Melanoma of the lip is a very rare condition that tends to behave in an aggressive manner. Surgery remains the mainstay of treatment. Five patients with melanoma of the lip are reported, illustrating some of the difficulties that may be encountered in its management. Radical local treatment is necessary to minimize the risk of local recurrence, and flap reconstruction will usually be required to achieve acceptable aesthetic and functional results.

Lip carcinoma is a relatively unique malignant disease because the lip is a junctional structure. It accounts for 12% of all noncutaneous head and neck cancers, yet has the lowest disease-related mortality rate among this group of cancers. The lip is bounded externally by the facial skin and is continuous with the buccal mucosa in the oral cavity. We reviewed 250 patients with carcinoma of the lip who were treated with radiation therapy during a 30-year period. Although radiation techniques underwent continued refinement, the radiation dose delivered to the tumor remained relatively constant. Nearly all tumors were squamous cell carcinoma (247/250) and located on the lower lip (240/250). Only 9% initially manifested lymph node metastases. The median survival was 13.8 years. Cleft lip or palate and lower lip pits are typical features of the autosomal dominantly inherited Van der Woude syndrome. Limb defects have not been reported in this syndrome so far. A girl with a unilateral complete cleft lip and palate, bilateral lower lip pits, and amniotic deformities of all four limbs is reported and the possibility of chance occurrence of cleft lip and palate, lower lip pits, and limb defects is discussed.
