Perianal hidradenitis suppurativa.

The management of hidradenitis suppurativa is individualized according to the site and extent of the disease. Initially, short-term antibiotics, local care, and incision and drainage are helpful. Unroofing of sinus tracks, total disease excision, and vigorous physical therapy are more beneficial. Primary or secondary closure or closure by an advancement flap or selective split-thickness skin grafting should be considered in some refractory cases. In most areas, recurrence rates are high unless wide excisions are performed, but perianal hidradenitis can usually be managed more conservatively. P…

Hidradenitis Suppurativa Epidemiology.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition characterized by painful skin abscesses, fistulizing sinus tracts, and scarring. Despite its devastating negative effects on patients' health, quality of life, performance, and mental health, HS epidemiology is not well understood. This study aims to better characterize the epidemiology of HS, including incidence, prevalence, various demographic distributions, and risk factors. This can further the understanding of HS disease development and improve recognition, diagnosis, and treatment.

Hidradenitis suppurativa: pathogenesis.

Hidradenitis suppurativa (HS) is a complex immune-mediated disorder, initially presenting as folliculitis, and progressively worsening in some patients to advanced lesions, such as fistulas, that may require extensive surgical interventions. In a subset of HS patients, genetic mutations affecting the gamma-secretase complex or keratinocyte dysfunction (pro-inflammatory and hyperproliferative keratinocytes) are implicated. The microbiota is thought to play a crucial role in the development of HS lesions, with dysbiosis observed in both lesional and non-lesional skin, suggesting preclinical alte…

Diagnosing Hidradenitis Suppurativa.

Three diagnostic criteria must be met for hidradenitis suppurativa: typical lesions, occurrence in one or more of the predilection areas, and that it is chronic and/or recurrent. Several outcome measures are used, including patient-reported pain and itch scales, Dermatology Life Quality Index, and Skindex. Hidradenitis suppurativa is associated with significant comorbidities that must be addressed in the evaluation of the patients.

Atypical hidradenitis suppurativa.

Hidradenitis suppurativa (HS) is a chronic relapsing inflammatory disease resulting in subcutaneous abscesses, sinus tracts and scarring, arising predominantly in apocrine gland-bearing skin. The sites affected, in order of frequency, are the axillae, groins, perianal and perineal region, mammary and submammary skin, buttocks, and pubic region. Other sites that may be affected more rarely include the chest, eyelids, scalp, retroauricular and preauricular skin, thighs, and abdomen. Research has suggested that mechanical friction and follicular occlusion is the primary cause of HS. HS is one of …

Hidradenitis suppurativa polyposa.

A case of severe chronic hidradenitis suppurativa of the perineum complicated by disfiguring fibrous, polypoid lesions is presented. The patient, a 41-year-old woman, had a long history of axillary hidradenitis which subsequently involved the perineum. Draining sinuses, scars and large pendulous masses of the vulva developed over 10 years. Cutaneous scars, ridges, papules and large fibrous polyps were present. Deep clefts, sinuses, dense fibrous scars and foci of chronic inflammation were seen. Rarely, large fibrous polyps may develop in chronic hidradenitis suppurativa and may be due to chron…

Idiopathic plantar hidradenitis.

We describe four cases of idiopathic plantar hidradenitis (IPH), a form of neutrophilic eccrine hidradenitis (NEH) localized on the feet of otherwise healthy young persons. Our patients were all males, 8-17 years old, in apparently good health, but presenting erythematous painful papules, plaques and nodules on the soles. They were afebrile and there were no constitutional symptoms. The differential diagnoses included erythema nodosum, vasculitis and traumatic plantar urticaria. The histological features of IPH are similar to those of NEH, except for the absence of syringosquamous metaplasia. …

Peristomal hidradenitis suppurativa.

Hidradenitis suppurativa is a chronic, suppurative, and cicatricial cutaneous disease with clinical manifestations including inflammatory lesions and nodules, abscesses, and sinus tracts. We describe an unusual case of peristomal lesions that may represent hidradenitis suppurativa. Peristomal hidradenitis suppurativa has not been previously reported. This unusual complication demonstrates the need for a thorough patient history to identify any systemic disease that may contribute to peristomal skin problems.

Idiopathic plantar hidradenitis: a neutrophilic eccrine hidradenitis occurring primarily in children.

Neutrophilic eccrine hidradenitis (NEH) has been described in a variety of clinical settings but is most often seen in leukemic patients receiving chemotherapy. We have recently encountered 6 healthy individuals, of whom 5 were children, who developed NEH localized to the feet. The patients were from 9 to 21 years of age. The presenting complaint was rapid development of tenderness of the feet with varying degrees of morbidity. At clinical evaluation, tender, erythematous papules and nodules were confined to the feet, primarily the plantar surfaces. The major differential diagnoses were erythe…

Coexistence of Hidradenitis Suppurativa and Steatocystoma Multiplex: Is It a New Variant of Hidradenitis Suppurativa?

Hidradenitis suppurativa and steatocystoma multiplex may coexist in the same patient. The overlap of these 2 conditions could be suggestive of an unrecognized defect in follicular proliferation mutual in the pathogenesis of both conditions. Here we present 5 patients with both hidradenitis suppurativa and steatocystoma multiplex. Recognizing the overlap between these 2 conditions is important for accurate diagnosis, management, and identification of potential surgical candidates, as well as future basic science research.
