Health Insurance and Trauma Outcomes: The Impact of Coverage on Survival and Recovery in Craniomaxillofacial Injuries.

Introduction Health disparities, particularly those related to insurance status, significantly impact trauma outcomes. Craniomaxillofacial (CMF) trauma patients without insurance often experience worse outcomes due to limited access to care and delayed treatment. This study examines the effects of insurance coverage on outcomes such as length of stay (LOS), survival, and surgical site infections (SSIs) among CMF trauma patients in Puerto Rico. Methods An institutional review board-approved retrospective analysis was conducted on 1,165 CMF trauma patients treated at the Puerto Rico Medical Cent

Association of Insurance Status with Patient Health at New Orleans Student-Run Free Clinics.

Since the closure of Charity Hospital after Hurricane Katrina, New Orleans Student-Run Free Clinics have helped fill the resulting void in healthcare access for the underserved New Orleans population. To better understand the health insurance status and health outcomes of this patient population, 1036 patient records from seven New Orleans Student-Run Free Clinics were collected and analyzed between February 2017 and March 2020. Insurance status was significantly associated with gender, race, homelessness, and prior incarceration, but not with education. Substance use rehabilitation centers ha

ASSESSING CATASTROPHIC HEALTHCARE EXPENDITURES IN THE EMERGENCY SURGICAL CARE OF CHILDREN WITH INTUSSUSCEPTION: INSIGHTS FROM A TERTIARY HOSPITAL IN NIGERIA.

BACKGROUND: Emergency and essential surgery should be affordable, accessible, and timely. However, this is not the narrative in our setting as patients present late with complications requiring operative treatment which has an attendant healthcare expenditure often tending towards household income depletion. This study assessed the proportion of patients who incurred catastrophic healthcare expenditures following operative treatment for intussusception in our facility. Catastrophic healthcare expenditure was defined as spending >10% of Gross Domestic Product per capita. Health care expenditure

Health workers' perceptions of access to care for children and pregnant women with precarious immigration status: health as a right or a privilege?

The Canadian government's recent cuts to healthcare coverage for refugee claimants has rekindled the debate in Canada about what medical services should be provided to individuals with precarious immigration status, and who should pay for these services. This article further explores this debate, focussing on the perceptions of healthcare workers in Montreal, a large multiethnic Canadian city. In April-June 2010, an online survey was conducted to assess how clinicians, administrators, and support staff in Montreal contend with the ethical and professional dilemmas raised by the issue of access

Cancer screening for undocumented-uninsured immigrants living with HIV. Results of a descriptive cohort study and 5-year follow-up.

BACKGROUND: Undocumented and uninsured immigrants living with HIV often face barriers to preventive services, including cancer screening. Evidence is limited regarding how structured screening can be implemented for this population. We evaluated the feasibility, clinical outcomes, and longer-term impact of a cancer screening program delivered through a voluntary HIV-Social clinic in Israel. METHODS: We conducted a single-center historic-prospective cohort study among undocumented and uninsured immigrants, aged 18 years or above, receiving care at the HIV-Social clinic at Sheba Medical Center.

Insurance Status and Quality of Care in Infective Endocarditis: A National Analysis of Disparities in Length of Stay, Discharge, and Mortality.

Background: Infective endocarditis (IE) requires 4-6 weeks of intravenous antimicrobial therapy, and timely transition to outpatient parenteral antimicrobial therapy (OPAT) allows clinically stable patients to complete treatment outside the hospital. Because OPAT requires home infusion services or post-acute facility placement that typically depend on coverage, insurance status may strongly influence length of stay (LOS); national data on this association in IE remain limited. Methods: We performed a retrospective cross-sectional analysis of the 2016-2019 National Inpatient Sample (NIS) using

The other health care system in Germany: care for people without health insurance.

BACKGROUND: A large number of people in Germany have no health insurance. Their access to the official healthcare system is significantly more difficult or impossible. Charitable institutions try to provide medical care and create parallel healthcare structures. Their possibilities and limits are still unknown. METHODS: This study identified 128 institutions for people without health insurance. A data query was used to determine the status of medical care for uninsured patients. RESULTS: Sixty-eight of 128 identified institutions participated in the study. They provided care to 26,300 people

Impact of Medicaid expansion on the multimodal treatment of biliary tract cancer.

INTRODUCTION: The impact of Medicaid expansion (ME) on the treatment of patients with cancer remains controversial, especially individuals requiring complex multidisciplinary care. We sought to evaluate the impact of Medicaid expansion (ME) on receipt of multimodal care, including surgical resection, for Stage I-III biliary tract cancer (BTC). METHODS: Patients diagnosed with BTC between 40 and 65 years of age were identified from the National Cancer Database and divided into pre- (2008-2012) and post- (2015-2018) ME cohorts. Difference-in-difference (DID) analysis was used to determine the i

Going the distance in acoustic neuroma resection: microsurgical outcomes at high-volume centers of excellence.

PURPOSE: High-volume hospitals are associated with improved surgical outcomes for acoustic neuromas (ANs). Due to the benign and slow-growing nature of ANs, many patients travel to geographically distant cities, states, or countries for their treatment. However, the impact of travel burden to high-volume centers, as well as its relative benefit are poorly understood. We compared post-operative outcomes between AN patients that underwent treatment at local, low-volume hospitals with those that traveled long distances to high-volume hospitals. METHODS: The National Cancer Database was used to a

Food Insecurity Was Associated With Greater Family Health Care Expenditures In The US, 2016-17.

Food insecurity has been associated with the health care expenditures of individuals, but it can affect the entire family. Evaluating the relationship between food insecurity and family expenditures provides a better understanding of the financial implications of food insecurity interventions. Our primary objective was to evaluate the association between food insecurity in one year (2016) and family health care expenditures-for all members, for children only, and for adults only-in the next year (2017). We also evaluated whether this association varied across types of insurance coverage within

Treatment Differences for Adrenocortical Carcinoma by Race and Insurance Status.

INTRODUCTION: To determine if treatment and clinical outcomes of adrenocortical carcinoma (ACC) vary by race and insurance status. METHODS: ACC patients from the National Cancer Database (2004-2017) were reviewed. Race was defined as White versus minority (Black and Hispanic). Insurance types were private (PI) versus other (Medicaid/uninsured/unknown). Metastatic ACC (M-ACC) was defined as distant metastases at the time of diagnosis; nonmetastatic ACC (NM-ACC) patient had no distant disease. RESULTS: Of 2351 NM-ACC patients, 83.6% were White and 16.4% minority. There were 1216 M-ACC patients

The Health Economic Impact of Nephrotic Syndrome in the United States.

BACKGROUND: Nephrotic syndrome (NS) is a rare kidney syndrome with high morbidity. Although a common contributor to the burden of chronic kidney disease, the direct and indirect costs of NS to patients and family caregivers are unrecognized. The objective was to characterize the direct and indirect costs of NS to patients. METHODS: Adults with NS and family caregivers of children with NS were eligible to participate if they had a diagnosis of primary NS, had disease for at least 1 year, and had no other severe health conditions. Data-collection surveys were generated with input from the Kidne

Mammography Screening Beliefs and Knowledge among a Multi-ethnic Sample of Low-income, Uninsured Women.

The purpose of this study was to examine screening mammography prevalence and its associated beliefs among a multi-ethnic sample of low-income, uninsured women. Data pertaining to the sample's demographic characteristics, mammography screening history and beliefs, and knowledge on recommended screening age were analyzed (n=533). Overall, 22.1% of the participants had never been screened. Black women were more likely than others to have never been screened, White women were more likely to be overdue, and Hispanic women were more likely to report recent screening. Fear of not knowing what will b

The effects of health insurance on the choice of medical procedures: Evidence from heart attacks and childbirths.

This paper investigates the effect of health insurance on the use of alternative procedures to treat a given medical condition. In particular, we estimate the effect of health insurance on the use of bypass surgery after a heart attack and on the use of a C-section after a normal pregnancy. These procedures are the most expensive, compared to the alternatives. Theoretically, the demand for some procedures like bypass surgery is likely to be inelastic. In this situation, health insurance should have no effect on the use of the procedure. For other procedures such as C-section, demand may be mor

Insurance among patients seeking care at a municipal sexually transmitted disease clinic: implications for health care reform in the United States.

INTRODUCTION: Limited data exist on insured patients who receive care at publically funded sexually transmitted disease (STD) clinics, despite having access to a primary care provider. In this analysis, we compare patients with and without health insurance who sought services at City Clinic, the San Francisco municipal STD clinic. METHODS: We analyzed San Francisco City Clinic patients between August 1, 2011, and December 31, 2012. Insurance was self-reported and included both private and public insurance. Variables from the clinic electronic medical record were examined and included basic de

Effect of a global longitudinal pathway on medical students' attitudes toward the medically indigent.

BACKGROUND: The increase in people with insufficient health insurance has largely been fueled by a record influx of immigrants, who often live in medically underserved areas of inner cities. An increase in the desire of medical students to practice in underserved areas is needed to counter low physician-to-population ratios in these areas. PURPOSE: To assess the effect of a Pathway on Serving Multicultural and Underserved Populations, which includes domestic and international experiences with recent immigrant groups, on the attitudes of students toward the indigent. METHODS: Students from th

Reforming 340B to Promote Program Integrity and Better Serve Vulnerable Populations: An American College of Physicians Policy Brief.

The United States pays some of the highest prices for prescription drugs in the world, with drug prices and overall spending increasing year-over-year. Ballooning prescription drug costs can result in low rates of medication adherence, negatively affecting public health and straining national health care systems and resources. The 340B Drug Pricing Program (340B Program), which allows health care safety-net settings to purchase prescription drugs at deep discounts, has long sought to function as a bulwark against increasing drug prices for institutions serving those most vulnerable in society.
