Lichen planus and lichen sclerosus share several common features, including lymphocytic infiltration at the dermal-epidermal junction, clinical involvement of both skin and mucosa, erosive disease of mucosal surfaces, and the occasional occurrence of squamous cell carcinoma at the site of chronic, erosive mucosal lesions. In spite of these similarities, there are remarkably few reports of patients who concurrently have evidence of both diseases. Lichen planus and lichen sclerosus are common, chronic inflammatory vulvar dermatoses with significant morbidity. The course may wax and wane but disease often persists for decades. These autoimmune diseases have varied clinical presentations that extend beyond the genitalia. Management is best undertaken using a multidisciplinary approach and active patient involvement. Lichen planus is a common disorder affecting the skin, oral and genital epithelium, hair and nails. It is said to account for approximately 1% of new patient referrals to skin clinics (Samman, 1979). It is, therefore, an important entity to the dermatologist. However, lichen planus and lichen-planus-like reactions have been described in a number of clinical situations which may bring them first to the attention of doctors practising in other specialties.

Although reports in the literature are sparse, a definite relationship exists between nail anomalies and lichen planus. Ungual lichen planus may occur without any mucocutaneous signs, as has been documented by histologic studies. It may be the initial and most important clinical manifestation of the disease process, and permanent anonychia may result unless corticosteroid therapy is initiated promptly. Increased awareness of nail involvement will result in a more frequent diagnosis and better understanding of the disorder.
