[Single corrective osteotomy after knee para-articular fractures].

Post-traumatic malalignment can occur on one or several planes following fractures in close proximity to the knee joint. In a clinical and radiological analysis of deformities, the frontal, sagittal and longitudinal alignment, as well as differences in femoral or tibial length and torsion, must be taken into account. The location of the correctional osteotomy is usually defined by the center of the angular and torsional deformity. Of decisive importance when choosing the site of the osteotomy are the local quality of bone and the soft tissue envelope, the condition of the articular cartilage i

Early development of patella infera after knee fractures.

The condition referred to as patella infera is characterized by a permanent shortening of the patellar ligament and is associated with a severely limited range of motion of the knee joint. Patella infera is a common complication of injury or surgery to the knee joint, and it is generally considered a condition that presents after a variable, albeit considerable time after injury. A review of the knee roentgenograms of patients with arthrofibrosis and patella infera showed this condition to be an immediate complication of knee fractures. Therefore, in order to better define the development of p…

Fracture in an arthrodesed Charcot's knee joint.

Charcot's joint is a type of neuro-arthropathy, where asymmetrical damage of the involved joint happens haphazardly, without following any described pattern. We present a rare case of Charcot's joint involving the knee joint in an adult male following spina bifida. His knee joint was successfully fused after two surgeries. Later, he sustained a fracture through the arthrodesis after a fall, which we managed surgically. The fracture through the knee arthrodesis was managed surgically by an open reduction and internal fixation, using a 14-hole broad low contact dynamic compression locking plate

Fracture of the Fabella: An Uncommon Injury in Knee.

The fabella is a sesamoid bone that may contribute to the stabilization of the posterolateral knee corner and it can very occasionally act as a source of atypical and rare knee pain and functional impairment. Fracture of the fabella is a rare but important clinical entity which may be overlooked clinically and radiographically. However, it causes an intermittent mechanical pain of the knee and it can mistakenly harm another knee pathology like intra-articular loose body. We report a case of a 21-year-old man who was sustaining a fracture of fabella following vehicle accident.

[Ségond fracture in acute knee injuries].

Two patients with a lateral tibia avulsion fracture (Ségond fracture) are presented. The correlation between the Ségond racture and rupture of the anterior cruciate ligament is pointed out, as well as the fact that patients with Ségond fracture and rupture of the anterior cruciate ligament often have other injuries such as meniscus lesions and lesion of the medial collateral ligament.

Fracture of the femur after knee arthroplasty.

The supracondylar fracture of the femur is a serious complication after total knee arthroplasty. We report our experience in the management of 16 patients with this complication and suggest that the main line of treatment should be conservative. Open reduction and plate fixation is precarious. In displaced fractures, however, the Rush pin technique was useful.

Femoral-sided fracture-dislocation of the knee.

OBJECTIVE: To document a previously undescribed fracture-dislocation of the knee that involves a femoral condyle and associated ligaments yet spares the tibial joint surface. DESIGN: Retrospective. SETTING: University. PATIENTS: Four cases of fracture-dislocation of the knee that involve ligamentous injury and fracture of the femoral condyle(s). RESULTS: At a minimum two-year follow-up (average 28.8 months, range 26 to 37 months), overall functional outcome is only fair to good in this injury pattern. The Lysholm knee rating averaged 60 (range 39 to 74) and KT-1000 I-N testing averaged thr

Knee ligament injuries combined with ipsilateral tibial and femoral diaphyseal fractures: the "floating knee".

The incidence of rupture of the knee ligaments was retrospectively studied in 47 patients with ipsilateral fractures of the femoral and tibial diaphyseal shaft. Fifteen patients proved to have an instability of the knee at the time of follow up. Disruption of the knee ligaments had not been recognised initially. At the time of initial treatment, injury of the knee ligaments had been diagnosed in only three cases. After stabilisation of both fractures in these cases, the knee ligaments had been repaired; at re-examination these patients had no complaints and their knees were perfectly stable. I

Unexpected Positive Cultures in Hip and Knee Periprosthetic Fractures.

BACKGROUND: The purpose of this study was to determine the prevalence of unexpected positive cultures noted at the time of revision surgery for periprosthetic fracture (PPF) after total hip or knee arthroplasty. Moreover, we evaluated whether this finding should be considered clinically significant and what type of treatment, if any, was required. METHODS: This was a single-center retrospective review of 270 patients undergoing surgery for PPFs from December 2010 to December 2021. Exclusion criteria included: open fractures; history of infection at fractured joint; non-operatively treated pat

Early Femoral Insufficiency Fractures After Primary Total Knee Arthroplasty.

BACKGROUND: Periprosthetic femur fracture following total knee arthroplasty (TKA) is a devastating complication. Although trauma-related periprosthetic femur fractures have been well studied, early atraumatic insufficiency periprosthetic fractures (IPFs) are gaining attention. We present the largest IPF series to date to better understand and prevent this complication. METHODS: A retrospective study of all patients who underwent a revision surgery for periprosthetic fracture within 6 months of primary TKA between 2007 and 2020 was performed. Patient demographics, preoperative radiographs, imp

Current Reconstruction Options in Periprosthetic Fractures Around the Knee.

BACKGROUND: Periprosthetic fractures are a rare complication after total knee arthroplasty (TKA). However, the incidence of these fractures … yze the epidemiology, classification, diagnosis, surgical treatment, and outcomes of periprosthetic knee fractures in order to give an exhaustive overview. RESULTS: Reduction and internal fixation with locking plates or intramedullary nails represents the preferred option in case of a stable prosthetic implant. Conversely, in case of loose tibial and/or femoral …

Total knee arthroplasty and fractures of the tibial plateau.

Tibial plateau fractures are common injuries that occur in a bimodal age distribution. While there are various treatment options for displaced tibial plateau fractures, the standard of care is open reduction and internal fixation (ORIF). In physiologically young patients with higher demand and better bone quality, ORIF is the preferred method of treating these fractures. However, future total knee arthroplasty (TKA) is a consideration in these patients as post-traumatic osteoarthritis is a common long-term complication of tibial plateau fractures. In older, lower demand patients, ORIF is poten

Outcome of periprosthetic distal femoral fractures following knee arthroplasty.

INTRODUCTION: The majority of periprosthetic fractures around the knee occur at the supracondylar region of the distal femur. Fixation of distal femoral fractures in osteoporotic bone with short segment remains a challenge, especially after total knee arthroplasty (TKA). Internal fixation of these fractures using locking plates has become popular. The purpose of this study was to evaluate a consecutive series of periprosthetic supracondylar femoral fractures treated with locked periarticular plate fixation with regard to surgical procedure, complications and clinical outcome. Rehabilitation after knee immobilization in octogenarians with patellar fractures.

Between January 1990 and December 1999, 14 octogenarians (8 women and 6 men) underwent surgery for comminuted patellar fracture. Average patient age was 83 years (range: 80-88 years). Follow-up ranged from 1-8 years (mean: 3.5 years). Ten of 14 patients were totally independent, whereas 4 patients used a cane for mobilization. Twelve patients had background diseases. Patients underwent operative treatment with tension band wires followed by cast immobilization (knee in approximately 10 degrees of flexion) for 6 weeks. Immediate full weight bearing was initiated in all patients, and intense reh

Periarticular fractures after manipulation for knee contractures in children.

We report two cases, each of which sustained two separate periarticular fractures from overzealous manipulation for knee contracture. The four fractures reported in this study involve one normal child sustaining asynchronous ipsilateral distal femoral and proximal tibial fractures and a child with the diagnosis of amyoplasia sustaining bilateral proximal tibial fractures. The child with knee contracture must be treated carefully and not exposed to overzealous physiotherapy or manipulation. The child who has developed a joint contracture secondary to lengthy immobilization may be at increased r

Knee-ligament injuries associated with leg fractures. Prospective study.

It has been proved in 25-35% of the cases that knee-ligament injuries are associated with fracture of the femoral diaphysis. No such association has been confirmed between leg fractures and knee ligaments. In order to find out if this is a coincidence, a prospective study was conducted on 229 patients who had undergone operations for leg fractures at various locations and of variable intensity. It was established in 41 cases (17.34%) that the leg fracture was associated with knee-ligament injuries, resulting in joint instability. A significantly higher percentage of associated ligament lesions
