[Chronic hiccups: diagnosis and treatment].

Chronic hiccups, lasting more than 48 hours, are a medical condition that remains challenging in both diagnosis and treatment. They can be the sole symptom of a serious underlying disorder, and should therefore not be overlooked, although gastroesophageal reflux disease (GERD) constitutes their most prevalent cause. Chronic hiccups mandate a comprehensive etiological assessment. Treatment strategy may include physical, pharmacological and interventional approaches, as described in literature, particularly when direct causal treatment is unattainable.

'What puts the 'Hic' into Hiccups?'.

Recurrent and persistent bouts of hiccups impact the quality of life by interfering with eating, social interaction and work. Popular home remedies, such as breath holding and drinking ice water, target activity in the vagal and phrenic nerves that are thought to trigger these repetitive, myoclonic contractions. However, the pathophysiology of hiccups and the mechanism by which any of these methods work are unclear. Indeed, so little is known that there is no agreement whether the 'Hic' sound is due to the abrupt closure of the epiglottis or the glottis, including the vocal cords.Investigation…

Hiccups associated with reflux esophagitis.

A 67-yr-old man with protracted hiccups, as well as reflux esophagitis and a stricture, was found to have heartburn and water brash only during episodes of hiccups. Intraesophageal pH monitoring showed acid gastroesophageal reflux increased during these episodes and decreased with cessation of hiccups. After all attempts failed to relieve his hiccups, a Nissen fundoplication relieved heartburn, healed the esophagitis, and stopped excessive reflux of acid gastric juice, even though the hiccups persisted. These observations showed that reflux provoked by prolonged hiccups could either have adver…

Hiccups: A Non-Systematic Review.

BACKGROUND: Hiccups are a universal phenomenon. They are usually benign and selflimited. Persistent or intractable hiccups, although rare, can be debilitating and may indicate the presence of an underlying pathological process. OBJECTIVE: To familiarize physicians with the pathophysiology, etiology, evaluation, and management of children with hiccups. METHODS: A search was conducted on December 10, 2019, in Pubmed Clinical Queries using the key terms "hiccup" OR "hiccough" OR "singultus". The selected publication types included all clinical trials (including open trials, non-randomized contr…

Hiccups as a myocardial ischemia symptom.

A hiccup is involuntary, paroxysmal inspiratory movements of the chest wall associated with diaphragm and accessory respiratory muscle contractions, with the synchronized closure of glottis. The mechanism underlying this common primitive reflex plays an important role in protecting airways against esophageal aspiration. The hiccup reflex mechanism is based on the afferent pathway (vagus and phrenic nerve and sympathetic fibers innervating chest organs, the abdomen, the ear, the nose and the throat stimulation, and the stimulation of hiccup center in the central nervous system, mainly reflectin…

[Renal cyst inducing hiccups].

INTRODUCTION: Hiccups are a sudden, irregular and spasmodic contraction of the diaphragm followed by an abrupt closure of the glottis. Besides etiologic treatment, its management is above all symptomatic and based primary on medical treatment. CASE: We report a case of chronic hiccups associated with an abscessed voluminous renal cyst. Drainage of the cyst resolved the hiccups. DISCUSSION: Various symptomatic treatments failed because the hiccups were due to irritation of the phrenic nerve by an abscessed renal cyst. This case reminds us that an extensive etiologic work-up may be necessary w…

Possible azithromycin-associated hiccups.

OBJECTIVE: To report a case of persistent hiccups associated by azithromycin therapy. CASE SUMMARY: A 76-year-old man presented with persistent hiccups after beginning azithromycin for the treatment of pharyngitis. Hiccups were persistent and exhausting. Discontinuation of azithromycin and therapy with baclofen finally resolved hiccups. No organic cause of hiccups was identified despite extensive investigation. DISCUSSION: Pharmacotherapeutic agents have been uncommonly associated with hiccups. Corticosteroids (dexamethasone and methylprednisolone), benzodiazepines (midazolam) and general an…

[Hiccup caused by epigastric compression in a hemiplegic patient].

The authors report the case of a patient with permanent, refractory hiccup, resistant at all classical treatment methods. Hiccup was due to compression of the epigastric region by the left fist of the patient immobilised by a spastic hemiplegia. Passive mobilisation of the diseased upper limb resulted in the disappearance of hiccup.

Dopamine Agonist-Associated Hiccup in Parkinson's Disease: A Case Report.

Hiccup is described as the sudden involuntary contraction of the diaphragm, and the intercostal muscles followed by the immediate closure of the glottis. Corticosteroids, benzodiazepines, and antibiotics may cause drug-induced hiccups. Dopamine agonist-induced hiccups were reported in patients diagnosed with Parkinson's Disease (PD) in small number of cases. Here we report a patient diagnosed with PD who had severe hiccups with the use of two dopamine agonists in treatment, however hiccup was not reported with the use of Levodopa. This information may help to manage the treatment of PD, and av…

Intractable hiccup and nausea with periaqueductal lesions in neuromyelitis optica.

Intractable hiccup and nausea (IHN) was found in eight of 47 cases of relapsing neuromyelitis optica (NMO) (17%) but in none of 130 cases of multiple sclerosis (MS). IHN resolved with methylprednisolone. In six cases, MRI detected linear medullary lesions involving the pericanal region, the area postrema, and the nucleus tractus solitarius. Like long and centrally located myelitis, a linear medullary lesion causing IHN may distinguish NMO from MS.

Fatal posterior circulation stroke with persistent hiccups, sinus arrest and post-hiccup syncope: A case report.

RATIONALE: Diagnosis of posterior circulation stoke is difficult, and magnetic resonance imaging especially diffusion-weighted imaging is superior to computed tomography. Persistent hiccups, sinus arrest, and post-hiccup syncope are extremely rare symptoms of posterior circulation stroke. However, there is no effective treatment for persistent hiccup. PATIENT CONCERN AND DIAGNOSIS: We describe a case of a 58-year-old hypertensive woman diagnosed with acute posterior circulation stroke who presented with persistent hiccups, sinus arrest, and post-hiccup syncope. Diffusion-weighted imaging reve…

Hiccup: an extremely rare presentation of thyrotoxicosis of graves' disease.

Persistent hiccup is a rare but potentially severe condition that can be symptomatic of a variety of diseases or idiopathic. Most episodes last only a few minutes and are self-limited, but hiccup can get persistent and become a real problem for physician and patient alike. The center of hiccup may be activated by a great variety of stimuli travelling along different nerve pathways and bring different effecter responses. We report a case of persistent hiccup as a presentation of impending thyroid storm of Graves' disease. Though the condition is rare, clinicians should remain alert to the possi…
