Successful treatment of acromegaly in a diabetic cat with transsphenoidal hypophysectomy.

UNLABELLED: PATIENT AND SURGICAL TREATMENT: An 11-year-old, castrated male cat was referred for insulin-resistant diabetes mellitus. It had … lle and an enlarged pituitary gland, indicating a pituitary neoplasm. Microsurgical transsphenoidal hypophysectomy was performed and microscopic examination of the surgical specimen revealed an acidophilic, infiltrative pituitary adenoma that showed positive immunostaining for GH. OUTCOME: The clinical signs resolved and 3 weeks after surgery the cat no longer …

Ultrastructure of the skeletal muscles in acromegaly--before and after hypophysectomy.

Electron microscopic examination of the skeletal muscles in a patient with acromegaly and marked elevation of the serum growth hormone reveals altered mitochondria (pleomorphism, elongation, matrical pallor, and cristae abnormalities), glycogen granules infiltration, inclusion bodies, and vesicular dilatations. Nine months after surgical removal of the pituitary tumor, the serum growth hormone levels were markedly diminished. A repeat examination of the skeletal muscle shows that the previous ultrastructural changes in the muscles are related to the growth hormone levels.

Endoscopic, endonasal, trans-sphenoidal hypophysectomy: retrospective analysis of 171 procedures.

INTRODUCTION: Endoscopic, transnasal management of pituitary gland neoplasms is a widely accepted alternative to the traditional microscopic approach. This study aimed to determine outcomes and complication rates for the largest UK series of endoscopic, trans-sphenoidal hypophysectomies reported to date. METHODS: We performed a retrospective analysis of 136 primary resections and 35 revision cases performed at a tertiary referral centre. RESULTS AND ANALYSIS: Total tumour resection was confirmed in over 85 per cent of primary and revision cases, with biochemical remission in 60 per cent. The

Stereotactic hypophysectomy for intractable pain secondary to metastatic prostate carcinoma.

Stereotactic radiofrequency transfrontal hypophysectomy was carried out in 7 patients with osseous metastasis from prostatic carcinoma resulting in intractable pain. 86% experienced significant pain relief with 5 of 7 demonstrating marked decrease in medication requirement and improved level of function. The procedure was done under local anesthesia only with no mortality and minimal morbidity. Total destruction of the pituitary gland and the development of diabetes insipidus were shown not to be necessary conditions for pain relief.

Immunohistochemical Evaluation of Canine Pituitary Adenomas Obtained by Transsphenoidal Hypophysectomy.

Hypophysectomy specimens from 16 dogs with pituitary adenoma were evaluated with periodic acid-Schiff (PAS), reticulin, and immunohistochemistry for adrenocorticotrophic hormone (ACTH), melanocyte stimulating hormone (MSH), growth hormone (GH), and Ki-67. The reticulin network was obliterated in all adenomas. One adenoma expressed ACTH and GH. Eight corticotroph adenomas were basophilic to chromophobic, and PAS- and ACTH-positive. Seven melanotroph adenomas were distinguished from corticotroph adenomas by expression of MSH. Pituitary-dependent hypercortisolism was diagnosed in 5 of 8 dogs with…

Transsphenoidal hypophysectomy for four dogs with pituitary ACTH-producing adenoma.

Four dogs diagnosed as pituitary-dependent hyperadrenocorticism were treated with transsphenoidal hypophysectomy and postoperative hormone supplementation therapy. On histological examination of the resected tissues, the tumors were ACTH-producing adenoma of the anterior lobe. Clinical signs such as alopecia and calcinosis cutis, as well as endocrinological abnormalities, were markedly alleviated after surgery. The clinical courses of these 4 dogs suggest that transsphenoidal hypophysectomy may be a useful treatment for pituitary-dependent hyperadrenocorticism.

Assessment of pituitary function after transsphenoidal hypophysectomy in beagle dogs.

Pituitary function was assessed in healthy adult beagle dogs before and after hypophysectomy. Anterior pituitary function was tested by use of the combined anterior pituitary (CAP) function test, which consisted of sequential 30-sec intravenous injections of four hypothalamic releasing hormones, in the following order and doses: 1 microgram of corticotropin-releasing hormone (CRH)/kg, 1 microgram of growth hormone-releasing hormone (GHRH)/kg, 10 micrograms of gonadotropin-releasing hormone (GnRH)/kg, and 10 micrograms of thyrotropin-releasing hormone (TRH)/kg. Plasma samples were assayed for a…

Case Report: Treatment of hypersomatotropism in a diabetic dog with transsphenoidal hypophysectomy.

BACKGROUND: Pituitary somatotroph adenoma is rare in dogs and may cause hypersomatotropism (HS) leading to insulin resistance and diabetes m … omegaly, and arthropathy. TREATMENT AND OUTCOME: The pituitary mass was removed by transsphenoidal hypophysectomy. Immunohistochemistry confirmed a growth hormone (GH)-producing pituitary adenoma. Postoperatively, GH normalized within hours, and IGF-1 values within a week. Although HS resolved and hyperinsulinemia improved postoperatively, the dog remained depe …

Effect of hypophysectomy on endocrine cell types in rat gastrointestinal mucosa.

The effect of hypophysectomy on the gastrointestinal tract was studied in the rat 8 wk after operation, particularly regarding the frequency and distribution of serotonin, somatostatin and gastrin-immunoreactive cells. Body weight, the length of the intestine and the thickness of the mucosa of the antrum and small intestine were all reduced in the hypophysectomised rats compared with sham-operated and untreated controls. In the hypophysectomised animals the serotonin-immunoreactive cells were fewer in the antrum and caecum, whereas they were more numerous in the proximal large intestine. There…

Experience with sublabial transseptal transsphenoidal hypophysectomy at the Universiti Kebangsaan Malaysia.

The transseptal transsphenoidal hypophysectomy has become a relatively frequent procedure in recent years. We performed 20 such procedures between January 1984 and December 1987 for various pituitary disorders. Significant complications such as CSF rhinorrhea, meningitis, diabetes insipidus, haemorrhage and septal perforation are discussed. The mortality rate for the series was 5%. In analysing the data, we feel that this technique of hypophysectomy is a safe procedure.

Failure of total hypophysectomy to remove intrasellar microadenoma in cushing's disease.

The pathological findings are described of a female patient with persistent Cushing's disease after two unsuccessful transsphenoidal operations: a left transsphenoidal hemihypophysectomy followed by a total hypophysectomy 1 month later. The patient was finally cured by bilateral adrenalectomy but suddenly died of heart failure 4 months later. Postmortem examination did not show invasive ACTH-secreting tissue in the pituitary region or an ectopic ACTH-secreting tumor, as initially presumed. Instead, a very small corticotroph adenoma was located immediately under the diaphragm sellae at the left…

Effects of hypophysectomy on pro-thyrotropin-releasing hormone concentrations in rats.

The effects of hypophysectomy on pro-thyrotropin-releasing hormone (pro-TRH) and TRH concentrations in the rat hypothalamus, cerebrum, cerebellum and brain stem, stomach and eye were studied. The hypophysectomy was performed via ear route and the rats were used for experiments at seven days after the operation. The hypophysectomized rats were administered T4 (500 micrograms/kg), T3 (100 micrograms/kg), TRH (1.0 mg/kg), or bovine TSH (1.25 IU/kg) was injected ip, and five rats in each subgroup were decapitated at four hours after the injection. Pro-TRH, TRH, TSH and thyroid hormone were measure…

Anaesthetic management and complications during hypophysectomy in 37 cats with acromegaly.

OBJECTIVES: The aim of this study was to describe the anaesthetic management and perianaesthetic complications encountered during hypophysectomy surgery in acromegalic cats. We explored relationships between animal demographic data, the anaesthetic protocol used and presence of perioperative complications. METHODS: Cats having undergone hypophysectomy surgery for the treatment of feline acromegaly at a single veterinary referral hospital were identified from hospital records. The anaesthesia records and clinical notes of these animals were retrospectively reviewed. Descriptive statistics were…
