The radiographic features of pericarditis due to Histoplasma capsulatum are described in 5 children. This diagnosis is suggested when enlargement of the cardiac silhouette is associated with pneumonia, parenchymal nodules or adenopathy. These findings are particularly significant if the patient lives or has lived in an area which is endemic for histoplasmosis. While pleural effusion is rare in primary histoplasmosis, it is frequently present in patients with histoplasma pericarditis.

In a 3-year-old boy disseminated histoplasmosis was recognized from a liver biopsy. The patient died shortly afterwards and had foci of the disease in liver, spleen, and kidneys. Large numbers of yeast cells of Histoplasma capsulatum were found within histiocytes in other organs including the eye. This rare finding is remarkable in view of the search for Histoplasma capsulatum in the syndrome known as "so-called histoplasmosis of the choroid."

Negative cultures in endocarditis often lead to delays in targeted life-saving therapies. We present the case of a 68-year-old man who presented with culture-negative endocarditis, which was complicated by coronary embolization resulting in anterior ST-elevation myocardial infarction (STEMI). Aspiration thrombectomy led to the diagnosis of fungal endocarditis, one of the most serious causes of culture-negative presentation.

Histoplasma infection of vascular grafts is extremely rare. To our knowledge, there are only 4 cases reported with Histoplasma capsulatum infection of the aortic graft. All had previous disseminated histoplasmosis and atherosclerotic peripheral vascular disease. They were treated surgically with explantation of the infected graft and reimplantation of new graft in extra-anatomic uninfected site. The authors present a new case of H capsulatum infection of aortofemoral bypass graft, but unlike the other cases, this case was managed without surgical intervention.

Histoplasma capsulatum is a facultative intracellular pathogen, and the causative agent of the most common systemic fungal infection. Over the past several years, many new insights have been learned concerning the biology, biochemistry, and molecular genetics of this microorganism. This review focuses on the immunology of host defense against H. capsulatum yeasts with emphasis on the development of cell-mediated immunity, and the strategies used by the fungus to survive and multiply within macrophages.
