Therapeutic decisions: assessing clinical fit.

Quality health care has been defined as the maximization of desired outcomes while minimizing undesirable consequences. Therefore, the optimal antimicrobial agent for a given clinical condition will be one that is the most rapidly effective, produces the least patient discomfort, results in minimal disruption of the patient's or hospital flora, and causes minimal dissatisfaction with the treatment program and its attendant costs. The clinical utility of antimicrobials is generally judged on the basis of in vitro activity, kinetic disposition, resistance trends, safety, and cost. Fluoroquinolon

[Comparative activity of tetracycline, doxycycline and minocycline against Haemophilus].

A total of 90 isolates of Haemophilus were studied for susceptibility to tetracycline, doxycycline and minocycline. In vitro activity was determined by agar dilution method (MIC) and disk diffusion test. Disk diffusion tests were done on chocolate agar and on supplemented Mueller-Hinton agar. MICs rang from greater than or equal to 128 to 0,25, 32 to 0,5 and 32 to 0,25 micrograms/ml for tetracycline, doxycycline and minocycline respectively. Strains can be divided into tetracycline-sensitive strains which are inhibited by concentration less than or equal to 2 micrograms/ml and tetracycline-res

Antifungal activity of bamemacrolactine C against Talaromyces marneffei and its possible mechanisms of action.

AIMS: The present study aims to investigate the in vitro antifungal activity and mechanism of action of bamemacrolactine C (BAC), a new 24-membered macrolide compound, against Talaromyces marneffei. METHODS AND RESULTS: The test drug BAC initially demonstrated antifungal activity through a paper disk diffusion assay, followed by determination of the minimum inhibitory concentration value of 35.29 μg ml-1 using microdilution. The association study revealed that combination therapy exhibited additive effects (0.5 < FICI < 1.0) when combined BAC with either amphotericin B or fluconazole. A time-

Comparison of the Sensititre YeastOne® and CLSI M38-A2 microdilution methods in determining the activity of nine antifungal agents against dermatophytes.

BACKGROUND: Dermatophytes are the most common fungal pathogens causing superficial infections in humans with a high prevalence worldwide. The treatment of these infections is based on the use of topical and systemic antifungal agents. A convenient method with a high predictive value for testing the susceptibilities of dermatophytes is necessary. OBJECTIVE: To evaluate the ability of the Sensititre YeastOne® in testing the activity of nine antifungal agents against dermatophytes. METHODS: We compared Sensititre® with reference procedure for anidulafungin (ANID), micafungin sodium (MCF), caspo

Kanglemycin A Can Overcome Rifamycin Resistance Caused by ADP-Ribosylation by Arr Protein.

Rifamycins, such as rifampicin (Rif), are potent inhibitors of bacterial RNA polymerase (RNAP) and are widely used antibiotics. Rifamycin resistance is usually associated with mutations in RNAP that preclude rifamycin binding. However, some bacteria have a type of ADP-ribosyl transferases, Arr, which ADP-ribosylate rifamycin molecules, thus inactivating their antimicrobial activity. Here, we directly show that ADP-ribosylation abolishes inhibition of transcription by rifampicin, the most widely used rifamycin antibiotic. We also show that a natural rifamycin, kanglemycin A (KglA), which has a

Antifungal activity of nikkomycin Z against Candida auris.

BACKGROUND: Nikkomycin Z is a competitive inhibitor of chitin synthase-an enzyme needed for synthesis of the fungal cell wall. Nikkomycin Z shows promise as a treatment for coccidioidomycoses and mixed activity has been described against other fungi and yeast. To our knowledge, it has not previously been tested against the emerging fungal pathogen Candida auris. OBJECTIVES: To determine the in vitro activity of nikkomycin Z against C. auris. METHODS: Nikkomycin Z was tested by broth microdilution against a panel of 100 isolates of genetically diverse C. auris from around the world. RESULTS:

[New antibiotics in infection treatment].

The introduction of new molecules with antimicrobial activity is an opportunity for modern antibiotic therapy in the era of rapidly growing resistance to antibiotics. Beta-lactamase-inhibiting antibiotics, cephalosporinases and carbapenemases represent a breakthrough in previous treatments for infections caused by resistant pathogens. Ceftazidime with avibactam and ceftolozane with tazobactam show exceptional efficacy in severe infections. Karbavans is a combination of meropenem and a new boron beta-lactamase inhibitor, as well as the combination of avibactam and aztreonam, are a hope for the

[Microbiological procedures for the diagnosis, management, and study of invasive fungal infections].

Invasive fungal infections (IFIs) are difficult to diagnose and cause a high mortality to an expanding spectrum of patients. Culture of clinical samples has limitations for the diagnosis of IFI and alternative procedures have been developed. Among them, serum determination of galactomannan or beta-1,3-d-glucan, and antimicelium and antimannan antibodies are relevant. The use of molecular procedures and mass spectrometry (MALDI-TOF) are encouraging tools for the optimization of the diagnosis and management of patients with IFI. The proposal of species-specific breakpoint to classify the isolate

Chapter 2: coccidioidomycosis.

Coccidioidomycosis is a systemic mycosis caused by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii. Infection is acquired by inhalation of infective arthroconidia that live in the soil. In 60% of cases, the infection is benign and resolves spontaneously. In the northern hemisphere, coccidioidomycosis is endemic to arid and semi-arid regions at latitudes between 40 degrees N and 40 degrees S, particularly in the southwestern United States and in northern Mexico. In the semi-arid northeastern region of Brazil, cases of coccidioidomycosis have recently been reported in four st

Emergence of black moulds in fungal disease: epidemiology and therapy.

Black moulds are a heterogeneous group of darkly pigmented (dematiaceous) fungi, widely distributed in the environment, that occasionally cause infection in humans. The clinical spectrum of infection includes mycetomas, chromoblastomycosis, sinusitis, and superficial, cutaneous, subcutaneous and systemic phaeohyphomycosis. During the last 2 years, there have been reports of infection caused by black moulds in previously healthy individuals and in immunocompromised patients, including an outbreak of fungemia in hospitalized patients. Molecular analysis of strains obtained from patients and from

Growth inhibition of Malassezia species by pharmacological concentrations of polidocanol.

In vitro antifungal properties of polidocanol (Thesi), a hydroxypolyethoxydecan, were investigated against various yeasts at concentrations of 0.05-10% (w/w) by means of the agar diffusion test; in the case of lipophilic Malassezia furfur, polidocanol-containing olive oil was used additionally for sensitivity study. Six strains of M. furfur of different clinical origin were tested as well as strains of C. albicans (3), C. krusei (3), C. parapsilosis (2), C. robusta (2), Tr. cutaneum (2) and one strain each of C. guilliermondii, C. glabrata, C. tropicalis and M. pachydermatis. Antimicrobial susceptibility of Rochalimaea quintana, Rochalimaea vinsonii, and the newly recognized Rochalimaea henselae.

The susceptibility of an isolate of Rochalimaea quintana, Rochalimaea vinsonii and the newly recognized Rochalimaea henselae to antimicrobial agents was determined by dilution in Mueller-Hinton agar supplemented with 5% sheep blood. Two inocula of 10(2) and 10(4) cfu were used and tests were read after incubation at 5 and 7 days at 37 degrees C in an atmosphere enriched to 5% carbon dioxide. Each isolate was susceptible to amoxycillin, third-generation cephalosporins, tetracyclines, macrolides and rifampicin, with complete inhibition of bacterial growth at concentrations lower than 0.125 mg/L.

In vitro susceptibilities of Mycoplasma genitalium to antibiotics.

The susceptibilities of seven clinical isolates of Mycoplasma genitalium and three strains of Mycoplasma pneumoniae to a variety of antibiotics were examined by an agar dilution method. Macrolides, pristinamycin, and tetracyclines were very active against both species. Sparfloxacin was the most active quinolone tested. None of the 21 antibiotics tested had differential activity toward the two organisms.

Ceforanide. A review of its antibacterial activity, pharmacokinetic properties and clinical efficacy.

Ceforanide is a 'second generation' cephalosporin administered intravenously or intramuscularly. It is similar to cefamandole and cefonicid in its in vitro superiority to 'first generation' cephalosporins against several species of Enterobacteriaceae as well as its activity against Haemophilus influenzae, including beta-lactamase-producing strains. Its activity against Staphylococcus aureus is less than that of cefamandole, cefuroxime and first generation cephalosporins. The in vitro activity against Neisseria gonorrhoeae is excellent. Pseudomonas, Acinetobacter and Serratia species, and Bacte

Population analyses of the susceptibility to ciprofloxacin of eight clinical strains of Pseudomonas aeruginosa.

Population analyses of the susceptibility to ciprofloxacin of eight strains of Pseudomonas aeruginosa, including mucoid strains and strains resistant to aminoglycosides or anti-Pseudomonas beta-lactams, were carried out. All strains were sensitive as judged by the broth-dilution technique, but four strains were found to yield resistant mutants with a frequency of less than 10(-6). Two strains yielded mutants homogeneously sensitive to ciprofloxacin at a level 8-16 times the MIC of the parent strains. Two other strains yielded mutants resistant to different higher concentrations of ciprofloxaci

[Antibiotic sensitivity of Pasteurella multocida and related bacteria (bacterial groups M5 and EF4). Studies of minimal inhibitory concentrations by agar dilution].

We have been isolating Pasteurella multocida and similar germs increasingly during the last few years: due to the rabies coming back in Eastern France, more consultations have been held following animal bites; samples are then taken for a bacteriologic research. We have studied their sensitivity towards 30 antibiotics. The determination of the MIC was achieved through the agar dilution method on 34 Pasteurella multocida of human origin issued after bites and expectorations, 4 EF4 and 4 M5. The Pasteurella are very sensitive to: beta-lactam antibiotics (the lowest MIC were observed for ureido-p
