Three-year stability of open-bite correction by 1-piece maxillary osteotomy.

INTRODUCTION: The purpose of this retrospective cephalometric study was to evaluate the long-term vertical stability of anterior open-bite correction by 1-piece Le Fort I osteotomy and rigid fixation. METHODS: The sample comprised 40 consecutively treated patients from the files of the Department of Orthodontics, University of Oslo, Norway. All subjects had received a 1-piece Le Fort I osteotomy as the only surgical procedure from 1990 through 1998 and were followed for 3 years according to a protocol for data collection. Lateral cephalograms were obtained before surgery and at 5 occasions af

Soft-tissue changes in association with anterior maxillary osteotomy: a pilot study.

INTRODUCTION: The purpose of this study was to investigate and predict soft-tissue changes in the forehead, nose, lips, and chin in association with anterior maxillary osteotomy. MATERIALS AND METHODS: The sample consisted of 20 adult patients (seven men and 13 women, 29.1 years of age immediately before surgery). Both hard- and soft-tissue changes were evaluated using a set of lateral cephalograms taken immediately before and at 7 months after surgery. Pearson correlation coefficients were computed to examine the relationship between hard- and soft-tissue changes. A prediction model was deve

Maxillary distraction osteogenesis using Le Fort I osteotomy without intraoperative down-fracture.

The aim of this study is to present a technique for maxillary distraction osteogenesis using Le Fort I osteotomy without down-fracture. Six cleft-related patients suffering from severe midfacial deficiency were treated with maxillary distraction osteogenesis. The RED II system was chosen as the extraoral device and the Leipzig retention plate system to anchor the maxillary segment. Maxillary distraction osteogenesis was successful in all cases. Cephalometric and clinical evaluation after an average follow-up period of 1 year showed stable results with respect to skeletal and dental relationshi

Single-Tooth Osteotomy Using Piezoelectric Devices to Treat an Ankylosed Maxillary Molar.

Single-tooth osteotomy is a surgical technique in which the tooth and adjacent bone with sufficient soft tissue are repositioned in a single step or moved orthodontically. It is not used in the maxillary posterior region because of poor accessibility, bleeding complications, and anatomical limitations such as the maxillary sinus. However, the development of piezoelectric surgical devices and the popularization of the sinus floor elevation procedure have simplified the approach to the posterior maxillary area. This article reports two cases of single-tooth osteotomy of ankylosed teeth that were

Outfracture osteotomy on lateral maxillary wall as a modified sinus graft technique.

PURPOSE: The purpose of this study was to provide feasibility of a new technique of sinus graft: outfracture osteotomy sinus graft. MATERIALS AND METHODS: The author modified the window opening method from the original trapdoor technique to an outfracturing and readaptation technique. It provides a few advantages in dental implant treatment of atrophic maxillary posterior edentulous alveolar ridges: 1) in combined cases of height and width problems; 2) in cases with anatomic problems such as sinus septum, thick lateral sinus wall, and intrabony bleedings; 3) possible use of the outfractured b

Influence of the maxillary sinus exposure in reduction malarplasty with an L-shaped osteotomy.

BACKGROUND: The zygomatic bone forms the prominence of the midface and plays an important role in determining the human facial attractiveness. The Oriental face is mesocephalic, with a prominent zygomatic body and wide zygomatic arch. Nevertheless, as an oval face is considered ideal, reduction malarplasty is always requested by Oriental people. Currently, L-shaped zygomatic osteotomy is often reported in literature yet no previous study has documented the exact outcome of this approach after exposure of the maxillary sinus. The objective of the study was to investigate the outcome and complic

An arteriovenous fistula of the maxillary artery as a complication of Le Fort I osteotomy.

INTRODUCTION: The Le Fort I osteotomy, one of the most common techniques used to correct dento-midfacial deformities, is generally considered to be operatively safe. However, sometimes this procedure can lead to fatal vascular complications. MATERIALS AND METHODS: This article describes a fifty-year-old woman with a giant facial venous malformation. The patient underwent a Le Fort I osteotomy in order to correct an open bite. The surgery was complicated by the development of an arteriovenous fistula (AVF) between the maxillary artery and the venous malformation, the diagnosis of which was mad

Cephalometric analysis of long-term airway space changes with maxillary osteotomies.

The cephalometric radiographs of 13 patients without sleep apnea who had undergone isolated maxillary surgery were studied. The authors found increases in both the nasopharyngeal and hypopharyngeal airway spaces 3 to 5 years after surgery. The results suggest that adaptive processes occur in both the upper and lower jaw, which contribute to an enlarged airway after surgery.
