An unusual case of an immersion hand presentation in a military signaller operating in the jungle in Belize.

Belize, hosting one of the British Army's overseas training areas, provides access to challenging terrain and austere environments, which al … re no reported cases of immersion hand, comparisons can be made with the well-reported warm weather immersion foot. This case highlights the importance of force preparation and soldier education for units deploying to the jungle. Simple preventive measures, including adequate 'wet-dry' drills and use of emollients can reduce the prevalence of immersion hand, a p …

Thermal and visceral hypersensitivity in irritable bowel syndrome patients with and without fibromyalgia.

BACKGROUND: Irritable bowel syndrome (IBS) is a chronic gastrointestinal disorder characterized by both visceral and somatic hyperalgesia, producing a similar effect seen with the central hypersensitivity mechanism in fibromyalgia (FM). OBJECTIVES: The aim of the current study was to compare magnitudes of visceral and thermal hypersensitivity in IBS patients and FM patients with IBS (FM+IBS) compared with healthy controls. METHODS: Female patients with IBS (n=12), FM+IBS (n=12), and control participants (n=13) rated pain intensity to hot water immersion (45 and 47 degrees C) of the hand/foot

[Disorders caused by heat, cold, and abnormal pressure].

Exposure to heat disturbs the homeostasis of body water, serum osmosis, and core temperature, resulting in the development of heat cramp, heat syncope, heat exhaustion, and heat stroke. Commonly coexisting risks are humidity, windlessness, infrared radiation, physical exertion, continuous work, chemical protective clothing, and lack of acclimatization. Exposure to cold constricts peripheral arteries and reduces metabolism, resulting in the development of chilblains, frostbite, immersion foot, and hypothermia. Wind, water immersion, and alcohol drinking will aggravate the symptoms. Exposure to …

Cold exposure injuries: prevention and treatment.

A variety of cold exposure injuries were discussed, including frostnip, chilblains, trench foot, frostbite, and hypothermia. The usual precipitating factors for each were discussed along with the clinical course, the pathophysiology, and management from the perspective of one being in the field or backcountry. Prevention is the key to all cold-related problems as adequate treatment is often not possible out of the hospital setting. Detailed recommendations were made for all conditions, relative to treatment and prevention, with special emphasis on hypothermia.

Cold-induced peripheral nerve damage: involvement of touch receptors of the foot.

A 31-year-old male developed paresthesia and numbness of mainly the right foot following exposure to nonfreezing temperatures under moist conditions over a period of 1 week. The symptoms gradually improved over several months. When seen for electrophysiological studies 6 months after the injury, there was no sensory loss on clinical examination, although he continued to complain of distal numbness of the right foot. The right extensor digitorum brevis muscle was atrophic, and the distal motor latency in the peroneal nerve was prolonged. Conduction studies of the right sural nerve showed a pred

Methicillin-resistant Staphylococcus aureus foot infection after fish pedicure.

Fish pedicure has become a popular cosmetic procedure involving immersion of the feet in a tank with freshwater fish (Garra rufa) that nibble off dead skin. There are concerns about the potential transmission of pathogens, but no cases of infections after this procedure have been published so far. We present a patient who developed foot infection with methicillin-resistant Staphylococcus aureus (MRSA) after fish pedicure.

No association between hand and foot temperature responses during local cold stress and rewarming.

PURPOSE: The purpose was to examine whether associations exist between temperature responses in the fingers vs. toes and hand vs. foot during local cold-water immersion and rewarming phases. METHODS: Seventy healthy subjects (58 males, 12 females) immersed their right hand or right foot, respectively, in 8 °C water for 30 min (CWI phase), followed by a 15-min spontaneous rewarming (RW) in 25 °C air temperature. RESULTS: Temperature was lower in the toes than the fingers during the baseline phase (27.8 ± 3.0 vs. 33.9 ± 2.5 °C, p < 0.001), parts of the CWI phase (min 20-30: 8.8 ± 0.7 vs. 9.7 ±

Cold weather issues in sideline and event management.

Exercise in cold environments exerts a unique physiologic stress on the human body, which, under certain conditions, may result in a cold-related injury. Environmental factors are the most important risk factors for the development of hypothermia in athletes. Frostbite occurs as a result of direct cold injury to peripheral tissues. The biggest risk for frostbite is temperature. Trench foot is a result of repeated and constant immersion in cold water. Chilblains are local erythematous or cyanotic skin lesions that develop at ambient air temperatures of 32°F to 60°F after an exposure time of abo

Increase of heat loss by wearing gloves and boots in wet-suited subjects working in cold water.

The effect of wearing protective gloves and boots on thermal exchanges of wet-suited subjects in cold water was evaluated. Four male subjects, clad in 5 mm-thick neoprene wet suits and either with or without neoprene gloves (5 mm-thick) and boots (5 mm-thick) were immersed up to the neck in water at 13 degrees C while resting for 3 h or exercising for 2 h. Rectal temperature, oxygen consumption and local (chest, back, upper arm, thigh, forearm, calf, hand and foot) skin temperatures, skin heat fluxes and thermal insulations were determined during immersion. The rectal temperature was not diffe

Influence of virtual reality with or without hypnosis on patient-related experience and functional recovery in outpatient foot surgery with regional anesthesia: a randomized controlled trial.

INTRODUCTION: Perioperative psychological stress significantly influences recovery after surgery. Sciatic nerve and ankle blocks are commonly used techniques for achieving postoperative analgesia in outpatient foot surgery. However, these procedures can cause discomfort for patients, potentially affecting their postoperative functional recovery. This study aims to assess the impact of virtual reality with or without hypnosis on patient satisfaction and postoperative functional recovery in patients undergoing peripheral nerve blocks for outpatient foot surgery. METHODS: A prospective randomize

Limb oxygenation during the cold pressor test in spinal cord-injured humans.

OBJECTIVE: To investigate changes in tissue oxygenation in the arm and leg during the cold pressor test in humans with spinal cord injury (SCI). METHODS: Subjects with SCI at cervical 6 (n=7) and subjects with SCI at thoracic 5 or thoracic 6 (n=5) experienced 3-min cold water immersion of the foot and subsequent 10-min recovery. Changes in tissue oxygenation and blood pressure were determined. Tissue oxygenation was assessed by hemoglobin/myoglobin concentration (Hb/MbO2) measured using near-infrared spectroscopy. RESULTS: Mean arterial blood pressures increased significantly by 15±9 and 6±6

Urinary responses to cold temperature during water immersion.

If cold temperature combines with ambient water pressure to stimulate the Henry-Gauer reflex in humans, then free water clearance (CH2O) should be greater during immersion in cold water (29.8 degrees C) than during exposure to cold air (14.8 degrees C) or immersion in thermoneutral water (35 degrees C). Urinary responses to these environments were compared with control measurements made during 6 h of sitting in thermoneutral air (27.6 degrees C). CH2O was not significantly greater in cold water than in the other environments. Rather, the diuretic response was characterized by an increased osmo

Cold-induced vasodilatation is not homogenous or generalizable across the hand and feet.

Cold-induced vasodilatation (CIVD), a paradoxical cyclical increase in finger temperature upon cold exposure, is assumed to serve against cold injury. Most existing research has focused on hand and finger responses, yet most cold injuries occur in the feet. We investigated if CIVD responses of one finger can predict the responses of other fingers, and also whether the fingers could predict CIVD responses of the feet and toes. Ten subjects immersed their left hand up to the wrist in 8 degrees C water for 30 min. They passively rewarmed for 15 min, followed by immersion of the left foot to the a

Immersion anaesthesia with ethanol in African giant land snails (Acathina fulica).

Giant African land snails (Achatina fulica) are becoming increasingly popular pets and may be anaesthetised to allow diagnostics and surgical procedures. The objective of the present study was to evaluate the anaesthetic effects and anaesthetic-related complications of immersion in 5% ethanol in client-owned African pet land snails, anaesthetised to allow biopsies of the foot for screening of parasites. Variables such as minutes elapsing from immersion to anaesthetic induction and from removal from the bath to return of tentacle withdrawal reflex and recovery from anaesthesia were recorded, as

Effect of pressure on thermal insulation in humans wearing wet suits.

The present work was undertaken to determine the critical water temperature (Tcw), defined as the lowest water temperature a subject can tolerate at rest for 3 h without shivering, of wet-suited subjects during water immersion at different ambient pressures. Nine healthy males wearing neoprene wet suits (5 mm thick) were subjected to immersion to the neck in water at 1, 2, and 2.5 ATA while resting for 3 h. Continuous measurements of esophageal (T(es)) and skin (Tsk) temperatures and heat loss from the skin (Htissue) and wet suits (Hsuit) were recorded. Insulation of the tissue (Itissue), wet
