cervicogenic headache was introduced into ICD-10 classification. The reasons of cervicogenic headache are changes within bones, soft tissue and nervous structures of cervical spine section. The pain may spread to the neck, occipital area of skull, area of jaw and eyeballs, and arms. There are many theories trying to explain spreading of the pain outside the area innervated by C1, C2 and C3 cervical roots. Their common denominator is communication between fibres running in those roots and neurons of trigeminal nerve. Many authors describe a possibility of such connection through the jel…

Thunderclap headache.

Thunderclap headache (TCH) is head pain that begins suddenly and is severe at onset. TCH might be the first sign of subarachnoid haemorrhage, unruptured intracranial aneurysm, cerebral venous sinus thrombosis, cervical artery dissection, acute hypertensive crisis, spontaneous intracranial hypotension, ischaemic stroke, retroclival haematoma, pituitary apoplexy, third ventricle colloid cyst, and intracranial infection. Primary thunderclap headache is diagnosed when no underlying cause is discovered. Patients with TCH who have evidence of reversible, segmental, cerebral vasoconstriction of circl…

Cerviogenic headache.

Cervicogenic headache has historically been misdiagnosed, ultimately leading to misguided therapies. Even with the growing body of literature in the field of headache, this particular entity is still vastly understudied. Efforts should be made to encourage appropriate clinical studies in order to provide evidence based therapeutic modalities for this patient population. The dental professional is provided with the unique opportunity to see these patients on a regular basis without the time constraints now shouldered by our physician colleagues. The goal of any health care provider should be to…

Diagnosing headache.

BACKGROUND: A systematic approach to the diagnosis of primary and secondary headache disorders requires the measurement of the frequency and the duration of headache. OBJECTIVE: This article presents thumbnail sketches of the most important headache subtypes and discusses some of the new revisions and headache types included in the International Classification of Headache Disorders (ICHD 2). DISCUSSION: The clinical features that distinguish secondary headaches requiring urgent investigation (red flags) from those that can be more safely monitored (blue flags) are discussed.

[Cluster headache].

Cluster headaches are characterized by attacks of strictly unilateral severe pain which is orbital, supraorbital, temporal or in any combination of these sites associated with one or more of the following, ipsilateral conjunctival injection, lacrimation, nasal congestion, rhinorrhea, forehead and facial sweating, miosis, ptosis, and eyelid edema. Most patients are usually unable to lie down, restless or agitated during an attack. The attacks last 15-180 minutes and occur from once every other day to 8 times a day. Cluster periods usually last for weeks or months separated by remission periods,…

Headache diagnosis.

Headache is an extremely common symptom in primary care practice. Despite the ubiquity of the pain, differential diagnosis of headache is not difficult, provided the clinician obtains a comprehensive history. A complete physical examination and specific testing may be required to rule out other underlying causes, but headache itself falls into 3 main classes that are readily identified. Migraine headache occurs most commonly in women, is of moderate to severe intensity, and is often accompanied by nausea and increased sensitivity to light and sound. Cluster headache describes multiple recurren…

Cluster headache.

Cluster headache is characterized by regular periodicity, high frequency during a cluster period, relative brevity, and extreme intensity. Lancinations, as in trigeminal neuralgia, are rare. An important behavioral difference between migraine and cluster headache is that the patient is usually hyperactive during a cluster headache, whereas the migraineur retreats to a dark, quiet room. Cluster headache is more common in middle-aged men; migraine is more common in young women. Intermediate or overlap syndromes occur. Many of the same prophylactic and abortive treatments are effective in both, a…

[The characteristics of sinus headache resembling the primary headaches].

It has to be excluded organic lesions to diagnose the primary headache, however they are tended to be misdiagnosed in a routine practice. Acute sinusitis is the most common disease to be misdiagnosed as the primary headaches and we have reported that the characteristics of sinus headache have closely resembled migraine, cluster headache or tension type headache. The effectiveness of triptans does not become an evidence for a diagnosis of migraine or cluster headache, because it was also effective for the pain of the acute sinusitis. The etiology of sinus headache that resembles the primary hea…
