Metatarsalgia: distal metatarsal osteotomies.

Metatarsalgia is a common pathologic entity. It refers to pain at the MTP joints. Pain in the foot unrelated to the MTP joints (such as Morton’s neuroma) must be distinguished from those disorders, which lead to abnormal pressure distribution, reactive calluses, and pain. Initial treatment options for metatarsalgia include modifications of shoe wear, metatarsal pads, and custom-made orthoses. If conservative treatment fails, operative reconstructive procedures in terms of metatarsal osteotomies should be considered. Lesser metatarsal osteotomy is an effective and well-accepted method for the m…

Metatarsalgia: an ultrasound perspective.

Research on metatarsophalangeal joint instability and the plantar plate was conducted from May 2002 to May 2006. Along with plantar plate tears, we found a range of pathologies. This is a pictorial review of our experience.

Surgical treatment of metatarsalgia.

Surgical treatment may be needed where conservative treatment fails in the management of metatarsalgia secondary to pes planus transversus sometimes associated with shortening of the first metatarsal. The author's technique of operation was employed in 85 feet of 45 patients with satisfactory results in 89.4%. The operation consists of a single or double V-shaped osteotomy of the middle metatarsal bones performed in the dorso-plantar plane of the proximal metaphysis. This allows realignment of the metatarsal heads and additional shortening of these bones if necessary. This type of osteotomy is…

Metatarsalgia: diagnosis and treatment.

Metatarsalgia is a symptom, not a diagnosis. A careful study of ninety-eight patients who had complaints of pain in the fore part of the foot revealed twenty-three distinct diagnoses. These diagnoses were grouped as primary metatarsalgia, secondary metatarsalgia, and pain under the fore part of the foot. This division assisted in determining the form of nonoperative or operative treatment. The use of a pressure-sensitive cholesterol-crystal force-plate analysis or intra-articular or digital nerve blocks was helpful both in accurately pinpointing abnormal pressures and in determining whether op…

Metatarsal osteotomy for metatarsalgia.

An oblique osteotomy in the distal half of the metatarsal shaft is described for the treatment of metatarsalgia due to prolapse of one or more of the middle three metatarsal heads. Thirty-eight patients who have had this operation have been followed up for a period of from two to five years. The operation is simple, recovery is rapid and symptoms have been well relieved.

[The Weil osteotomy in metatarsalgia].

AIM: The purpose of the prospective study is to evaluate the results of Weil osteotomy in painful overloaded central forefoot with dislocation of the MTP joint. METHOD: 19 left and 16 right feet underwent the Weil osteotomy. In 10 patients the procedure was performed on both feet. In 15 cases only one metatarsal was corrected (the 2 (nd) in 12 and the 3 (rd) in 3 cases), in 17 cases both the 2 (nd) and the 3 (rd) metatarsals and in 4 cases also with the 4 (th) one together. The simultaneous correction of hallux valgus was performed in 19 forefeet. The mean follow-up was 31 months (range, 19 t

[Metatarsalgia: Differential diagnosis and therapy].

Metatarsalgia refers to localized or generalized forefoot pain in the region of the metatarsal heads. Symptoms can be isolated or in combination with accompanying deformities occurring in the forefoot and/or hindfoot. Anamnesis and clinical investigation usually yield to the diagnosis, the underlying cause on the other hand is not always easy to identify. In the foreground of the treatment is the exhaustion of conservative forms of therapy to minimize the symptoms of local pressure increase and callus under the metatarsal heads. In addition, various surgical methods are available, such as corr…

Differential diagnosis of lesser metatarsalgia.

Pain in the lesser metatarsophalangeal area of the foot remains a diagnostic challenge. This pathology often is attributed to neuromas. These painful lesions, although predisposed to this area, are not nearly as common as they are thought to be. The "systems approach" to diagnosis can be extremely helpful in establishing an accurate cause and subsequent proper diagnosis of lesser metatarsophalangeal joint pain.

Lesser metatarsalgia evaluation and treatment.

Anatomic and biomechanical aspects of the complex topic of metatarsalgia are reviewed. Cause is classified as structural, functional, and a combination of both. Evaluation of the lesion symptom complex is presented and light is shed on reasons for complications after metatarsal osteotomy. Ways to avoid complications are also discussed. The oblique osteotomy with rigid fixation performed at the distal surgical neck or the proximal metaphysis is recommended.

Treatment of Metatarsalgia with Proximal Osteotomies.

Metatarsalgia is among the most common sources of forefoot pain. Proximal metatarsal osteotomies are an important technique in the armamentarium of the surgeon treating metatarsalgia that has failed nonoperative management. Proximal osteotomies can provide powerful deformity correction with precise control to both shorten and elevate the metatarsal head. However, they can be technically challenging, difficult to attain satisfactory fixation, require increased postoperative immobilization, and can result in transfer lesions. There are numerous described techniques with little supporting evidenc…

Treatment of Metatarsalgia with Distal Osteotomies.

Many different distal metatarsal osteotomies have been described in the surgical treatment of metatarsalgia. The surgeon should use such osteotomies judiciously, and indeed, in the author's experience, they are infrequently required and are certainly not a first port of call. In cases where nonoperative treatments have failed, a thorough understanding of the causes of metatarsalgia and a detailed clinical assessment of the patient are essential if good surgical outcomes are to be achieved. If using distal metatarsal osteotomies as part of the surgical plan, then the author favors extra-articul…
