Severe akathisia as a side effect of metoclopramide.

CASE DESCRIPTION: A case of severe metoclopramide-induced akathisia in a breast cancer patient being treated with chemotherapy is presented, eventually culminating in hospital admission. In retrospect, this adverse effect was not recognized for several weeks because the prescription had not been properly recorded in the chart, the patient initially denied using the drug, and extensive psychological adjustment difficulties were also present. CONCLUSION: Movement disorders as an adverse effect of metoclopramide have been described on a regular basis over the past decades. Case reports such as t…

Galactorrhea with metoclopramide use in the neonatal unit.

We report a 3(1/2)-month-old infant with trisomy 21 presenting with galactorrhea in the neonatal intensive care unit (NICU). Endocrine work-up showed a high prolactin level (64.4 ng ml(-1)--normal: 0.5 to 30 ng ml(-1)). Cessation of therapy with metoclopramide (0.2 mg kg(-1) per dose q 6 h) resulted in the resolution of galactorrhea with a decrease in serum prolactin level (20.1 ng ml(-1)). We present this case to highlight this uncommon side effect of a commonly used medication in the NICU.

Metoclopramide for migration of naso-enteral tube.

BACKGROUND: Enteral alimentation by feeding tube is a common problem and an efficient method of providing nutritional support to hospitalized patients with insufficient oral intake, but adequate gastrointestinal function. The use of metoclopramide, a prokinetic agent, has been recommended to achieve transpyloric placement, but its efficacy is controversial. OBJECTIVES: To determine the effect of intravenous metoclopramide on transpyloric passage of the naso-enteral tube. SEARCH STRATEGY: Relevant RCTs were identified by electronic search through MEDLINE, EMBASE, LILACS and the Cochrane Contr…

Dihydroergotamine and metoclopramide in the treatment of organic headache.

Dihydroergotamine and metoclopramide have been used in the treatment of benign headache for many years. The presumed mechanism of action of dihydroergotamine and metoclopramide is related to these drugs' affinity for serotonergic receptors. We present three cases of the use of dihydroergotamine and metoclopramide in patients with organic headache (two patients with viral meningitis and one patient with meningeal carcinomatosis). All three patients had excellent symptomatic relief. Our results demonstrate that dihydroergotamine and metoclopramide can be effective in treating organic headache an…

[Use of metoclopramide (cerucal) in Tourette syndrome].

The action of cerucal was investigated in 22 children with Tourette syndrome. Positive effect was obtained in 59.1% of the cases. Personality types were described which presented a continuum between asthenic and psycho-asthenic+ poles. Cerucal proved effective in patients with psycho-asthenic+ traits.

Dystonic reactions with metoclopramide: is there a risk population?

The authors examine 18 cases of early dystonic syndromes and review all the pediatric cases published previously in order to determine the evolutive and epidemiological characters of these intolerance reactions. These unrecognized manifestations normally occur within the first 24 hours of treatment and disappear within a few hours when the treatment is discontinued. The use of diazepam and trihexyphenidyl accelerates the regression of the disorders. The doses of metoclopramide were mostly above the recommended doses, 60% of the cases correspond to doses in excess of 0.5 mg/kg per day or 0.2 mg…

Dystonic reactions in children caused by metoclopramide.

Five cases of alarming dystonic reactions are reported in children receiving a normal dosage of metoclopramide, a new anti-emetic drug. The symptomatology consisted mainly of neck pain, torticollis, retrocollis, oculogyric crisis, and trismus, and patients were admitted with presumed diagnoses of encephalitis, meningitis, and tetanus. After discontinuing the drug, all symptoms disappeared completely. We believe that metoclopramide should be used with caution in paediatrics and that all doctors should be familiar with its possible bizarre side-effects.

Metoclopramide-induced delayed non-thrombocytopenic purpuric rash.

A 62-year-old man with no drug allergies was admitted for coronary artery bypass grafting. Postoperatively, metoclopramide was commenced for the treatment of gastroparesis. Ten days after the drug was initiated a purpuric rash was noticed on his lower limbs. His platelet count was normal. Infection was excluded and the cause was not obvious. No change was noticed in the condition of rash despite discontinuation of all other medications started in the hospital one at a time. The rash later began to subside upon discontinuation of metoclopramide. A rechallenge was not attempted for ethical reaso…

Sensory trick with metoclopramide-associated tardive tremor.

Tardive tremor is a 3-5 Hz bilateral resting and action tremor, associated with the use of dopamine receptor blocking drugs, accompanied by other tardive movement disorders and responsive to tetrabenazine or clozapine. We describe a case of a sensory trick associated with tardive tremor which raises important points about semiology and management. First, the presence of a sensory trick with tardive limb tremor suggests that the disorder may be a form of dystonia. Second, further study of osteopathic manipulative therapy for treatment of dystonia or tardive tremor is supported by a symptomatic

Case of acute akathisia from intravenous metoclopramide.

Intravenous (IV) metoclopramide is a frequently prescribed medication in the emergency department (ED). Extrapyramidal side effects like tardive dyskinesia are known to develop with chronic use of metoclopramide, while acute akathisia is a lesser known side effect following IV administration. Akathisia is characterised by a sensation of restlessness and distress, as well as constant, non-purposeful limb movement. It can present as a diagnostic challenge, cause distress and hinder the management of the primary condition of the patient in the ED. However, akathisia can be readily reversible, wit…

Sumatriptan plus metoclopramide in triptan-nonresponsive migraineurs.

OBJECTIVES: We evaluated the effectiveness of combination treatment using sumatriptan plus metoclopramide versus sumatriptan alone for the treatment of acute migraine. The patients who were treated had failed to respond to triptans in the past despite adequate doses on at least 2 separate trials of the same triptan or 2 trials involving different triptans. BACKGROUND: There is limited evidence that dopaminergic antagonists may benefit the migraineur by relieving migraine pain and associated symptoms. The exact mechanism of action in migraine is unknown. The postulated action is the inhibition…

Tetanus-like syndrome secondary to metoclopramide administration.

A case of metoclopramide-induced pseudotetanus in a 24-year-old woman is described. Symptoms included flexor spasms of the neck muscles, neck pain, left deviation of the lower jaw and tongue protrusion. Tetanus was initially suspected. Mydriasis, hyperhydrosis and clinical observation indicated that this syndrome could be caused by metoclopramide taken for gastrointestinal symptoms. Orphenadrine hydrochloride, diazepam and ketoprofen completely resolved the symptoms. Pseudotetanus presents in many different ways, and it is sometimes very hard to distinguish it from infective tetanus, which is …

Respiratory failure following oral administration of metoclopramide.

OBJECTIVE: To report a case of respiratory failure, possibly due to anaphylaxis or asthma exacerbation, following the administration of metoclopramide. CASE SUMMARY: A 32-year-old white woman with a history of severe asthma and short-bowel syndrome was admitted for Hickman catheter line sepsis. Two doses of oral metoclopramide 10 mg in solution were administered for nausea and vomiting. Transient dyspnea followed the first dose of metoclopramide, but respiratory failure requiring intubation followed the second dose. DISCUSSION: Respiratory failure has been reported with metoclopramide-induce…
