PURPOSE: To report two cases of Horner's syndrome. One presented after the ablation of a schwannoma of the cervical sympathetic chain, the second after upper thorascopic sympathectomy for primary palmar hyperhidrosis.
METHODS: A 42-year-old man underwent excision of a left neck mass found during routine physical examination. A 20-year-old girl with axillary and palmar hyperhidrosis was treated with cervical sympathectomy.
RESULTS: In the early postoperative days, miosis, ptosis, anhidrosis, and enophthalmos were observed.
CONCLUSIONS: In the ablation of a schwannoma, postoperative Horner's

A puppy was presented with unilateral Horner's syndrome thought to have been in existence since the eyes first opened. No other clinical signs were evidence and the condition was attributed to trauma during assisted birth. All abnormalities resolved spontaneously by 11 weeks of age.

We studied the accuracy of the clinical diagnosis of Horner's syndrome in 62 patients. The final criterion was serial photography of the darkness reflex of the pupil. This technique confirmed oculosympathetic dysfunction in only 50%. The positive predictive value of the clinical diagnosis could be increased to more than 80% by measuring the degree of miosis and ptosis on single photographs, or by assuming independent confirmation of the clinical diagnosis by a second observer. However, the chance that a patient who is judged normal in this way does still have oculosympathetic dysfunction remai

Ten cases of Horner's syndrome with pharmacologic testing and computed tomography scans are described in patients up to age 8 years. The patients also were assessed for iris color and facial sweating. Classical preganglionic Horner's syndrome associated with brachial plexus birth injury was not identified in any case. Two patients who presented with ptosis had neuroblastoma. Two other children had undergone corrective cardiothoracic surgery. Two patients had major congenital abnormalities. In four patients, no cause of Horner's syndrome was determined.

The diagnosis of Horner's syndrome is divided into two stages: (1) the recognition of the sympathetic deficit, and (2) the localization of the lesion. The second step is of vital importance in the management of the patient with Horner's syndrome. If the lesion is preganglionic, the risk of malignancy is high; the patient should have cervical spine and chest roentgenograms (PA, lateral and apical lordotic) and perhaps a referral to a surgeon. If the lesion is postganglionic, it is most likely a benign vascular headache syndrome and the patient should go to a neurologist.

INTRODUCTION: Horner syndrome (HS) is a rare neurological disorder arising from disruption of the oculosympathetic pathway. Pediatric HS is uncommon and may be congenital, but underlying sinister causes need to be excluded.
CASE PRESENTATION: An 18-week-old boy presented with right peri-orbital swelling, initially thought to be pre-septal cellulitis. Further ophthalmic review revealed a right-sided HS. Imaging identified a probable cervical neuroblastoma, leading to an urgent referral to oncology.
CONCLUSION: Early recognition of pediatric HS is crucial as it may signal underlying malignanci…

Horner syndrome is a combination of signs and symptoms caused by abnormalities in the oculosympathetic pathway. Typically, patients present with ipsilateral miosis, ptosis, and anhidrosis. Imaging the oculosympathetic pathway is complex because of its extensive course, need for targeted protocols, and the wide range of pathology that can cause it. For this document, we focus our attention on workup guidelines based on clinical features: 1) acuity (acute, nonacute including chronic, insidious, and unknown onset), 2) painless or painful (neck pain, occipital region pain, or headaches), 3) whethe…
