Treating leg ulcers.

The term ulcer is an all- embracing one. It simply means an erosion or loss of continuity of the skin or of a mucus membrane, usually with inflammation. Strictly speaking, a wound with superficial loss of tissue following trauma is not primarily an ulcer, but may become ulcerated if infection occurs. However, Abramowich ( 1987 ) quotes from a Scottish study which defines a chronic leg ulcer as 'an open sore below the knee, anywhere on the leg or foot which takes more than six weeks to heal'.

[Chronic leg ulcers].

Chronic wounds often manifest as ulcers on the lower legs. The main causes of chronic leg ulcers are chronic venous insufficiency and/or peripheral arterial occlusive disease in approximately 80% of all patients. However, there are also numerous other potentially relevant factors which can cause or almost impede the healing of chronic leg ulcers. Therefore, disease such as vasculitis, livedo vasculopathy, pyoderma gangrenosum, necrobiosis lipoidica, Martorell hypertensive leg ulcer, calciphylaxis, infectious diseases, neoplasms, drugs, cutaneous artefacts and genetic defects are exemplarily pr…

[A woman with an ulcer of the leg].

A 47-year-old woman with a history of ulcerative colitis and rheumatoid arthritis presented with a large ulcer with an erythematous halo of the right lower leg. The clinical course and the histopathological results were indicative of pyoderma gangrenosum.

Management of leg ulcers.

Chronic wounds, and venous ulcers in particular, are often managed by dermatologists. The first step in the management of any lower extremity ulceration is accurate diagnosis. Although there are several basic tenets of good wound care that remain static regardless of wound etiology, such as maintaining a moist and clean wound healing environment, many of the maneuvers that have been shown to improve the chances of healing must be individualized to the wound type. Many of the techniques in wound management have not changed over the past century: compression remains the cornerstone of venous ulc

Venous leg ulcers.

In 1837, Piorry, a French professor of medicine stated, "It is rather difficult to understand why the investigation of veins has been passed over almost in silence, while such a great diagnostic value has been attached to the investigation of arteries." Even today, our understanding of venous disease pales in comparison to our understanding of arterial disease. This is despite the fact that millions of Americans are afflicted with chronic venous insufficiency and hundreds of thousands suffer from debilitating lower extremity venous ulcers. A better understanding of the pathophysiology of venou

What's new: Management of venous leg ulcers: Treating venous leg ulcers.

Venous leg ulcers account for approximately 70% of all leg ulcers and affect 2.2 million Americans annually. After a comprehensive patient and wound assessment, compression therapy remains the cornerstone of standard care. Adjuvant care with topical or systemic agents is used for wounds that do not heal within 4 weeks. Once healed, long-term compression therapy with stockings or surgical intervention will reduce the incidence of recurrence. This continuing medical education article aims to outline optimal management for patients with venous leg ulcers, highlighting the role of a multidisciplin…

Malignancy and leg ulceration in a community-based leg ulcer clinic in New Zealand.

The high prevalence of non-melanoma skin cancers in this patient population highlights the need for early, accurate diagnosis. This can be achieved by biopsying atypical ulcers early and all chronic ulcers within three months of starting treatment.

[An extremely painful ulcer on the lower leg; Martorell arteriolosclerotic ulcer].

A 63-year-old woman was referred to the dermatology outpatient department with extremely painful ulcers on the right lower leg. Medical history listed hypertension, diabetes mellitus and chronic obstructive pulmonary disease. Intensive analgesia was insufficient. Blood analysis, microbial cultures and venous ultrasound did not reveal a cause. At histopathologic examination of an ulcer, arteriolosclerosis was revealed. The patient was treated for Martorell arteriolosclerotic ulcer with nifedipine, acenocoumarol and split-thickness skin grafts followed by vacuum-assisted closure therapy. Two wee

Leg ulcer with long-term hydroxyurea use.

Long-term use of hydroxyurea can cause leg ulcers which usually do not heal unless the drug is discontinued. Patients should be counseled regarding alternative lines of treatment like anagrelide and pegylated-interferon.

Vasculitic Leg Ulcer Revealing an Essential Thrombocythemia.

A 64-year-old man was referred to our department due to painful ulcers on the right leg that had evolved over the previous 6 months. There was also progressive weight loss. He had no relevant medical history. Clinically, we observed multiple ulcers, some of them with a necrotic base, located over the medial malleolus and calcaneus (Figures 1 and 2), with an associated livedoid appearance of the dorsum of the feet. A skin biopsy revealed epidermis with ulceration. The superficial and deep dermis showed perivascular and interstitial neutrophilic inflammatory infiltrate, with fibrinoid necrosis o
