Cardiac mass assessment is challenging in the intraoperative scenario. We present the case of a 22-year-old woman with a new-onset left atrial mass during cardiopulmonary bypass following closure of a ventricular septal defect. We discuss the role of intraoperative echocardiographic examination, the differential diagnosis, and raise awareness of the left atrial appendage as a rare etiology of an acute left atrial mass.

We report a case of a patient who experienced awareness during general anesthesia with sevoflurane. A 71-year-old man, weighing 57 kg, was operated on for a malignant tumor of the parotid gland. BACKGROUND: Abdominal splenosis is a benign condition that occurs when splenic tissue is implanted within the abdominal cavity after splenic trauma or splenectomy. Most patients remain asymptomatic, and the condition is often detected incidentally. However, when multiple nodules are present, they can closely resemble peritoneal metastases and create uncertainty, particularly when they are encountered unexpectedly during surgery. Awareness during general anesthesia for cesarean delivery continues to be a major problem. The key to preventing awareness is strict attention to anesthetic technique. The prevalence and implications of aortocaval compression have been firmly established. Compression of the vena cava is a real occurrence when assuming the supine position. Relief of this compression most likely does not occur until the patient is turned 30°, which is not feasible for performing cesarean delivery. Although it is still wise to tilt the patient, the benefit of this tilt may not be as great as once thought.

An experience with two rare complications during surgery of vestibular schwannomas (VSs) is presented, and measures to avoid and manage the complications are discussed.Case A: Spinal cord ischemia in semi-sitting position: A 47-year-old with a giant vestibular schwannoma (VS) underwent surgery through a retrosigmoid approach in the semi-sitting position. The intraoperative phase was uneventful, except for an episode of moderate hypotension. Postoperatively, the patient woke up with quadriparesis. MRI on the cervical spine revealed restricted diffusion from C4 to C7 suggestive of cord ischemia.
