Legionellosis (Legionnaires' disease and Pontiac fever) outbreaks have been associated with aerosols ejected from contaminated cooling towers--wet-type heat rejection units (WTHRUs) used to dissipate unwanted heat into the atmosphere. The Vermont Department of Health undertook a program to inventory, inspect, and sample all WTHRUs in Vermont from April 1981 to April 1982. All WTHRUs were sampled for Legionella pneumophila and data were obtained for location, design, construction, and operating characteristics. Of the 184 WTHRUs operating, statistical analyses were performed on those 130 which

Health care facilities are planning for changes in regulations regarding chlorofluorocarbon-based refrigerants. These changes are imminent, leaving little time to figure out what to do with equipment using these refrigerants. This article looks at the background of the CFC phaseout program and reviews options available to engineers in assessing their equipment.

In 2004, an outbreak of legionnaires disease occurred in a hospital in northern Italy with a water system that had been disinfected multiple times since 1990 and equipped with a continuous disinfecting system. Molecular typing linked the outbreak to contamination of the hospital water system and demonstrated the persistence of a predominant strain of Legionella pneumophila for 15 years.

Skin disorders were reported in 33% of catering staff and 35% of women cleaners who returned a questionnaire, and were employed in a large hospital. Hand dermatitis occurred in 15% of the caterers and 12% of the cleaners. In the majority, the dermatitis was irritant in origin and related to their wet work occupations. Cleaners had a high prevalence of jewellery dermatitis. Limited patch testing revealed a majority positive to nickel, but a third were negative, indicating that jewellery reactions often but not invariably predict nickel sensitivity. Few subjects were atopic, but some psoriatic p

The estates team at St George's Hospital in south London say the deployment of a web-based software system which verifies that 'responsible' staff across the site have regularly flushed low-use water outlets to maintain flow, and thus prevent potentially dangerous waterborne bacteria building up in pipework, has 'very substantially' increased compliance with approved flushing practice--as set out in the HSE's L8 Approved Code of Practice on controlling Legionella bacteria in water systems, and the new HTM 04-01, Safe Water in Healthcare Premises. As HEJ editor, Jonathan Baillie, reports, the h

The aim of this paper is to assess the activation phenomena and to evaluate the risk of external exposure and intake doses for the maintenance staff of two medical cyclotrons. Two self-shielded cyclotrons are currently operating in the facility for the routine production of (11)C and (18)F. Four radiochemistry laboratories are linked to the cyclotrons by means of shielded radioisotope delivery lines. Radiopharmaceuticals are prepared both for the PET Diagnostic Department, where four CT-PET scanners are operating with a mean patient workload of 40 d(-1) and for [(18)F]FDG external distribution

In a 30-month prospective study, we evaluated the efficacy of chlorine dioxide to control Legionella organisms in a water distribution system of a hospital with 364 patient beds and 74 skilled nursing beds. The number of hot water specimens positive for Legionella organisms decreased from 12 (60%) of 20 to 2 (10%) of 20. An extended time (18 months) was needed to achieve a significant reduction in the rate of Legionella positivity among hot water specimens. At the time of writing, no cases of hospital-acquired Legionnaires disease have been detected at the hospital since the chlorine dioxide s

A hospital maintenance employee developed an airborne contact dermatitis when cleaning respiratory therapy equipment. Patch-testing determined that she is allergic to glutaraldehyde, an ingredient in a popular commercial germicidal product. The increasing concern over acquired immunodeficiency syndrome (AIDS) and other infectious diseases has resulted in more intensive cleaning, sterilizing, and disposal procedures in health care facilities. This, in turn, may lead to an increased risk of contact dermatitis among health care workers. Glutaraldehyde is commonly used as a sterilizing agent in me

According to Alan Hambidge, who has over 22 years' experience in health and safety risk management, and is an expert in legionellosis and water hygiene, 'recent developments and research' have resulted in 'an increase in the Health and Safety Executive's expectations' on the standards adopted by healthcare organisations in looking after their water systems. In our latest technical guidance article, he explores 'some of the developments, pitfalls, and traps, that indicate the requirement for improvement in competence'.

BACKGROUND: Thermal treatment and copper-silver ionization are often used for controlling Legionella pneumophila in high-volume hospital plumbing systems, although the comparative efficacies of these measures in high-volume systems are unknown.
METHODS: Thermal treatment of a hot water circuit was accomplished by flushing hot water (> 60 degrees C) through distal fixtures for 10 minutes. Copper-silver ionization was conducted in three circuits by installing units into return lines immediately upstream from hot water tanks. Recovery rates of L. pneumophila were monitored by culturing swab samp

OBJECTIVE: To address the issues of high maintenance costs, patient care disruption due to equipment downtime, and low maintenance efficiency in large-scale hospital diagnostic equipment, this study aims to develop a maintenance strategy that is more aligned with actual clinical practices.
METHODS: First, the specific characteristics of diagnostic equipment maintenance in terms of patient safety, equipment quality control, and downtime impact are analyzed, and a single-component failure rate evolution model that accounts for equipment performance degradation is constructed. Second, a mixed ti

Several cases of Legionnaires' disease occurred in a French hospital in 1982. Thirteen strains of a legionella-like organism with several unusual characteristics were subsequently isolated from the hospital hot water system. The various features of these strains show that they are identical to the new species 'Legionella anisa' described by the Centers for Disease Control. The possible pathogenicity of these strains to man and their relationship with the recently described Legionella bozemanii serogroup 2 are discussed.
