Hemoglobin C (Hb C) disease is caused by a mutation that leads to reduced hemoglobin solubility, crystallization, and chronic hemolysis. We report on a patient with a homozygous Hb C disease, persistent fetal hemoglobin, and a heterozygous factor II mutation. This case illustrates the complex interaction between hemoglobinopathies and thrombophilia disorders, where the coexistence of Hb C disease and a factor II mutation significantly heightens the risk of thrombosis. Its geographic prevalence in the Caribbean, particularly Puerto Rico, reflects historical migration patterns and underscores th…

INTRODUCTION: Anemia of chronic inflammation is associated with many inflammatory diseases. Little is known about anemia in hidradenitis suppurativa (HS). This study aimed to review the levels of hemoglobin (Hb) and investigate its relationship with serum C-reactive protein (CRP) and disease severity in HS patients.
METHODS: This was a retrospective chart review of all HS patients from 2015 to 2017 with Hb and CRP blood work. Patient demographics, disease severity, and laboratory results were extracted. Data were analyzed descriptively. A linear regression model was used for the association b

Hepatitis C clearance with directly acting antivirals (DAAs) may be associated with acute decreases in hemoglobin A1c (HbA1c). We prospectively evaluated 251 chronic hepatitis C virus (HCV)-infected subjects (31% human immunodeficiency virus [HIV] positive) pre- and post-DAA therapy (median follow-up 28 months). Changes in HbA1c and glucose were minimal and did not differ by sustained virologic response (SVR), HIV, diabetes, or fibrosis. Following SVR, mean change in HbA1c was -0.022 ± 0.53%; however, total and low-density lipoprotein cholesterol increased significantly. Subjects with HIV had

OBJECTIVE: A number of studies, including the Diabetes Control and Complications Trial (DCCT), have shown that good glycemic control, as assessed by GHb measurements, can reduce the chronic complications of diabetes. The National Glycohemoglobin Standardization Program (NGSP) was established to insure that GHb measurements by different methods were comparable and could be related to the candidate reference method used in the DCCT. The measurement of HbA1c in patients with Hb variants is one area not directly addressed by the NGSP. Therefore, we assessed the comparability of two DCCT-traceable

Of 148 adults with sickle cell hemoglobin C (SC) disease seen at Cook County Hospital and Clinics, Chicago, Illinois, eight patients had coexistent noninsulin-dependent diabetes mellitus. The clinical findings were notable for the paucity of retinal vascular changes. No patient showed proliferative diabetic retinopathy; one patient showed background diabetic retinopathy consisting of a few microaneurysms. Six of the eight patients showed no lesions of proliferative sickle retinopathy. Coexistence of noninsulin-dependent diabetes mellitus and SC disease does not appear to have an additive adver…

Several nutritional surveys have shown that African-American children have lower mean hematocrit and hemoglobin values than white children. While the high prevalence of both alpha and beta thalassemias in African-American children can partly account for the lower hematologic values, the remaining differences cannot be explained. African-American individuals have a high prevalence of traits for hemoglobin S (HbAS) and hemoglobin C (HbAC). The mean hematocrit of children with normal hemoglobin AA (HbAA) (n = 15637) was compared with that of children with traits for HbAS (n = 1197) and HbAC (n = …

Elevated hemoglobin (Hb) concentration, erythrocytosis, is a common phenomenon that can be caused by primary or secondary underlying conditions. In this report, we describe a previously unknown mutation in the β globin gene resulting in high Hb concentrations combined with elevated reticulocytes in two family members spanning two generations. In the index patient the variant was first noted during HbA1c monitoring. While both ion-exchange chromatograms used for HbA1c analysis and isoelectric focusing indicated the presence of a β globin variant, the chromatographic method used for hemoglobinop

We report a patient with hemoglobin sickle cell-hemoglobin C disease who developed the clinical syndrome of thrombotic thrombocytopenic purpura (TTP) during admission for typical acute pain crisis. The potential for multiorgan involvement secondary to vaso-occlusive crisis complicated the diagnosis and overlapped with the patient's clinical presentation of chronic bone pain and hemolytic anemia. Clinical improvement and normalization of laboratory parameters followed rapidly in response to plasma exchange therapy.

BACKGROUND: Type 2 diabetes mellitus (T2DM), driven by insulin resistance and β cell dysfunction, necessitates reliable assessment of β cell function. C-peptide (CP) measurement, a stable marker of endogenous insulin secretion, is useful for this clinically as it avoids interference from exogenous insulin. While fasting CP (FCP) and postprandial CP (PCP), along with glucose-adjusted indices and ratios, such as FCP/fasting plasma glucose (FPG), 2 hours postprandial CP (2hCP)/postprandial blood glucose (PBG) and CP ratio, are used, their comparative efficacy in reflecting β cell function remains

BACKGROUND: It has been reported that mineralocorticoid receptor antagonist improves the prognosis of chronic heart failure (CHF). Recently, hemoglobin A₁(c) (HbA₁(c)) levels have been reported to be an independent risk factor for mortality in CHF, suggesting the important role of insulin resistance in CHF. We compared the metabolic effect of a selective mineralocorticoid receptor blocker eplerenone with spironolactone in CHF patients.
METHODS: One hundred seven stable outpatients with mild CHF, who were already receiving standard therapy for CHF, were randomized (1:2) to spironolactone (25 m

A 31-year-old black diabetic man had acute oliguric renal failure. Retrograde pyelography showed filling defects in the ureters, presumably blood clots. Essential thrombocythemia was subsequently diagnosed. We believe no such presentation of essential thrombocythemia has been previously described in the English literature.

Hb Dompierre [β29(B11)Gly→Arg, HBB: c.88G>C] is a rare β-globin gene variant that was previously described in the heterozygous state in a 24-year-old female patient. It is defined in the HbVar database as being clinically and biologically asymptomatic. A few years after the first description, we had an opportunity of reassessing the index case because she presented with splenomegaly and clinical and biological manifestations of hemolysis. After ruling out the most common causes of hemolysis, further analyses on the variant hemoglobin (Hb) using brilliant cresyl blue staining, indicated that it

Acute splenic sequestration crisis (ASSC) is a potentially life-threatening complication and one of the leading causes of death in children with sickle cell disease. It is rarely reported in adults with hemoglobin S-C disease and its natural history in these patients has not been well studied. We describe here the clinicopathological features of ASSC in nine adults with hemoglobin S-C disease treated between 1972 and 2000 and followed for a mean period of 9 years (range 0-21 years). ASSC was characterized by acute left upper quadrant abdominal pain, splenomegaly, fever, and a rapid decrease in

OBJECTIVE: Glycated hemoglobin (HbA₁(c)), used to monitor and diagnose diabetes, is influenced by average glycemia over a 2- to 3-month period. Genetic factors affecting expression, turnover, and abnormal glycation of hemoglobin could also be associated with increased levels of HbA₁(c). We aimed to identify such genetic factors and investigate the extent to which they influence diabetes classification based on HbA₁(c) levels.
RESEARCH DESIGN AND METHODS: We studied associations with HbA₁(c) in up to 46,368 nondiabetic adults of European descent from 23 genome-wide association studies (GWAS) a…

The poor prognosis of rheumatoid arthritis (RA) and its potential for complications highlight the importance of understanding factors that are associated with incidence and mortality rates. The inclusion criteria of this study were RA-related data from 1999 to 2018 in the National Health and Nutrition Examination Survey (NHANES) dataset. Based on certain screening criteria, a total of 610 subjects were recruited. The Low CRP group (< 3 mg/L) and the High CRP group (> 3 mg/L) were significantly different in gender, poverty-to-income ratio, body mass index, hypertension, hemoglobin (Hb), hematoc

BACKGROUND: Diabetes mellitus (DM) is a disorder of the endocrine-metabolic system, identified by ongoing hyperglycemia and exacerbated by liver function problems. The association of the glycated albumin/hemoglobin A1c (GA/HbA1C) ratio and C-peptide (C-PEP) with liver injury in diabetes mellitus type 2 (T2DM) patients remains a topic of controversy.
METHODS: 530 T2DM patients were divided into two groups: non-liver injury and liver injury group. All patients' clinical information and lab indices were gathered. In the univariate analysis, variables having a p < 0.05 were evaluated to determine

We report a clinical update of the hemoglobin (Hb) variant [β27(B9)Ala→Gly; HBB: c.83C>G], named Hb Siirt, that was previously described as a silent variant in a 23-year-old Kurdish female. The patient was also a carrier of the codon 5 (-CT) (HBB: c.17_18delCT) frameshift mutation and of the ααα anti 3.7 triplication. Her initial moderate β-thalassemia intermedia (β-TI) phenotype worsened with time, causing the patient to become a transfusion-dependent subject at the age of ∼40 years. Subsequent molecular characterization of both parents revealed that the Hb Siirt variant was inherited by the

Sickle cell disease (SCD) is an inherited hemoglobinopathy in which deformed erythrocytes impair microvascular blood flow, leading to complications across multiple organ systems. Variants of SCD exist, and a less common form involves the inheritance of one hemoglobin S allele and one hemoglobin C allele, manifesting as sickle cell-hemoglobin C (HbSC) disease. Although HbSC disease can cause significant sequelae, a rare but serious complication is hemorrhagic stroke. In such cases, chronic endothelial injury, vascular fragility, and abnormal hemodynamics associated with this hemoglobinopathy ma…

Sickle cell disease (SCD) is a complex disease that is characterized by the polymerization of deoxyhemoglobin S, altered red blood cell membrane biology, endothelial activation, hemolysis, a procoagulant state, acute and chronic inflammation, and vaso-occlusion. Among the physiological changes that occur during pregnancy, oxygen is consumed by fetal growth, and pregnant women with SCD are more frequently exposed to low oxygen levels. This might lead to red blood cells sickling, and, consequently, to vaso-occlusion. The mechanisms by which SCD affects placental physiology are largely unknown, a
