Biomechanical analysis for total mesialization of the mandibular dentition: A finite element study.

OBJECTIVES: To clarify the mechanics of tooth movement in mesialization of the whole mandibular dentition when changing the force angulation. SETTING: A finite element method was used to simulate long-term movements of the whole mandibular dentition. MATERIAL AND METHODS: Tooth movement was simulated by accumulating the initial displacement, which was produced by elastic deformation of the periodontal ligament. Mesial forces of 3 N were applied to the second molar bracket at -30°, -15°, 0°, 15° and 30° to the occlusal plane. RESULTS: The whole dentition and occlusal plane were rotated depen

Pre-eruptive movements of the mandibular third molar.

1. It would appear that the developing mandibular third molar is continually changing its angular position relative to the mandibular plane and adjacent teeth. 2. For normal uprighting and eruption to occur growth at the mesial root must dominate. 3. In the absence of sufficient space, uprighting movements of the mandibular third molar may result in mesioangular impaction. 4. Vertical and distoangular impactions can be explained by continued growth at the mesial root while the crown is prevented, at its distal surface, from erupting. 5. If growth at the distal root becomes more influential, se

A 3D comparison of planned versus achieved anterior tooth position in clear aligner treatment.

This study has compared the predicted anterior teeth alignment with the actual clinical results after the first set of aligners without refinement using digital intraoral scans and Clincheck digital models for evaluation. The sample included 40 adult individuals, with an average age of 27.8 years, treated with clear aligners for mild to moderate crowding. Initial and final intra-oral scans, along with predicted final digital models, were used for assessment. Four specific orthodontic tooth movements, vertical movement (including both intrusion and extrusion as linear displacements), along with

Third molar impaction in extraction cases treated with the Begg technique.

The aim of this study was to investigate the differences between two groups of patients with either erupted or impacted mandibular third molars before and after orthodontic treatment. All patients were treated with Begg appliances following extraction of the four first premolars. The erupted group consisted of 14 subjects and the impacted group 13 subjects. Lateral cephalometric films, peri-apical radiographs, orthopantomographs and orthodontic casts of each subject were taken before and after treatment. Thirteen measurements were carried out on the lateral cephalometric radiographs. The resul

Unilateral correction of space loss in the mixed dentition.

Distalization of maxillary molars is indicated for correction of Class II dental malocclusion and for space gain in cases of space deficiency. The ideal treatment with an intraoral fixed appliance for molar distalization should fulfill the following requirements: patient compliance; acceptable esthetics; comfort; minimum anterior anchor loss (as evidenced by inclination of incisors); bodily movement of the molars to avoid undesirable effects and unstable outcomes; and minimum time required during sessions for placement and activations. The purpose of this paper was to present an alternative tr

Protraction of mandibular molars through a severely atrophic edentulous space in a case of juvenile periodontitis.

Moving the mandibular posterior teeth into a severely atrophic edentulous space is a challenge. A carefully designed force-and-moment system that results in bodily protraction of the posterior teeth with balanced bone resorption and apposition is needed in such cases. This report describes the treatment of a 19-year-old woman with missing mandibular first molars due to juvenile periodontitis. Miniscrews were used as absolute anchorage during protraction of the mandibular second and third molars. Bodily mesial movement of the mandibular second and third molars was achieved over a distance of 11

Distalization of Maxillary First Permanent Molar by Pendulum Appliance in Mixed Dentition Period.

INTRODUCTION: Mesial drifting of molar teeth in maxillary arch is corrected by movement of the molars distally. In addition to traditional distal movement techniques, such as extraoral force application and removable appliances, various intra-arch devices have been introduced since 1980s. These intra-arch appliances have nearly eliminated the need for patient cooperation. CASE REPORT: The purpose of this paper is to report a case of 10-year-old male patient with loss of space in maxillary molar teeth treated by intra-arch appliance-pendulum appliance by distalization of maxillary first perman

Fixed retention of congenitally missing maxillary lateral incisors using a chairside, prefabricated fiber-reinforced composite bridge.

Adolescents with congenitally missing lateral incisors require an interim space maintainer to restore function and esthetics before craniofacial growth is complete and an endosseous implant can be placed. This article presents 2 patients, each with a missing lateral incisor, who underwent orthodontic therapy. A chairside, prefabricated fiber-reinforced composite bridge was used as a fixed space maintainer after the appliances were removed.

Interdisciplinary treatment of an end-to-end occlusion due to congenitally missing maxillary lateral incisors.

Odontogenic aplasia, or the congenital absence of permanent teeth, is relatively common. When the missing teeth include the maxillary lateral incisors, treatment considerations must reconcile both esthetic and functional objectives. In developing dentition, erupting adjacent teeth can drift from their intended positions into the edentulous space created by the congenitally missing tooth. When this movement happens (e.g., cuspids shifting more mesially than normal), esthetic and functional problems occur. A restorative solution for missing maxillary lateral incisors subsequent to the loss of th

Longitudinal post-eruptive mandibular tooth movements of males and females.

Unbiased estimates of post-eruptive eruption and migration of the mandibular teeth for large representative samples are presently unavailable. The purpose of this study was to evaluate pure tooth movements of untreated children and adolescents longitudinally. Lateral cephalograms of 214 French-Canadians, followed bi-annually between 8 and 15 years of age, were traced, and the positions of the mandibular permanent central incisors and first molars were digitized. Temporal changes in tooth position were evaluated relative to naturally stable mandibular reference structures, using the mandibular

Rapid orthodontic treatment after the ridge-splitting technique--a combined surgical-orthodontic approach for implant site development: case report.

This article presents a clinical case of bilateral partial edentulism in the posterior mandible with severe horizontal and moderate vertical bone atrophy. A new technique using rapid orthodontics after ridge splitting is presented. The split-crest technique was carried out using piezosurgical instruments in the first molar and second premolar areas to widen the bone crest and open a channel for tooth movement. Immediately after, orthodontic appliances were used to move the first premolars distally and the second molars mesially into the surgical site. The rationale was to facilitate and accele

Local delivery of osteoprotegerin inhibits mechanically mediated bone modeling in orthodontic tooth movement.

INTRODUCTION: The RANKL-OPG axis is a key regulator of osteoclastogenesis and bone turnover activity. Its contribution to bone resorption under altered mechanical states, however, has not been fully elucidated. Here we examined the role of OPG in regulating mechanically induced bone modeling in a rat model of orthodontic tooth movement. METHODS: The maxillary first molars of male Sprague-Dawley rats were moved mesially using a calibrated nickel-titanium spring attached to the maxillary incisor teeth. Two different doses (0.5 mg/kg, 5.0 mg/kg) of a recombinant fusion protein (OPG-Fc), were inj

Effects of low-level laser therapy on orthodontic tooth movement and root resorption after artificial socket preservation.

BACKGROUND: Low- level laser therapy has been used to stimulate the orthodontic tooth movements (OTM) previously. Furthermore, in the orthodontic treatments accompanying tooth extractions, the adjacent teeth move towards the extraction sites and close the space in some cases. Then, the adjacent tooth movements must be prevented in the treatments requiring space. Laser stimulates and at some doses decelerates tooth movement; it also improves healing process and enhances osteogenesis. Hence, it can prevent movement by osteogenesis adjacent to the tooth. The present study investigated the effects
