Age-Related Hearing Loss.

Age-related hearing loss (ARHL) is the most prevalent sensory deficit in the elderly. This progressive hearing impairment leads to social isolation and is also associated with comorbidities, such as frailty, falls, and late-onset depression. Moreover, there is a growing evidence linking it with cognitive decline and increased risk of dementia. Given the large social and welfare burden that results from ARHL, and because ARHL is potentially a modifiable risk factor for dementia, there is an urgent need for therapeutic interventions to ameliorate age-related auditory decline. However, a prerequi…

Blast-induced hearing loss.

The incidence of blast injury has increased recently. As the ear is the organ most sensitive to blast overpressure, the most frequent injuries seen after blast exposure are those affecting the ear. Blast overpressure affecting the ear results in sensorineural hearing loss, which is untreatable and often associated with a decline in the quality of life. Here, we review recent cases of blast-induced hearing dysfunction. The tympanic membrane is particularly sensitive to blast pressure waves, since such waves exert forces mainly at air-tissue interfaces within the body. However, treatment of tymp…

Vertigo and hearing loss.

Symptoms referable to disorders affecting the inner ear and vestibulocochlear nerve (eighth cranial nerve) include dizziness, vertigo, tinnitus, and hearing loss, in various combinations. Similar symptoms may occur with involvement of the central nervous system, principally the brainstem and cerebellum, to which the vestibular and auditory systems are connected. Imaging choices should be tailored to patient symptoms and the clinical context. Computed tomography (CT) should be used primarily to assess bony structures. Magnetic resonance imaging (MRI) should be used primarily to assess soft-tiss…

Sudden sensorineural hearing loss.

Sudden onset sensorineural hearing loss is a medical emergency that continues to be poorly understood despite being recognized in the literature since 1944 (De Kleyn, 1944). A commonly used criterion to qualify for this diagnosis is a sensorineural hearing loss over three contiguous pure-tone frequencies of 30 dB or more that develops within 72 hours. The vast majority of cases are unilateral and the estimated annual incidence is 20 per 100 000 persons (Nosrati-Zarenoe et al, 2007). A cause for the hearing loss is only identified in up to 10% of cases but 50% of patients will improve spontaneo…

Sudden sensorineural hearing loss.

Sudden sensorineural hearing loss is usually unilateral and can be associated with tinnitus and vertigo. In most cases the cause is not identified, although various infective, vascular, and immune causes have been proposed. A careful examination is needed to exclude life threatening or treatable causes such as vascular events and malignant diseases, and patients should be referred urgently for further assessment. About half of patients completely recover, usually in about 2 weeks. Many treatments are used, including corticosteroids, antiviral drugs, and vasoactive and oxygen-based treatments. …

Imaging of hearing loss.

In this article, we review the common entities causing the hearing loss and divide these pathologies into conductive hearing loss, sensorineural hearing loss, and mixed hearing loss. With this format, congenital pathologies are addressed first followed by the acquired entities. We review the common pathologies and the imaging findings affecting the hearing pathways from outside to inside, starting at the external auditory canal and moving inward toward the midbrain.

[Hearing loss in adults].

The management of hearing loss in adults depends of etiology and its severity. It can be as simple as treating an external otitis, removing an impacted cerumen or a more complex one such as a surgery for otosclerosis. The hearing loss is managed mainly by new advances in hearing aids technology and implantable hearing devices which include BAHA, middle ear implant and cochlear implants. The research is focused on developing new molecules for intracochlear drug therapy to treat noise induced hearing loss, drug ototoxicity as well as hearing loss related to cochlear implant insertion trauma. Ant…

Syphilis causing hearing loss.

An HIV-positive man with hepatitis B co-infection, naïve to highly active antiretroviral therapy, with a CD4 of 594 copies/mL and HIV-1 viral load of 140,070 copies, presented with right-sided facial weakness and hearing loss. He had been treated for secondary syphilis three months earlier when his rapid plasma reagin (RPR) result was 1:16, this had fallen to neat. At presentation, his RPR had risen to 1:16 again. A magnetic resonance imaging scan showed enhancement of the internal auditory canal and right cochlea. His cerebrospinal fluid examination was normal. He was treated with acyclovir a…

Imaging of hearing loss.

A wide range of pathology involving the external, middle, and inner ear contribute to conductive and sensorineural hearing loss. Diagnostic imaging plays a critical role in the evaluation and management of hearing loss. High resolution computed tomography (CT) of the temporal bone and magnetic resonance imaging (MRI) are the preferred imaging modalities to evaluate the ear structures for causes of hearing loss, with the specific type of hearing loss and location of defect dictating which type of imaging is preferred. In general, the external auditory canal, middle ear space, mastoid, petrous a…

Hearing loss in aging.

UNLABELLED: Aging is a natural consequence of a society developing process. The city of São Paulo has almost one million people who are above sixty years of age. Age-related hearing loss equals the total hearing loss resulted from cell degeneration caused by noise exposure, ototoxic agents and the loss caused by disorders and medical treatments. AIM: To study age-related hearing degeneration by means of higher thresholds and hearing sensitivity measures. MATERIALS AND METHODS: Cross-sectional contemporary cohort study in which we assessed 211 elderly patients with mean age of 75.24 years, of…

Age-related hearing loss.

Age-related hearing loss or presbycusis is hearing loss that progressively worsens with age. With the expected increase in the number of elderly in the United States, the problem is anticipated to be increasingly common, and the impact widely felt in coming years. This article discusses the causes and mechanisms of this type of hearing loss and current research that may lead to new treatments.

Noise-induced hearing loss.

Hearing loss caused by exposure to recreational and occupational noise results in devastating disability that is virtually 100 percent preventable. Noise-induced hearing loss is the second most common form of sensorineural hearing deficit, after presbycusis (age-related hearing loss). Shearing forces caused by any sound have an impact on the stereocilia of the hair cells of the basilar membrane of the cochlea; when excessive, these forces can cause cell death. Avoiding noise exposure stops further progression of the damage. Noise-induced hearing loss can be prevented by avoiding excessive nois…

Autism and hearing loss.

A group of 199 children and adolescents (153 boys, 46 girls) with autistic disorder was audiologically evaluated. Mild to moderate hearing loss was diagnosed in 7.9% and unilateral hearing loss in 1.6% of those who could be tested appropriately. Pronounced to profound bilateral hearing loss or deafness was diagnosed in 3.5% of all cases, representing a prevalence considerably above that in the general population and comparable to the prevalence found in populations with mental retardation. Hearing deficits in autism occurred at similar rates at all levels of intellectual functioning, so it doe…

Hearing loss and motorcyclists.

Motorcyclists are known to be exposed to excessive wind noise levels when riding. The potential adverse effects of this exposure on their he … ignificant TTS was found at 0.25, 0.5, 1 and 2 kHz. The greatest TTS occurred at 1 kHz, with a mean hearing loss of 10.3 dB. The hearing thresholds of the motorcyclists were significantly worse than the controls at 0.25, 0.5, 1 and 2 kHz, and was most marked at 0.5 and 1 kHz where their hearing loss (PTS) was, respectively, 3.7 and 3.6 dB greater than expected …

Hearing loss in diabetics.

The relationship between diabetes and sensorineural hearing loss has been disputed. This study compares 44 insulin-dependent diabetics with 38 age and sex matched controls. All had pure tone and speech audiometry performed, with any diabetic showing sensorineural deafness undergoing stapedial reflex decay tests. In 14 diabetics stapedial reflex tests showed no tone decay in any patient, but seven showed evidence of recruitment. Analysis of variance showed the diabetics to be significantly deafer than the control population. The hearing loss affected high frequencies in both sexes, but also low…

Treatable sensorineural hearing loss.

Sensorineural hearing loss is generally felt to be an untreatable medical condition. However, in some cases, prompt diagnosis and treatment of the underlying condition may reverse the deafness. This article summarizes various treatable forms of sensorineural hearing loss and provides illustrative cases histories of patients who have had sensorineural hearing losses that were improved by medical or surgical intervention. Patients with reversible sensorineural deafness due to inadvertent aminoglycoside over-dosage, congenital cholesteatoma, Meniere's syndrome, blood coagulopathy, and perilymphat…
