Hoarseness: is it really laryngopharyngeal reflux?

OBJECTIVE/HYPOTHESIS: This study will test the hypothesis that proton pump inhibitor (PPI) use is prevalent among patients referred for hoarseness and will assess the ultimate diagnosis and factors associated with patients' voice outcomes. STUDY DESIGN: Retrospective review of patients in a tertiary care voice clinic. METHODS: Patients with a primary diagnosis of hoarseness, who were taking or had taken PPIs in the previous 2 months and referred to a tertiary care voice clinic, were identified. The dosage and length of PPI administration, patient report of gastroesophageal reflux (GER), pres…

Cause of hoarseness in elderly patients.

Hoarseness is a common symptom in older individuals and may reflect a wide variety of pathologic, medical, physiologic, and/or functional causes. Although vocal fold atrophy is one of the more common reported findings in the elderly, inconclusive information is known about the differential diagnosis and cause of dysphonia in older individuals. The purpose of this investigation was to review the cause of hoarseness in all patients older than 65 years and to determine any correlation with advancing age and other demographic factors. Additionally, we wanted to determine the effect vocal pathology…

Hoarseness and gastroesophageal reflux in children.

The importance of a hoarse voice or voice change in children has not been stressed in the literature in the same way as it has been in adults. We present 21 children who had been suffering from chronic hoarseness for more than three months and had on fibre-optic laryngoscopy findings suggestive of gastroesophageal reflux. None of them had complained of gastroesophageal symptoms. Twenty-four hour pH monitoring revealed that 13 (62 per cent) of these children had gastroesophageal reflux, seven (33 per cent) having gastroesophageal reflux more than three times the upper limit of normal. The pH gr…

[Clinical analysis of hoarseness after thyroidectomy].

The present paper reports 86 cases of hoarseness after thyroidectomy. In 37 cases, glottic paralysis was confirmed. Among them the injury of recurrent laryngeal nerve were 89.91% (33/37). In 33(36 side) cases of recurrent laryngeal nerve paralysis, left injury was 20 and right was 16. Referring to the literature author consider that: 1. the recurrent laryngeal nerve was injured easy by thyroidectomy because that thyroid gland was located closely with recurrent laryngeal nerve in neck; 2. recurrent laryngeal nerve injury after thyroidectomy was related to the character of thyroid gland tumor an…

Hoarseness Due to Aortic Arch Aneurysms.

OBJECTIVE: To give an overview of the Ortner's syndrome caused by an aortic arch aneurysm. METHODS: By comprehensive retrieval of the pertinent literature published in the past two decades, 75 reports including 86 patients were collected and recruited into this study along with a recent case of our own. RESULTS: The aortic arch aneurysms causing hoarseness were most commonly mycotic aneurysms. In this patient setting, in addition to the left recurrent laryngeal nerve, trachea was the most commonly affected structure by the aortic arch aneurysm. Surgical/interventional/hybrid treatments led t…

Aripiprazole-Induced Hoarseness: A Case Report.

OBJECTIVES: Aripiprazole is an atypical antipsychotic drug that is commonly used in children and adolescents. METHODS: The most common adverse effects of aripiprazole include fatigue, nausea, increased appetite, headache, sedation, and somnolence. RESULTS: To our knowledge, there are no data regarding aripiprazole-induced hoarseness in the existing literature. CONCLUSIONS: We present a case of a preschool boy with attention-deficit/hyperactivity disorder and oppositional defiant disorder who displayed hoarseness after aripiprazole therapy.

Pulmonary thromboembolism presenting with hoarseness of voice.

A young man in his late 30s with a history of pulmonary tuberculosis presented with complaints of hoarseness of voice persisting for 6 months. Chest auscultation revealed diminished breath sounds over the right lung field. An initial chest X-ray showed collapse of the right lung with mediastinal shift towards right. Fibreoptic laryngoscopy revealed sluggish movement of the left vocal cord. CT neck showed focal nodular thickening in the left false vocal cord and thrombus in the right main pulmonary artery. The subsequent CT pulmonary angiogram performed revealed right pulmonary thromboembolism …

Hoarseness: When to observe and when to refer.

The terms hoarseness and dysphonia are used interchangeably, and both describe a type of altered vocal quality affecting one-third of patients. While hoarseness may be secondary to benign conditions such as reflux or viral laryngitis, it may suggest benign or malignant vocal-fold pathology. It is important for caregivers to know how to evaluate, treat, and when to refer patients for direct visualization via laryngoscopy. In this article, we review basic laryngeal anatomy and function, symptoms of vocal-fold pathology, and current guidelines from the American Academy of Otolaryngology-Head and …

Hoarseness in an older adult: Ortner syndrome.

A detailed examination in an older adult presenting with chronic hoarseness is mandatory to exclude an upper aerodigestive tract malignancy. We describe a 71-year-old chronic smoker with essential hypertension who presented with hoarseness and vocal fatigue for 5 months. Laryngoscopic examination showed left vocal fold paralysis with phonation gap. No growth was seen at all laryngeal and hypopharyngeal subsites. The rest of the head and neck, chest, upper limbs and neurovascular examination were unremarkable. A plain chest radiograph demonstrated a cause for the left recurrent laryngeal nerve …
