Hearing loss following myringoplasty - implications for informed consent.

BACKGROUND: There are many reports of operations performed to successfully close ear drum perforations. Hearing deterioration after myringoplasty is not a widely published topic. This paper presents an audit of this complication. METHODS: A six-year retrospective analysis of a series of myringoplasty operations was performed using electronic patient records. Patients with post-operative hearing loss were identified and those with hearing loss greater than 10 dB were further scrutinised. RESULTS: Out of 187 patients who underwent myringoplasty procedures, 44 (23.53 per cent) experienced a red

Different aspects of informed consent in aesthetic surgeries.

Providing an informed consent has an important role in promotion of medical treatments and reduction of judiciary litigations in this process. Today with cultural changes and wide propagation that is usually charming, the request for aesthetic surgery has an increasing trend. These problems with complexity of cosmetic surgeries lead to deeper differences of information between plastic surgeons and patients, so the discussion on giving information to a patient is of great importance. Regarding the elective choice of aesthetic surgeries, there is a need on providing a standard informed consent f…

Orthognathic surgery: a new preoperative informed consent model.

The authors of this study propose a new informed consent form for orthognathic surgery. In a previous study from 2004, a careful review of the international literature and clinical practice suggested the feasibility of dividing the informed consent form into 2 parts, one describing diagnostic procedures and therapeutic/surgical times, and another describing possible problems. During these 4 years, we noticed that this model has 2 setbacks. We replaced point 2, "temporomandibular joint disorders," with "postoperative temporomandibular joint derangement," so that the surgeon is now supposed to i…

The effectiveness of patient verbalization on informed consent.

OBJECTIVE: To determine if preoperative patient verbalization of the risks and benefits of anterior cruciate ligament (ACL) reconstruction e … p received a standard surgical consultation, consisting of knee models, diagrams, open dialogue and informed consent to surgery. Eight patients in the experimental group were exposed to the same surgical consultation and were required to accurately verbalize the associated risks and benefits before operation. One month after informed consent was obtained, patients …

Study protocol for a randomised controlled trial of enhanced informed consent compared to standard informed consent to improve patient understanding of early phase oncology clinical trials (CONSENT).

INTRODUCTION: Early phase cancer clinical trials have become increasingly complicated in terms of patient selection and trial procedures-this is reflected in the increasing length of participant information sheets (PIS). Informed consent for early phase clinical trials has been contentious due to the potential ethical issues associated with performing experimental research on a terminally ill population which has exhausted standard treatment options. Empirical studies have demonstrated significant gaps in patient understanding regarding the nature and intent of these trials. This study aims to…

Transient or transformative? Psychedelics, agency and informed consent.

The literature on psychedelic experiences often highlights their transformative potential. This raises ethical questions about informed consent in psychedelic-assisted therapy: if the altered state of consciousness induced by psychedelic drugs can profoundly alter a person's value system in ways that are epistemically inaccessible to them beforehand, it is unclear whether their informed consent before the experience can be considered valid. I argue that this view stems from a simplistic understanding of psychedelic-mediated transformation, which overemphasises the power of the psychedelic expe…

Therapeutic effectiveness is however depende … 42% could not name one medication used in their IVT. 55% felt that the information provided during informed consent had been adequate. 69% did not know the post-surgical occurrence of endophthalmitis. Informed Consent Decision-Making in Deep Brain Stimulation.

Deep brain stimulation (DBS) has proved useful for several movement disorders (Parkinson’s disease, essential tremor, dystonia), in wh … mory, attention, and executive functioning, which may have an impact on the mental capacity to give informed consent to neurosurgery. Depression, anxiety, and compulsivity are also common in DBS candidate disorders, and could also be associated with an impaired capacity to consent to treatment or clinical research. Despite these issues, there is limited empirical …

Informed Consent in the Changing Landscape of Research.

The history of informed consent dates back as early as the 16th century (Selek, 2010). The current tenets of informed consent pertaining to the ethical conduct of research on human participants predominately stems from the 1947 Nuremberg Code (National Institutes of Health, 2016), which was created following the Nuremberg trials at the end of World War II. The unethical conduct of research on human participants during the Holocaust, coupled with experiments (e.g., the Tuskegee syphilis study), prompted a more formalized structure for ensuring the well-being and autonomy of human participants i…

Informed consent to breech birth in New Zealand.

The authors note significant room for improvement in facilitating informed consent in the management of breech presentation. New Zealand maternity care providers, including midwives, general practitioners and specialist obstetricians, have legal duties to provide full and unbiased information about risks and benefits of all relevant treatment options. In the case of breech presentation, such options include the interventions of external cephalic version or planned caesarean section, as well as the option to decline intervention and proceed with a planned vaginal breech birth. Information shoul…

Electroconvulsive therapy, the placebo effect and informed consent.

Major depressive disorder is not only the most widespread mental disorder in the world, it is a disorder on the rise. In cases of particular … ECT and given the risk of serious side effects that ECT may produce, I contend that the process of informed consent must include comprehensive accounts of these uncertainties. I examine the possible consequences of providing adequate information to potential ECT patients, including the consideration that ECT may still prove to be effective even if physicians are …

[What is and what is not informed consent].

Coequalitisation of the relation between the physician and his patient, which respects the professional dominancy of the first one, has promoted the full guidance before any serious diagnostic or therapeutic intervention. Patient has the right to be informed of his health status and of the therapeutic plan. He has also the right to refuse such information. Only after the full information, the patients can express his approval with the plan for therapy. Before any intervention, the patient's approval is necessary; exceptional are only by the law implicated situations. To get the lawful and ther

Valid informed consent: a process, not a signature.

Surgeons have dutifully used "consent forms" for most of the last century. But only in the last two decades have we begun to understand the difference between a patient's legalistically signed form and the meaningful process of communicating the reasons for considering surgical or other invasive intervention. Furthermore, we must be reasonably assured that the patient is legally competent, understands the situation and alternatives, and is not coerced (by family or medical professionals) in the decision-making. To accomplish this, time-consuming patient education and negotiation are necessary.

Standards for informed consent in recovered memory therapy.

Malpractice suits against therapists for either instilling or recovering false memories of sexual abuse have increased in the last few years and some of the awards have been large. Failure to give informed consent, that is, failing to inform patients concerning the risk of recovering false memories, is one of the main allegations increasingly made against therapists in recovered memory cases. In the landmark case on informed consent, Canterbury v. Spence fashioned a standard of disclosure that focused on how material the potential warnings were to the patient's decision and specifically stated…
