Combined unilateral high frequency jet ventilation and contralateral intermittent positive pressure ventilation.

The anaesthetic management of a patient who required right lower lobectomy for bronchial carcinoma associated with emphysema, pneumoconiosis and a previous thoracoplasty for pulmonary tuberculosis, is described. A technique of unilateral high frequency jet ventilation plus conventional intermittent positive pressure ventilation to the contralateral lung was used.

Efficacy of high-frequency ventilation in presence of extensive ventilation-perfusion mismatch.

Ten anesthetized normal dogs were each given two methacholine inhalational challenges to produce large amounts of low ventilation-perfusion (VA/Q) regions but little shunt. After one challenge, high-frequency ventilation (HFV) was applied, whereas after the other conventional mechanical ventilation (MV) was used, the order being randomized. Levels of both ventilatory modes were selected prior to challenge so as to result in similar and normal mean airway pressures and arterial PCO2 levels during control conditions. Gas exchange was assessed by both respiratory and multiple inert-gas transfer. …

High-frequency oscillatory ventilation with low oscillatory frequency in pulmonary interstitial emphysema.

BACKGROUND: Pulmonary interstitial emphysema (PIE) is a common respiratory illness in preterm infants associated with significant morbidity and mortality for which the ventilatory management is imperfect. OBJECTIVES: To evaluate the impact of high-frequency oscillatory ventilation (HFOV) with a low oscillatory frequency and thus prolonged expiratory time in preterm infants with severe PIE. METHODS: In a retrospective cohort study, preterm infants ≤30 weeks' gestation with radiological findings of severe PIE, and either high FiO2 or persistent respiratory acidosis were studied if managed on H

Should High-Frequency Ventilation in the Adult Be Abandoned?

High-frequency oscillatory ventilation (HFOV) can improve ventilation-perfusion matching without excessive alveolar tidal stretching or collapse-reopening phenomenon. This is an attractive feature in the ventilation of patients with ARDS. However, two recent large multi-center trials of HFOV failed to show benefits in this patient population. The following review addresses whether, in view of these trails, HFOV should be abandoned in the adult population?

A review of high frequency oscillation ventilation in the neonate.

In this study the use of high frequency oscillation (HFO) to treat neonates with respiratory failure is analysed. The theories behind gas exchange during HFO are reviewed and its specific application to neonatal care discussed. The mechanical performance of three HFO ventilators currently in use is compared with the views of medical staff operating them on a regular basis. The complex interactions between initial ventilator settings have led to difficulties in accurately comparing performance characteristics and ventilation strategies; each ventilator is seen to have its own strengths and weak

High-Frequency Positive Pressure Ventilation as Primary Rescue Strategy for Patients with Congenital Diaphragmatic Hernia: A Comparison to High-Frequency Oscillatory Ventilation.

OBJECTIVE: The aim of this article was to evaluate high-frequency positive pressure ventilation (HFPPV) compared with high-frequency oscillatory ventilation (HFOV) as a rescue ventilation strategy for patients with congenital diaphragmatic hernia (CDH). HFPPV is a pressure-controlled conventional ventilation method utilizing high respiratory rate and low positive end-expiratory pressure. STUDY DESIGN: Seventy-seven patients diagnosed with CDH from January 2005 to September 2019 who were treated with stepwise progression from HFPPV to HFOV versus only HFOV were included. Fisher's exact test an

High-frequency ventilation in the management of tracheal trauma.

Two cases of rupture of the trachea were treated by surgical repair. Intraoperative and postoperative ventilation was managed with high-frequency ventilation. This mode of ventilation afforded good intraoperative exposure and support as well as good postoperative ventilation despite pulmonary contusion without requiring inflation of the endotracheal tube cuff against tracheal suture lines. We recommend high-frequency ventilation in the management of tracheobronchial trauma intraoperatively and postoperatively, because it allows good ventilation despite pulmonary contusion without high positive…

Diaphragmatic flutter with a manifestation of high frequency ventilation.

A case of diaphragmatic flutter following status asthmaticus is presented. The initial symptom was tachypnea resembling hyperventilation syndrome. When spontaneous ventilation was completely suppressed for 15 minutes with intravenous administration of pethidine, levels of arterial blood gases were not much deviated from normal limits, because of possible high frequency ventilation. Although the medical therapeutics previously reported in literatures, including diphenylhydantoin, were ineffective in this case, butylophenones provided favourable results despite of some complications. The etiolog

Respiratory syncytial virus pneumonia ventilated with high-frequency oscillatory ventilation.

Four infants below 6 months of age with proven respiratory syncytial virus infection in need of assisted mechanical ventilation were successfully treated by high-frequency oscillatory ventilation. One of the four infants fulfilled the criteria for extracorporeal membrane oxygenation before the start of oscillation, and one on the second day on high-frequency oscillatory ventilation. However, extracorporeal membrane oxygenation was not needed in any of the infants. All survived, and three appeared to be without any pulmonary sequelae.

Mechanical ventilation in the newborn; a simplified approach. Part 2: High-frequency ventilation.

High frequency oscillatory ventilation (HFOV) is becoming an increasingly popular intervention in the neonatal intensive care unit. This article will attempt to explain the principles of HFOV. It is inherently more difficult to become skilled in this technique than in other forms of mechanical ventilation, so caution is warranted.

Combined high-frequency oscillatory ventilation and intermittent mandatory ventilation in critically ill neonates.

Combined high-frequency oscillatory ventilation (HFOV) and intermittent mandatory ventilation (IMV) was used in 12 neonates with inadequate gas exchange with conventional IMV. Diagnoses included diaphragmatic hernia with hypoplastic lungs, pneumonia, persistent fetal circulation, and severe respiratory distress syndrome. In most patients there was severe air leak. Within 10 hours of beginning HFOV-IMV the mean arterial PCO2 fell from 60 +/- 5 (means +/- SEM) to 38 +/- 2 mm Hg (P less than 0.01) and the mean IMV rate was reduced from 96 +/- 8 to 17 +/- 4 breaths per minute (P less than 0.001).

First intention high-frequency jet ventilation for periviable infants.

PURPOSE OF REVIEW: Ventilation of periviable infants born at 22-23 weeks gestation remains a challenge in neonatology. This review highlights the evidence surrounding the use of first intention high-frequency jet ventilation (HFJV) in infants born near the limits of viability with a review of pulmonary fetal development and a focused overview of HFJV strategies including an in-depth analysis of the management strategies used in the initial randomized trials. RECENT FINDINGS: A paucity of recent trials exists, with no randomized control trials assessing the use of first intention HFJV performe

High frequency percussive ventilation in pediatric acute respiratory failure.

OBJECTIVE: High frequency percussive ventilation (HFPV) is used in acute respiratory failure, but is poorly described in pediatrics. We aimed to describe the clinical characteristics, ventilator settings, and outcomes of a large pediatric cohort, and to determine predictors of who would benefit from HFPV. HYPOTHESIS: Gas exchange 2 h after HFPV initiation predicts success. DESIGN: Single center retrospective cohort study testing association between gas exchange 2 h after HFPV initiation with success, defined a priori. PATIENTS: Intubated children on HFPV for ≥2 h from 2012 to 2018. METHODS

High-Frequency Jet Ventilation in Pediatric Acute Respiratory Failure.

BACKGROUND: High-frequency jet ventilation (HFJV) is primarily used in premature neonates; however, its use in pediatric patients with acute respiratory failure has been reported. The objective of this study was to evaluate HFJV use in the pediatric critical care setting. We hypothesized that HFJV would be associated with improvements in oxygenation and ventilation. METHODS: Medical records of all patients who received HFJV in the pediatric ICU of a quaternary care center between 2014 and 2018 were retrospectively reviewed. Premature infants who had not been discharged home were excluded, as
