The authors report a complicated leiomyosarcoma gastric case with hemoperitoneum. The exploratory laparotomy reveals an hemoperitoneum constituted from both gastric tumors. One was localised in the antrum and was encapsulated, the other in the cardia and had an encephaloid and necrotized core. The pathological examination of the total gastrectomy piece is consistent with a double gastric localisation of a leiomyosarcomosis. 5 years after the initial operation, the patient developed hepatic metastases. A review of literature allows the authors to make the point of this disease.

Brazil has an average of 27,000 cases per year, with a fatality rate of 0.5%, and the Bothrops genus is the most common causative agent, accounting for about 70-90% of the accidents. Isolated splenic peliosis is an exceedingly rare condition of unclear etiology characterized by blood filled cyst-like cavities in the primary mononuclear phagocytic organ. People with splenic peliosis are typically asymptomatic. However, splenic peliosis may present as an incidental mass on imaging mimicking a neoplasm during life, or present as sudden death after hemoperitoenum when ruptured, mimicking traumatic injury. Awareness of this condition is important to forensic pathologists so as to provide an accurate cause and manner of death.

The arc of Buhler is a persistent connection between the celiac trunk and the superior mesenteric artery, which is an embryogenic remnant. This case report details the series of events that led to the discovery of an arc of Buhler after a patient was found to have significant hemoperitoneum secondary to the rupture of this pathology. Portal hypertension, responsible for the formation of oesophageal varices, also generates intra-abdominal varicose dilations, especially of the perisplenic and mesenteric veins, which, like the oesophageal veins, are susceptible to rupturing and bleeding, in this case within the peritoneal cavity. However, the spontaneous rupture of these intraperitoneal varices is a rare complication, and poorly described in the literature. We present the case of a 72-year-old woman with CHILD B liver cirrhosis of unknown aetiology with portal hypertension on primary prophylaxis with carvedilol.

AIM: The purpose of this study was to evaluate the current state and clinical characteristics of spontaneous hemoperitoneum in pregnancy (SHiP) in Japan by performing a comprehensive survey.
METHODS: We reviewed data on pregnant women who developed SHiP during 2013-2017 (for 5 years), and were admitted to any of the perinatal centers in Japan. Postpartum haemorrhage is a major cause of maternal morbidity and mortality worldwide. Early dia-gnosis and treatment are essential to prevent sequelae or even death. A young woman presented to the emergency department with lethargy, hemodynamic instability, and diffuse abdominal tenderness. On point-of-care ultrasound (PoCUS), she was found to have intraperitoneal free fluid and a large pelvic mass, which were discovered intraoperatively to be hemoperitoneum due to ruptured vessels of a uterine leiomyoma. Although rare, a life-threatening, ruptured leiomyoma may be treated surgically if recognized in an expedient fashion. A PoCUS can aid the emergency clinician in prompt diagnosis.
