We have presented a previously unreported complication of enteral hyperalimentation, namely drug-induced fever due to tube feeding formula. Clinicians caring for patients receiving enteral hyperalimentation should be alert to this complication so that needless tests and medications can be avoided.

Malignant hyperthermia (MH) is a rare syndrome seen when susceptible individuals are exposed to one or more of various triggering agents, most commonly a depolarizing muscle relaxant or an inhalational anaesthetic agent. There is a dramatic rise in metabolic rate and oxygen consumption which if not treated promptly and effectively results, in the majority of cases, in the patient's death. Fatal responses to physical stress associated with the familial muscular disease known as MH are being increasingly reported in situations not related to anaesthesia or drugs. Moreover, cases of MH were recog…

Malignant hyperthermia, a rapidly progressive, frequently lethal syndrome was not described until quite recently. Although treatment with the drug dantrolene has changed its face, a clear understanding and deep respect for the syndrome are necessary to keep fatalities and tragic sequelae to a minimum. The etiology and pathophysiology, preanesthetic diagnosis, incidence, management of the acute crisis, and anesthesia for susceptible patients are discussed, and a successfully treated fulminant case is analyzed.

Malignant hyperthermia is a potentially fatal complication of general anesthesia that may occur with greater frequency in some patients with ptosis or strabismus. The history of our knowledge of this recently described condition is outlined. The clinical features, pathophysiology, and differential diagnosis are reviewed in detail. A protocol for the treatment of the acute malignant hyperthermia crisis and guidelines for the management of susceptible patients are suggested. The medicolegal implications of the occurrence of such a crisis are discussed.

A report is made of a 65-year-old male who died of a malignant hyperthermia of 42 degrees C. Symptoms included muscle rigidity at the termination of operation for neck-clipping of an aneurysm of the anterior communicating artery. Latent myopathy was observed in skeletal muscle, and a bleeding focus was present in the left anterior region of the hypothalamus, coinciding with the temperature regulation center. The mechanism of onset of this disease is still poorly understood, but it seems that this disease occurred due to synergic effects of the hypothalamic hemorrhage and the pre-existing myopa…

A case has been presented that illustrates successful managment of a patient with suspected malignant hyperthermia. The causes of this disorder are uncertain. If screening procedures identify a patient as susceptible to this disorder, careful planning in the preoperative stage is indicated. Preparedness during the operative procedure for any emergency is mandatory. Early and effective treatment seems to be the only method of preventing mortality with patients experiencing malignant hyperthermia.

Malignant hyperthermia is now recognized as a distinct entity in anesthetic practice and can be considered as a pharmacogenetic disease of obscure etiology occuring in man and pigs with a dominant inheritance. A close association with myopathy has been noted. Commonly used muscle relaxants or anesthetic drugs can act as triggering agents in genetically susceptible patients, who develop a real hypermetabolic state, characterized by a rapid rise in body temperature, muscular rigidity, tachycardia and tachypnoea, cyanosis and severe respiratory and metabolic acidosis, the lethality being about 60…

Malignant hyperthermia is a genetic disease that may become apparent after exposure to drugs; some of these drugs are commonly used by the oral surgeon. One in 14,000 young, apparently health individuals may be affected; mortality is 64 percent. Many cases of malignant hyperthermia can be prevented if an adequate history is obtained. Oral surgeons should be familiar with the prevention, diagnosis, and initial treatment of malignant hyperthermia.
