The clinical course and causative organisms were studied in 18 patients with acute mastoiditis, 13 of whom (72%) had no previous history of middle ear disease. Their age ranged from 5 months to 21 years, and duration of middle ear symptoms immediately prior to admission ranged from 1 to 45 days (average 9.7 days). None had undergone a myringotomy prior to admission, while 13 (72%) had been receiving antibiotic treatment for acute otitis media. Three were admitted with intracranial complications. Bacteria were isolated in 10 of the 16 patients in whom samples were available for bacterial cultur…

A retrospective review of 11 cases of acute coalescent mastoiditis from 1974 to 1979 is presented. The common historical data include the recent onset of purulent otitis media, which was often incompletely treated with antibiotics, and fever, lethargy, and irritability. The eardrums were red, bulging, and usually intact. A postauricular abscess was present, and the affected ear protruded downward and outward. The mastoid radiographs invariably showed cloudiness and dissolution of the air cell septations. Initial therapy included paracentesis for culture and sensitivity, and initiation of paren…

Acute coalescent mastoiditis is an uncommon sequela of acute otitis media. It occurs principally in the well-pneumatized temporal bone. The findings of fever, pain, postauricular swelling, and otorrhea are classic. Cholesteatoma, on the other hand, being associated with chronic infection, usually occurs in the sclerotic temporal bone. The signs and symptoms are isidious in nature and consist of chronic discharge and hearing loss which result from its mass, bone erosion, and secondary infection. Of 17 consecutive cases of acute mastoiditis over a six-year period, four were atypical because they…

Seven cases of elderly diabetics suffering from ths so-called 'malignant external otitis' are presented. Chandler (1968) and others consider that this disease is localized to the external ear canal. We do not agree and believe that it is due to a masked middle-ear infection extending through the tympano-mastoid cells to the mastoid process, sparing the antrum and bulging into the floor of the external meatus. In time, extension to petrous apex results in petrositis and a fatal end in many cases.

Acute mastoiditis is an acute inflammation of the mastoid process with bone erosion. It is a complication of acute otitis media, which is rare but with increasing incidence. Distinct characteristics are an erythema and oedematous swelling of the skin of the mastoid process. A fluctuant swelling points to a subperiosteal abscess. Laboratory examination and imaging only support the diagnostics. Therapy involves obligatory systemic antibiotic treatment. At the beginning of the inflammation a paracentesis can be sufficient. A mastoidectomy must be carried out if clear signs of an osseous necrolysi…

We present a retrospective study of 37 infants who were operated for acute mastoiditis during the period 2000-2004 in Mother and Child Health Care Institute, Belgrade, Serbia and Montenegro. About 23 patients (62.2%) were male and 14 (37.8%) were female. Acute mastoiditis developed just after the first infection of the middle ear in 26 patients (70.3%). All patients had local and general symptoms. The most common local symptoms were blurred tympanic membrane in all patients, painful tenderness of mastoid in 21 (57%) and redness of tympanic membrane in 13 (36%). General signs of infection were …

We retrospectively studied seventeen cases of acute mastoiditis admitted to the Paediatric Ward of S. Francisco Xavier Hospital during 8 years and 8 months. Seven patients were admitted in 1995. We found no reason for this increase. Eleven children were male and 5 were under 2 years of age. All cases, except one of post-traumatic mastoiditis, occurred after acute otitis media. Only 5 children referred symptoms for more than seven days before admission. Seven patients had surgery. The surgical group of children had a longer period of illness when compared to those only treated medically (7.6 ve…

BACKGROUND: Sarcoidosis is a common granulomatous systemic inflammatory disease mainly affecting the interstitial lung tissue and perihilar lymph nodes. Since its first description by Bernier 1889 its etiology has remained obscure.
METHOD: Sarcoidosis of the right middle ear in a 38-year-old woman was diagnosed by biopsy.
RESULT: After mastoidectomy and treatment with prednisolone the patient has been free of relapse for two years.
CONCLUSION: According to our knowledge this is the second reported case of sarcoidosis of the middle ear. In both cases the interstitial lung tissue has been inv…

The diagnosis 'acute' mastoiditis is not an unambiguous entity. It contains both 'classical' and 'latent' mastoiditis. 'Classical' mastoiditis was often seen before the antibiotic era, had serious complications and was cured by mastoidectomy. After the introduction of antibiotics, the number of cases of 'classical' mastoiditis decreased and was replaced by a more prolonged condition called 'latent' mastoiditis. Lately in Oslo, we have seen an increase in numbers of 'classical' mastoiditis and at the same time a decrease in the incidence of 'latent' mastoiditis. A four year study of patients wi…

During the period from July 1979 to December 1979, the bacteriology of 35 consecutive patients undergoing surgery for acute mastoiditis was studied. The patients' ages ranged from 3 to 13 years. All patients presented with a purulent otorrhea and a postauricular subperiosteal abscess and were subjected to a mastoidectomy within 12 hours of presentation. Specimens were obtained intraoperatively from the mastoid cavity and, using appropriate technology, were cultured for aerobic and anaerobic organisms. Bacteria were isolated from specimens of 32 patients (91.4%); specimens from three patients (…
