Bilateral herpes simplex keratitis presenting as peripheral ulcerative keratitis.

PURPOSE: To report a case of bilateral Herpes simplex keratitis (HSK) masquerading as peripheral ulcerative keratitis (PUK). METHODS: A case of a 47-year-old female complaining of painful red eyes with a history of arthritis and anterior uveitis attacks with positive antinuclear antibodies (ANA). Biomicroscopy revealed PUK, stromal infiltrations and bilateral central corneal epithelial erosions. Slit-lamp examination disclosed +3 anterior chamber cells in both eyes. RESULTS: Blood testing was positive for ANA. Herpes simplex virus (HSV) antigen was identified in both eyes using polymerase ch…

Viral Keratitis, Surgical Intervention in Viral Keratitis, Challenges in Diagnosis and Treatment of Viral Keratitis, HSV, HZV.

Viral keratitis is a significant cause of ocular morbidity and visual impairment worldwide. In recent years, there has been a growing understanding of the pathogenesis, clinical manifestations, and diagnostic modalities for viral keratitis. The most common viral pathogens associated with this condition are adenovirus, herpes simplex (HSV), and varicella-zoster virus (VZV). However, emerging viruses such as cytomegalovirus (CMV), Epstein-Barr virus (EBV), and Vaccinia virus can also cause keratitis. Non-surgical interventions are the mainstay of treatment for viral keratitis. Antiviral agents s…

Keratic Precipitates: An Overlooked Sign of Microsporidial Stromal Keratitis?

PURPOSE: To elucidate a distinctive clinical feature in cases of microsporidial stromal keratitis (MSK). METHOD: A retrospective observational study of cases with a histopathological and/or microbiological diagnosis of MSK on corneal biopsy or host corneal button between 2016 and 2022 was conducted. RESULTS: Eighteen cases with a confirmed histopathological and/or microbiological diagnosis of MSK were detected. Careful review of slit-lamp photographs revealed the presence of pigmented keratic precipitates (KPs) beyond the area of stromal keratitis in five out of eighteen cases (27.7%). CONC…

Herpes simplex virus keratitis mimicking Acanthamoeba keratitis: a clinicopathological correlation.

A 36-year-old male, soft contact lens wearer was referred by his primary ophthalmologist for corneal ulcer of the right eye (OD), which was … The postoperative immunohistochemical analysis suggested a diagnosis of herpes simplex virus (HSV) keratitis. The patient ultimately improved after initiation of oral valacyclovir following penetrating keratoplasty. We report a case of a commonly encountered clinical entity, HSV keratitis, with an atypical clinical presentation, masquerading as Acanthamoeb …

Atypical microbial keratitis.

Atypical microbial keratitis refers to corneal infections caused by micro-organisms not commonly encountered in clinical practice. Unlike infections caused by common bacteria, cases of atypical microbial keratitis are often associated with worse clinical outcomes and visual prognosis. This is due to the challenges in the identification of causative organisms with standard diagnostic techniques, resulting in delays in the initiation of appropriate therapies. Furthermore, due to the comparatively lower incidence of atypical microbial keratitis, there is limited literature on effective management…

Burkholderia cenocepacia keratitis.

A 33-year-old lady with history of failed keratoplasty for decompensated cornea due to childhood trauma and secondary glaucoma, post glaucoma drainage implant, with pseudophakia in the right eye, developed bacterial keratitis following foreign body trauma to corneal graft. Corneal cultures yielded Burkholderia cenocepacia identified by matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF- MS, bioMerieux, France). She healed with topical antibiotics (moxifloxacin 0.5%) in 1 month. Ours is the first report of ocular Burkholderia cenocepacia infection, possibly …

[Peripheral ulcerative keratitis].

Ulcerative keratitis is frequently associated with collagen vascular diseases and presents a predilection for peripheral corneal localization, due to the distinct morphologic and immunologic features of the limbal conjunctiva, which provides access for the circulating immune complexes to the peripheral cornea via the capillary network. Deposition of immune complexes in the terminal ends of limbal vessels initiates an immune-mediated vasculitis process, with inflammatory cells and mediators involvement by alteration of the vascular permeability. Peripheral ulcerative keratitis generally correla…

Tarantula hair keratitis.

We describe a 12-year-old boy in England with keratitis secondary to tarantula hairs embedded within the stroma of his cornea. Every attempt must be made to isolate these hairs at the first visit as they have a barbed nature and have a propensity to propagate through ocular tissues. A chronic keratitis requiring long-term steroid use may result if hairs persist in the cornea. Children who keep tarantulas as pets should be instructed on safe handling to prevent the tarantula from adopting defence mechanisms and shedding their hairs.

Brevundimonas diminuta keratitis.

PURPOSE: To report the first documented case of keratitis secondary to the gram-negative bacillus Brevundimonas diminuta. METHODS: A 66-year-old woman with an ocular history of bilateral soft contact lens wear presented with a large, 6×9-mm, culture-positive Pseudomonas aeruginosa-induced corneal ulcer. After weeks of therapy, she developed secondary keratitis. Repeat cultures grew a rare, gram-negative bacillus, B. diminuta. RESULTS: Targeted therapy followed by adjustments based on repeat bacterial cultures with sensitivities resulted in slow resolution of the ulcer. After months of therap…

Mycobacterium massiliense keratitis.

PURPOSE: The aim of this study is to report a case of keratitis caused by Mycobacterium massiliense, which is a subtype of non-tuberculous mycobacteria. METHODS: A 23-year-old Chinese man with a history of metal corneal foreign body removal was diagnosed with keratitis of his right eye 4 days after corneal trauma. He presented with intractable redness and continued corneal lesion with further vision loss after 2 weeks of conventional therapy. Pathogen testing and drug susceptibility testing were done. RESULTS: Corneal scraping and culture showed acid-fast staining positive bacilli. Polymeras…

Trichophyton fungal keratitis.

OBJECTIVE: To report 5 cases of fungal keratitis caused by Trichophyton schoenleinii. METHODS: Case reports. RESULTS: Trichophyton schoenleinii is responsible for aggressive fungal keratitis that is associated with keratolysis as a result of collagenase expression by the organism. Clinically, the organism seems to be sensitive to topical natamycin but resistant to topical amphotericin B and miconazole. Two patients achieved clinical cures with medical therapy alone, but with visual outcomes of only 20/125 and hand motions because of corneal scarring. Two patients achieved clinical cures with…
