Femoral neck fracture is an important early complication after hip resurfacing. Our aims were firstly to determine the incidence of fracture in an independent series and secondly, in a case control study, to investigate potential risk factors. Fifteen femoral neck fractures occurred in a series of 842 procedures, representing an incidence of 1.8%. No relationship existed between age, sex, and fracture incidence. Mechanical factors such as notching, femoral neck lengthening, and varus alignment of the femoral component were found to have a similar incidence in both fracture and control groups.

The results of treatment of 116 intraoperative femoral fractures were reviewed. Excluding calcar and greater trochanter fractures, there were 14 Type I cases (Vancouver classification). All healed with cerclage wiring. Three cases of vertical fractures of the proximal segment following a subtrochanteric osteotomy did not heal and required revision. There were 8 Type II fractures, most being minor splits or limited, nondisplaced, vertical fractures. Control was lost in 2 wired cases, suggesting that Type II fractures should be converted to Type I by the use of a long stem, or, if impractical, b

Treatment of periprosthetic fractures around a total hip arthroplasty is based on fracture timing (intraoperative or postoperative) and fracture location (acetabulum or femur). Most intraoperative fractures are treated by ensuring that the implant and fracture are stable and employing minor technique modifications to ensure that the fracture remains stable postoperatively. Major intraoperative fractures require more complex reconstruction. Postoperative fractures associated with well-fixed implants typically are treated with open reduction and internal fixation. Postoperative fractures associa

We tried to find the trauma mechanism and treatment rationale of ipsilateral concomitant hip and distal femoral fractures involving the articular surface. Between 1988 and 1995, 15 cases of ipsilateral hip (confined to neck or trochanteric areas of the femur) and distal (confined to supra- and intercondylar area of the femur) femoral articular fractures were collected. The hip fractures consisted of 10 trochanteric fractures and five neck fractures, which were managed with reduction and fixation in 14 (Knowles' pin in eight, DHS in four and standard Gamma nail in two), and primary bipolar hemi…

Over a 20-year period, 24 patients were treated for ipsilateral hip and femoral shaft fractures. The majority of the hip fractures were extracapsular. All resulted from high-energy impaction injuries (motor vehicle accidents and falls from over 15 feet), with 25% being open. There were numerous associated injuries, many requiring multiple operations. One-third had an associated knee injury, with 75% occurring in those with extracapsular fractures. The diagnosis and treatment of three hip fractures were delayed, but all healed without avascular necrosis. Patients were treated by traction and va…

During the 2-year period 1978-1979, a total of 2109 hip fractures (of the proximal end of the femur) occurred in Oslo. The age- and sex-specific annual incidence was the highest ever reported. A previous hip fracture had occurred in 13 per cent of the women and 6.8 per cent of the men. In 1979, a total of 5920 hip fractures was reported in Norway. Compared with Oslo, all other counties had a lower incidence. The number of fractures in Oslo was five times greater in 1982 compared with 1950. This increase cannot be explained only by the increasing number of elderly persons.

INTRODUCTION: Timely surgery within the next calendar day is recommended for native hip fractures (NHF) according to NICE guidelines. In contrast, no standardized fast-track protocols exist for periprosthetic hip fractures (PPFx), potentially resulting in surgical delays. This study aimed to evaluate time to surgery and reasons for delay in surgically treated PPFx patients, and to assess complications and mortality according to surgical timing. A previously published NHF cohort was used as a clinically relevant benchmark reflecting the established geriatric native hip fracture pathway.

BACKGROUND/PURPOSE: Treating options of failed internal fixation include revision fixation and salvage arthroplasty. The purpose of this study was to evaluate the results, complications and the final functional outcome associated with modular hip arthroplasty.
METHODS: Modular arthroplasty was performed in 11 patients due to failed treatment of peritrochanteric fracture. Each patient examination included Trendelenburg test, Harris Hip Score, SF-36, and X-rays of the hip. Examinations were recorded and a paired t test was applied for further statistic analysis.
RESULTS: Two years postoperativ

BACKGROUND: The ideal thromboprophylaxis regime following lower limb arthroplasty and proximal femur fractures remains controversial. Guidelines disagree on the type of chemical prophylaxis, its dose or duration. This article describes a method of monitoring venous thromboembolism (VTE) rates following Total Hip (THA), Total Knee Arthroplasty (TKA) and surgery for hip fractures (NOF#).
METHODS: Over 3 years, all patients investigated for VTE were analysed using Picture Archiving Communications System (PACS). All positive scans were then cross-referenced using PACS and local registry data to s…

BACKGROUND: Failed treatment of an intertrochanteric fracture typically leads to profound functional disability and pain. Treatment with repeated attempts to gain union and to preserve the host femoral head usually is preferred for young patients, but salvage treatment with hip arthroplasty may be considered for selected older patients with poor bone quality, bone loss, or articular cartilage damage. The purpose of the present study was to evaluate the results and complications of hip arthroplasty performed as a salvage procedure after the failed treatment of an intertrochanteric hip fracture.

The cascade of events leading to hip fracture is: a fall, protective responses, impact to the hip, local energy absorption and bone strength. A fall from standing height on the hip corresponds to a force of about 3500 N and the bone strength of the proximal femur in elderly women and men ranges between 2000 to 6000 N. Efficient hip protective systems have been developed and may be a significant factor in the prevention of hip fractures among the elderly with propensity to fall and osteoporosis. Theoretically, more than 90 per cent of all hip fractures can be prevented, and a substantial reduct…

Osteoporosis international : a journal established as result of cooperation between the European Foundation for Osteoporosis and the National Osteoporosis Foundation of the USA2020eng
Cost-effective preventive interventions are necessary for tackling the increasing number of hip fractures, which are frequently occuring as a serious consequence of osteoporosis. Several interventions have been available for preventing and treating osteoporosis. The aim of this study was to systematically review and critically appraise studies that assessed cost-effectiveness of hip protectors for the prevention of hip fractures and to investigate the effects of age, gender and residence situation on cost-effectiveness. A systematic review was conducted in order to identify economic evaluation…

BACKGROUND: Patient-reported health-related quality of life is an important outcome measure when assessing the quality of hip fracture surgery. The frequently used EQ-5D index score has unfortunately important limitations. One alternative can be to assess the distribution of each of the five dimensions of the patients' descriptive health profile. The objective of this paper was to investigate health-related quality of life (HRQoL) after hip fractures.
METHODS: Data from hip fracture operations from 2005 through 2012 were obtained from The Norwegian Hip Fracture Register. Patient reported HRQo…

OBJECTIVE: Patients with diabetes have an increased risk of osteoporosis and shorter life expectancy. Hip fracture (HF) is the most serious consequence of osteoporosis and is associated with increased mortality risk. We aimed to assess the association of antidiabetic medications with HF and the post-hip fracture mortality risk among diabetic patients ≥50 years.
DESIGN: In this nationwide case-control study 53 992 HF cases and 112 144 age-, sex- and region-matched non-hip fracture controls were analyzed. A cohort of hip-fractured diabetic patients were followed-up for an all-cause mortality.

BACKGROUND: There is a paucity of data comparing periprosthetic hip fracture (PPHFx) outcomes and resource utilization to native fractures. Many surgeons consider periprosthetic hip fractures to be more severe injuries than native fractures. The aim of this systematic review is to characterize the outcomes of PPHFx and assess their severity relative to native hip fractures (NHFx).
METHODS: A Preferred Reporting Items for Systematic Reviews and Meta-Analysis systematic review was conducted using Medline, Biosis, and Cinahl. Primary outcomes were time to surgery, length of stay (LOS), cost of m…
