Mechanisms of arterial injuries associated with total hip replacement.

Five cases of arterial complications of total hip replacement, all of which can be explained on the basis of intraoperative injury, are reported, with diagrammatic analysis of the intraoperative mechanisms of injury involved. The injuries were manifested as hemorrhage, ischemia, or development of a pulsatile mass. Vascular structures affected include the external iliac, common femoral, medial femoral circumflex, and lateral femoral circumflex arteries. Arterial damage may result from retractor injury, thermal injury from methylmethacrylate, or direct penetration from polymer or gouging during

[Diagnosis and treatment of postoperative infections following hip injuries].

The authors studied the diagnosis and treatment of posttraumatic infections of the hip joint, based on their own material. In case of latent infection the increased sedimentation rate and taking of comparative X-ray films are held important, that shows a narrowing and disappearance of the sclerosed edge of the acetabulum. Following dislocation of the hip joint ankylosis may be attained, but after fractures of the proximal and of the femur the infection cannot be cured, unless the nail and the head of the femur are removed.

[Safe surgical dislocation of the hip to treatment of injuries after traumatic hip injury in children].

Traumatic dislocation of the hip in children is rare. Immediately closed reduction is important but contains a risk of tissue becoming intertwined in the joint space, and a risk of development of avascular femoral head necrosis. In this case report, we present two cases of traumatic hip luxation in an 11-year-old boy and a nine-year-old boy, where intertwined labral tears were recognised immediately following closed reduction in one case and after two weeks in the other. They were treated with safe surgical dislocation of the femoral head. Blood flow to the femoral head was ensured by perioper

Sports Injuries about the Hip: What the Radiologist Should Know.

Injuries of the hip and surrounding structures represent a complex and commonly encountered scenario in athletes, with improper diagnosis serving as a cause of delayed return to play or progression to a more serious injury. As such, radiologists play an essential role in guiding management of athletic injuries. Familiarity with hip anatomy and the advantages and limitations of various imaging modalities is of paramount importance for accurate and timely diagnosis. Magnetic resonance (MR) imaging is often the modality of choice for evaluating many of the injuries discussed, although preliminary

Hip injuries and labral tears in the national football league.

BACKGROUND: Injuries to the hip account for approximately 10% of all injuries in football, but definitive diagnosis is often challenging. Although these injuries are often uncomplicated contusions or strains, intra-articular lesions are increasingly found to be sources of hip pain. PURPOSE: The objective was to define the incidence and etiologic factors of intra- and extra-articular hip injuries in the National Football League (NFL). STUDY DESIGN: Descriptive epidemiology study. METHODS: The NFL Injury Surveillance System was used to define all hip-related injuries from 1997 to 2006. Injuri

Evidence-based treatment of hip and pelvic injuries in runners.

The runner is especially at risk for development of injury to the hip and pelvis secondary to chronic repetitive microtrauma. The key to treatment is establishing complete and accurate diagnosis, and, in particular, identifying the functional biomechanical deficits in the kinetic chain that contribute to this repetitive microtrauma. A long-term successful outcome and prevention of reinjury are more likely if the focus of rehabilitation is on the restoration of the functional kinetic chain, rather than on a specific injured tissue. For example, the typical treatment of "iliotibial band syndrome

Injuries about the hip and pelvis in the young athlete.

Athletic injuries to the hip and pelvis in pediatric and adolescent athletes, although uncommon, may encompass a wide spectrum of entities. A familiarity with this spectrum and a high index of suspicion in the proper clinical setting will ensure timely diagnosis and help to facilitate implementation of a proper treatment plan thereby assuring safe return to play.

The management of vascular injuries associated with total hip arthroplasty.

Approximately 100,000 total hip reconstructions are done annually in the United States. The nature of the surgical technique in a field close to the iliac and femoral vessels makes the occurrence of vascular injury an occasional but serious complications. We have reviewed retrospectively our experience of five cases of vascular injuries with total hip replacement and an additional 63 cases in the literature to identify those patients at risk and to define the management of these injuries. For the entire group of 68 patients, most injuries were sustained on the left side (66%), and 39% were see

Neurovascular Injury in Hip Arthroplasty.

Neurological and vascular complications following hip arthroplasty are uncommon, and their impact ranges from transient and trivial to permanent and devastating. The proximity of neural and vascular structures makes any operation on the hip potentially hazardous. Direct or indirect injuries of these structures may occur during operative exposure and subsequent procedures. Thus, complete awareness of the anatomy of the pelvis and proximal femur is required. Peripheral nerve injuries can involve either distant sites or nerves in the immediate vicinity of the hip joint. Sciatic nerve injury is th

The assessment of athletic hip injury.

The differential diagnosis of athletic hip pain is extensive. The physical demands of sport place the athlete at increased risk to injure the hip. The basis of successful treatment of the athlete with hip pain lies in the ability of the physician to assess and diagnose the etiology accurately. A thorough understanding of the evaluation of the athlete with hip pain allows for an earlier diagnosis, earlier treatment, and ultimately an earlier return to sport.

Rehabilitation of Soft Tissue Injuries of the Hip and Pelvis.

The athlete's hip is complex when it comes to the surrounding musculature-approximately 21 different muscles can cross the hip and pelvis region, all synchronously working to maintain pelvic stability and functional hip activities. Commonly injured muscle groups for high-level athletes include flexors, adductors, abductors, and/or proximal hamstring musculotendinous complex. These muscle groups work in harmony; however, each has an independent function and propensity for injury. Rehabilitation phases for each injury group can be broken down into 3 phases: acute management, strengthening, and r

Editorial Commentary: Proximal Hamstring Injuries-Is the Hip to Blame?

There is a growing understanding of the relation between femoroacetabular impingement (FAI) and injury to adjacent structures. Patients with proximal hamstring pathology appear to have a high prevalence of underlying FAI. The kinetic chain phenomenon is a potential explanation of the high correlation between proximal hamstring injury and underlying FAI of the hip.

Ultrasound-guided Interventions for Core and Hip Injuries in Athletes.

Trauma and the mechanical strain of high-energy activity predispose athletes to pelvic injuries. Magnetic resonance imaging remains the primary modality for diagnosing these conditions, but ultrasonography-guided interventions are important in the management of core muscle, hip, and other pelvic conditions. This article reviews ultrasonography-guided interventions for injuries at the core, including the pelvis and hips. It reviews common injuries, procedure optimization, medication preparation, clinical evidence behind injections, tenotomy, and platelet-rich plasma. These interventions are esp

Endovascular treatment of external iliac artery injuries after hip arthroplasty.

PURPOSE: To report endovascular repair of injuries to the external iliac artery during hip surgery. CASE REPORTS: Two elderly women with histories of chronically infected hip prostheses were found to have iatrogenic leaking pseudoaneurysms of the external iliac artery due to hip surgery trauma in the past. Both were treated successfully with a Jostent Peripheral Stent-Graft. The first patient died 17 months after treatment, and the second was well, with an excluded false aneurysm, at the 6-month follow-up. CONCLUSIONS: Stent-graft repair may be an appropriate and effective treatment for some

Avoidance and management of neurovascular injuries in total hip arthroplasty.

Neural injuries that occur after total hip arthroplasty (THA) can be classified as involving either the central nervous system or peripheral nerves. Central nervous system changes after THA may be attributed to increased appreciation of fat embolism syndrome associated with THA. Certain maneuvers such as impacting the acetabulum, femoral reaming, and cement pressurization can force marrow fat into the venous system. When there is an associated right to left shunt, paradoxical embolization can occur, which may account for previously unexplained cases of confusion and mental status changes after

Hip dislocation in patients with multiple injuries. A followup investigation.

The combination of traumatic hip dislocation and multiple trauma have not been well documented in the medical literature to date. The role of surgery therefore has not been well defined in these patients. During a 12-year period, 29 consecutive patients with 31 traumatic hip dislocations with concomitant Injury Severity Scores exceeding 18 points were treated at the authors' institution. An initial satisfactory closed reduction was achieved in 23 patients, two patients had failed reductions, and four patients had noncongruous reductions. Open surgical procedures were done on these patients and

Associations Between Hip Pathology, Hip and Groin Pain, and Injuries in Hockey Athletes: A Clinical Commentary.

UNLABELLED: Femoroacetabular impingement (FAI), particularly cam morphology, is highly prevalent among elite hockey athletes. Moreover, hip and groin pain has become a common issue in hockey, with approximately 50% of European professional athletes reported to experience a hip or groin problem during a season. While most athletes will not miss training or competition due to this, restricted competitive performance and increased risk of reduced physical and psychological well-being are likely. Recent research suggests that the development of cam morphology is related to the repetitive shear str
