Linking epidemiological and genomic data in cases of enteric fever in England to inform clinical management and public health action.

OBJECTIVES: To explore the feasibility of linking data from enhanced surveillance patient questionnaires from each enteric fever case in England with genome sequencing data, including antimicrobial resistance (AMR) profiles, from the corresponding isolate of typhoidal salmonellae. METHODS: After linking data we interrogated the merged dataset and assessed the utility of passive surveillance data to match and monitor antimicrobial treatment regimens in enteric fever patients with the AMR profiles of the infectious agent. RESULTS: A high proportion of cases were given antibiotics (n = 1230/141

Beliefs about medications when treating hypertension in primary health care: results from "PERson-centredness in hypertension management using information Technology (PERHIT)".

PURPOSE: Hypertension is a major global health concern. Despite of efficient antihypertensive medications a low percentage of patients reach a blood pressure (BP) of <140/90. Nonadherence is a great concern in hypertension treatment and patients' beliefs about medications has been shown to have a strong impact on adherence. The objective of this study is to examine beliefs about medications and its impact on BP treatment in a group of Swedish primary healthcare patients treated for hypertension with or without an E-health platform. MATERIALS AND METHOD: In a randomised unblinded controlled tr

Non-Surgical Management of People With Frozen Shoulder in the National Health Service: A Review of Publicly Available Patient Information Leaflets.

BACKGROUND: Frozen shoulder, also known as adhesive capsulitis, is a common and disabling condition that causes shoulder pain and progressive stiffness. Patient information leaflets (PILs) are produced by UK National Health Service (NHS) Trusts to help patients understand frozen shoulder and treatment options. However, the content and consistency of these PILs and their alignment with national clinical guidance are currently unclear. OBJECTIVES: This study aimed to identify, analyse and describe the non-surgical management recommendations presented in publicly available NHS Trust PILs for fro

Implementation of an electronic health information system using DHIS2 tracker to manage and evaluate the National cervical screening programme in Bangladesh.

BACKGROUND: The cervical screening programme in Bangladesh upgraded its electronic health information system (e-HIS) in 2019 using DHIS2 tracker application. The upgraded e-HIS collects individual level data and has useful functionalities like sending SMS reminders. Also, the system facilitates data linkage between community clinics, VIA (visual inspection with acetic acid) screening centers, and colposcopy clinics using a unique national identifier. Our present mixed-method study aimed to perform an in-depth assessment of functioning of the new s-HIS and recommend measures to overcome the imp

Prevention and management of osteoporotic fractures by non-physician health professionals: a systematic literature review to inform EULAR points to consider.

OBJECTIVE: To perform a systematic literature review (SLR) about the effect of non-pharmacological interventions delivered by non-physician health professionals to prevent and manage osteoporotic fractures. METHODS: Eight clinical questions based on two criteria guided the SLR: (1) adults≥50 years at high risk of osteoporotic fracture and (2) interventions delivered by non-physician health professionals to prevent and manage osteoporotic fractures. Interventions focused on diagnostic procedures to identify risk of falling, therapeutic approaches and implementation strategies. Outcomes include

Designing Consumer Health Information Technology to Support Biform and Articulation Work: A Qualitative Study of Diet and Nutrition Management as Patient Work.

BACKGROUND: Diet and nutrition management is an integral component of Crohn disease (CD) management. This type of management is highly variable and individualized and, thus, requires personalized approaches. Consumer health information technology (CHIT) designed to support CD management has typically supported this task as everyday life work and, not necessarily, as illness work. Moreover, CHIT has rarely supported the ways in which diet and nutrition management requires coordination between multiple forms of patient work. OBJECTIVE: The purpose of this study was to investigate diet and nutri

Evaluation of the Impact Personality Disorder Project - A psychologically-informed consultation, training and mental health collaboration approach to probation offender management.

BACKGROUND: The Camden and Islington Impact Personality Disorder Project provided psychologically informed consultation and training to probation staff in a Public Protection Unit and staff in two 'approved premises' (probation hostels) working with high-risk prison sentenced offenders on licence in the community; some direct therapeutic work was also conducted jointly with probation officers. Most of the offenders had behaviours associated with personality disorder, although had not necessarily received a diagnosis of personality disorder. AIMS: To evaluate this service (the intervention) an

Exploring the relationship between self-management behaviour, family function and health information adoption behaviour in Chinese diabetic foot patients: a mixed-methods study protocol.

INTRODUCTION: Diabetic foot is a major burden and threat to individuals, families and society, making it imperative to promote good self-management behaviour. However, although nurses have provided these patients with excellent health knowledge, their self-management remains unsatisfactory. Although researches have shown that self-management requires family involvement, no research has been conducted in China on family function, specifically in the diabetic foot. Therefore, this study aimed to explore the relationship between self-management, family functioning, and health information adoption

A mixed methods study to inform and evaluate a longitudinal nurse practitioner/community health worker intervention to address social determinants of health and chronic obstructive pulmonary disease self-management.

BACKGROUND: Individuals with low socioeconomic status experience higher prevalence and worse outcomes of chronic obstructive pulmonary disease (COPD). We undertook a quality improvement initiative at our safety net hospital in which a nurse practitioner (NP)/community health worker (CHW) team followed patients with COPD, frequent admissions, and unmet SDOH needs from hospitalization through one month post-discharge. We report our mixed methods approach to inform development and preliminary evaluation of this intervention. METHODS: We first assessed characteristics of patients admitted with CO

Evidence-informed prioritization of interventions for integrating hypertension management into cardiovascular-kidney-metabolic care in primary health care: HEARTS 2.0 Phase 3.

BACKGROUND: HEARTS in the Americas is the regional adaptation of the WHO Global HEARTS Initiative. This paper presents Phase 3 of HEARTS 2.0, an evidence-informed process to prioritize clinical and health-system interventions to support the operationalization of integrated cardiovascular, kidney, and metabolic (CKM) care in primary health care (PHC) and to inform the update of the HEARTS Clinical Pathway. CKM conditions frequently coexist, sharing common risk factors and pathophysiological mechanisms that exacerbate cardiovascular disease burden. Despite their overlap, clinical practice often

Utilizing health information technology to improve the recognition and management of life-threatening adrenal crisis in the pediatric emergency department: medical alert identification in the 21st century.

Background Many barriers exist to the appropriate recognition and management of life-threatening adrenal crisis in the emergency department (ED). Clinical decision support (CDS) is a health information technology (IT) component that provides useful information to providers as healthcare is being delivered. We hypothesized that CDS incorporated into the electronic health record (EHR) could improve the recognition and management of adrenal crisis within the pediatric ED. Methods We retrospectively analyzed the impact of electronic CDS on the management of patients with known adrenal insufficienc

Family Health Literacy, Family Functioning, and Type 2 Diabetes Self-Management in the Context of Informal Caregiving: An Embedded Mixed-Methods Multiple-Case Study.

Background/Objectives: Type 2 diabetes is a prevalent chronic disease that requires continuous self-management and adaptation by both individuals and their families. Health literacy is recognised as an important determinant of chronic disease management, influencing the ability to access, understand, and use health information. This study aimed to explore how family health literacy and family functioning were related to type 2 diabetes self-management across four family cases in the context of informal caregiving and to describe the changes observed following tailored family nursing interventi
