A new hernia: Meckel's diverticulum within a parastomal hernia.

A Meckel's diverticulum is a true diverticulum containing all layers of bowel wall. An infrequently encountered sequela of this is a Littre's hernia, typically inguinal or femoral. This is the first description of a Meckel's diverticulum within a parastomal hernia. When encountered incidentally at surgery, resection should be considered on a case-by-case basis to prevent future complications of bleeding, obstruction or perforation.

Another type of diaphragmatic hernia to remember: parahiatal hernia.

BACKGROUND: Parahiatal hernia is a less common form of diaphragmatic hernia, with the defect lateral to the crus and oesophageal hiatus, and the unfamiliarity of this disease would cause confusion to general surgeons. In the present study, the literature on parahiatal hernia were reviewed, the clinical profile of this disease, as well as our own experience is presented and analysed. METHODS: In the present study, a thorough identification of all published reports on parahiatal hernias was made, together with our own cases, the available data were summarized, analysed and discussed. RESULTS: …

Traumatic abdominal wall hernia - a case of handlebar hernia.

Traumatic abdominal wall hernia (TAWH) after blunt injury is uncommon. Diagnosis requires careful examination and high index of suspicion. We report a case of a 12-year-old boy who complained of painful abdominal swelling over the left iliac fossa after a bicycle-handlebar hit his abdomen. TAWH was diagnosed clinically and confirmed with ultrasound and computed tomography (CT) scan. He developed incarceration after 12 hours of admission and subsequently underwent primary repair without mesh. As TAWH is usually associated with other concomitant injuries, it is important that we are meticulous t…

["Handlebar" hernia: a rare type of traumatic parietal hernia].

We report the case of a 20-year old patient with a personal history of fall over his motorcycle handlebar occurring 28 days earlier. He was admitted in emergency surgery with abdominal pain. Clinical examination showed a circular impact area at the level of the left hypochondrium associated with painful swelling, irreducible and with no impulse on coughing The diagnosis of traumatic parietal strangulated hernia was established. The patient underwent surgical treatment using midline laparotomy revealing parietal breach associated with incarceration of a portion of the omentum which was necrotic…

Handlebar hernia: a rare type of abdominal wall hernia.

Traumatic abdominal wall hernias are a type of acquired hernia secondary to blunt trauma Caused, by direct trauma from handlebar like objects. This rare hernia is named 'Handlebar hernia'. We report a case of such hernia without any significant intra-abdominal injury. The abdominal wall defect was repaired in layers by Jones technique. Postoperative course was uneventful. The authors recommend clinical suspicion for traumatic hernia in all patients with traumatic abdominal wall injury. Definitive treatment includes surgical exploration with primary repair of all tissue layers of the abdominal …

Morgagni hernia: a rare form of congenital diaphragmatic hernia.

A congenital diaphragmatic hernia (CDH) occurs in approximately 1 per 2000 pregnancies. Most CDHs that are recognized in utero are of the Bochdalek type, resulting from a posterior defect in the diaphragm. Morgagni hernias result from an anterior defect and are the least common type of CDH (5%). They are rarely seen in utero. Furthermore, when they are seen in utero, they may be mischaracterized. We present a case of an unusual type of Morgagni hernia diagnosed in utero. The type of Morgagni hernia reported here is particularly uncommon. Two cases with similar findings have been reported previ…

Amyand's Hernia: Perforated Appendix in an Incarcerated Inguinal Hernia.

Amyand's hernia is an unusual condition characterized by the presence of a normal or inflamed appendix located within an inguinal hernia. We present a rare situation wherein a 56-year-old male patient presented with an incarcerated inflamed appendix in a right inguinal hernia. He was emergently taken to the operating room, with diagnostic laparoscopy changed to open, due to incarcerated cecum and terminal ileum. The incarcerated segment had to be resected with primary anastomosis. The inflamed and purulent contents were washed out, and the hernia defect was left unrepaired due to the presence …

An unusually large paraesophageal hernia mimicking a Bochdalek hernia.

Hiatal hernias are due to defects in the esophageal hiatus in the diaphragm and can be classified into sliding or paraesophageal hernias. A 31-year-old male raised a suspicion of a Bochdalek hernia but at surgery had a large paraesophageal hernia. Bochdalek hernia, a congenital diaphragmatic hernia presents in adulthood asymptomatically or with vague abdominal symptoms. It is paramount to confirm the diagnosis and rule out any fatal complications with imaging studies. Prompt surgical management with large complicated hernias, such as in our case presentation would ensure the most favorable out…

Two cases of paraduodenal hernia, a rare internal hernia.

Paraduodenal hernia is a rare congenital internal hernia which arises from an error of rotation of the midgut with entrapment of the small intestine beneath the developing colon. It is important as it usually presents as intestinal obstruction, and before laparotomy is often misdiagnosed. Mortality increases significantly with delays in surgical treatment. Two cases are presented: an 8 year old boy and a 52 year old man. Both presented with a short history of abdominal pain and an acute abdomen. With prompt surgical treatment, they recovered rapidly.

Lessons learned from an unusual condition of incidental synchronous multiple hernias of indirect inguinal hernia, femoral hernia, and incarcerated obturator hernia.

Hernias are one of the most common diseases. The frequency of multiple hernias is much higher than the frequency reported during herniorrhaphy. Multiple hernias may account for some of the so-called recurrences, in which the small hernia may be veiled by the bigger one and omitted. In this study, we present a case of an 83-year-old female who suffered from multiple hernias namely a left-sided indirect inguinal hernia, a right-sided femoral hernia, and a right-sided incarcerated obturator hernia. Additionally, the characteristics and treatment of obturator hernia were discussed. Ultrasound and …

Two Hernias, One Patient: A Case of Organoaxial Gastric Volvulus in a Paraesophageal Hernia With an Incisional Hernia.

A 65-year-old male presented with abdominal pain and swelling persisting for five years. On clinical examination, a 15 cm transverse scar was noted above the umbilicus, along with a reducible incisional hernia. Contrast-enhanced CT of the abdomen revealed an incisional hernia with a single defect at the umbilical and epigastric regions with European Hernia Society classification of M2-3, W3, along with a rolling-type paraesophageal hernia. He underwent surgical repair for the incisional hernia by hybrid procedure. One month post-discharge, he developed progressive nausea, vomiting, and food in…

[Petersen hernia, a rare type of hernia in our department].

61 years old female with previous surgical history of Roux-en-Y gastric bypass (3 years ago) and earlier hysterectomy admitted to our surgical department with clinical and radiological signs of small intestinal obstruction. Urgent intervention had been performed with following findings: Petersen herniation of alimentary tract including the – biliopancreatic tract and the small bowel extending to the midpart of the terminal ileum. Viability of herniated intestinal tract had been confirmed, and reposition of herniated parts through the Petersen hernia had been done. Closure with non-absorbable r…

Prognostic value of a hernia sac in congenital diaphragmatic hernia.

OBJECTIVE: To investigate the prognostic value of a hernia sac in isolated congenital diaphragmatic hernia (CDH). METHODS: Our database was searched to identify all consecutive cases of CDH referred to our fetal medicine unit between January 2004 and August 2011. Presence or absence of a hernia sac was assessed in liveborn cases using surgery or postnatal autopsy reports. We studied the correlation between the presence of a hernia sac and prenatal findings and perinatal morbidity and mortality. RESULTS: Over the study period, there were 70 cases with isolated CDH born alive in which either a…
