Micronutrient status in phenylketonuria.

Patients with phenylketonuria (PKU) encompass an 'at risk' group for micronutrient imbalances. Optimal nutrient status is challenging particularly when a substantial proportion of nutrient intake is from non-natural sources. In PKU patients following dietary treatment, supplementation with micronutrients is a necessity and vitamins and minerals should either be added to supplement phenylalanine-free l-amino acids or given separately. In this literature review of papers published since 1990, the prevalence of vitamin and mineral deficiency is described, with reference to age of treatment commen…

Zinc: an essential micronutrient.

Zinc is an essential micronutrient for human metabolism that catalyzes more than 100 enzymes, facilitates protein folding, and helps regulate gene expression. Patients with malnutrition, alcoholism, inflammatory bowel disease, and malabsorption syndromes are at an increased risk of zinc deficiency. Symptoms of zinc deficiency are nonspecific, including growth retardation, diarrhea, alopecia, glossitis, nail dystrophy, decreased immunity, and hypogonadism in males. In developing countries, zinc supplementation may be effective for the prevention of upper respiratory infection and diarrhea, and

Anaemia and micronutrient deficiencies.

Anaemia in pregnancy is a common and worldwide problem that deserves more attention. For many developing countries, prevalence rates of up to 75% are reported. Anaemia is frequently severe in these situations and can be expected to contribute significantly to maternal mortality and morbidity. After a discussion of definitions, screening for anaemia and prevalence, the relationship between anaemia and maternal mortality and morbidity will be reviewed. Micronutrient deficiency and especially iron deficiency is believed to be the main underlying cause for anaemia. More recently the role of vitami

Micronutrient deficiency in children.

Malnutrition increases morbidity and mortality and affects physical growth and development, some of these effects resulting from specific micronutrient deficiencies. While public health efforts must be targeted to improve dietary intakes in children through breast feeding and appropriate complementary feeding, there is a need for additional measures to increase the intake of certain micronutrients. Food-based approaches are regarded as the long-term strategy for improving nutrition, but for certain micronutrients, supplementation, be it to the general population or to high risk groups or as an…

Chromium: an essential micronutrient.

A balanced, varied and "prudent" diet is likely to furnish the chromium requirement, whereas a high-fat, high-sugar, high-alcohol diet would not. A "prudent" diet will confer health benefits in addition to those derived form the adequacy of chromium intake, and it will obviate the need for supplementation with chromium compounds that are commercially available, but not scientifically proven to be effective. Chromium supplementation, other than by dietary means, should be used only when advised by and under the supervision of a physician.

Micronutrients and ICU-Acquired Weakness: A Narrative Review.

Intensive Care Unit-Acquired Weakness (ICU-AW) is a secondary neuromuscular disorder diagnosed during a critical illness and related to ICU stay and intensive treatments, once other possible etiologies have been excluded. Micronutrients are essential components of human nutrition that are needed in small amounts to maintain pivotal organ functions, but their role in critical care nutrition and in prevention and treatment of ICU-AW is unclear. This narrative review aimed to provide a synthesis of the current evidence on the potential role of micronutrients in ICU-AW, with a glance at what is kn…

Global impact of micronutrients in modern human evolution.

Micronutrients are essential components of the human diet, but dietary levels above or below their narrow, recommended range are harmful. Deficiencies increase the risk of stunted growth and metabolic, infectious, and respiratory disorders, and have likely been pervasive in human history, as local soils poor in micronutrients are widespread. Deficiencies are also common today, affecting approximately 2 billion people. Limited evidence exists for selenium, zinc, iodine, and iron deficiencies driving local adaptation in a few human populations, but the broader potential role of micronutrients in…

LLL 44 - Module 3: Micronutrients in Chronic disease.

Micronutrients (MN), i.e. trace elements and vitamins, are essential organic molecules, which are required in the diet in relatively small amounts in any form of nutrition (oral, enteral, parenteral). The probability of MN depletion or deficiencies should be considered in all chronic illnesses, especially in those that can interfere with intake, digestion, or intestinal absorption. Low socio-economic status and food deprivation are recognized as the most prevalent reasons for MN deficiencies world-wide. Elderly multimorbid patients with multimodal therapy, as well as patients with long-lasting…

Host-pathogen interactions: can micronutrients tip the balance?

Nutrients are essential to the human host and to its invading pathogens. The purpose of this International Nutrition Council symposium on Micronutrient Regulation of Host-Pathogen Interactions held at Experimental Biology 2006 was to examine new knowledge about the mechanisms by which certain limiting micronutrients can mediate the balance of power between the human host and its numerous potential pathogens. In this introductory article, we briefly review how competition for nutrients is critical to the survival of both host and pathogen and describe some of the evolved mechanisms by which eac…

Micronutrients and reproductive health issues: an international perspective.

Micronutrients may have a role in enhancing reproductive health of women living in the developing world. Two illustrative micronutrients, zinc and vitamin A, have received some attention in this regard. Numerous animal experiments and observational studies suggest the potential role of zinc deficiency in labor and delivery-related complications such as premature rupture of membrane, placental abruption, preterm labor and inefficient uterine contraction. These associations have not been confirmed in supplementation studies. Zinc does not appear to be a limiting factor in intrauterine growth in …

Newer aspects of micronutrients in chronic disease: copper.

Cu ions are pro-oxidants when added to biological material in vitro and excessive levels of Cu in the body, such as in Wilson's Disease (Yarze et al. 1992) promote oxidant-related pathologies. In contrast there is now substantial evidence that an optimum level of Cu is required to maintain antioxidant defence and that Cu deficiency in animals increases oxidant stress. There are abundant mechanistic relationships linking Cu deficiency and processes associated with IHD, some of which do not directly involve oxidant damage. These mechanistic relationships, however, have mostly been demonstrated i

Enhancement of methanogenic activity by micronutrient control: Micronutrient availability in relation to sulfur transport.

The main purpose of this research was to clarify the influence of the addition of iron (Fe) alone (0-100 mg/L) or 50 mg/L of Fe with 2 mg/L … 2S in both the aqueous and gaseous phases as well as the minimization of added amounts of all other micronutrients to fulfil the sufficiency of all micronutrients for anaerobic digestion (AD). The addition of 2 mg/L each of Co, Cu and Ni together with 50 mg/L Fe resulted in the greatest enhancement in process performance, as indicated by the improved CH4 yield ( …

Ultrasonication-mediated multi-micronutrient fortification of polished rice to address micronutrient deficiencies.

Ultrasonication-mediated multi-micronutrient fortification of polished rice was evaluated as a rapid and efficient strategy to enhance micronutrient loading and bioaccessibility. A one-pot process integrating soaking and sonication was optimized using response surface methodology (RSM), yielding optimal conditions of 2.73 min sonication, 5000 ppm fortificant concentration, and 240 min soaking time. Under these conditions, fortified rice showed markedly higher micronutrient concentrations than non-fortified controls, including iron (2802.42 ± 31.25 µg/g; ∼407-fold increase), zinc (813.05 ± 14.2

[Micronutrients and phytotherapy in the response to stress].

Introduction: micronutrients fulfil essential functions as cofactors and regulators of the metabolic pathways involved in the response to stress. When present in appropriate quantities, they help to maintain neuroendocrine and immune homeostasis in situations of high physiological demand. Adaptogens can normalize biological functions and strengthen systems affected by stress, protecting the organism against psychological and environmental insults. Objectives: to evaluate the bidirectional relationship between micronutrients and stress, with emphasis on the role of adaptogens. Methods: we revie…

LLL 44 - 2 - Micronutrients in clinical nutrition: Vitamins.

Vitamins are essential organic molecules, which are required in the diet in relatively small amounts in any form of nutrition (oral, enteral, parenteral). Despite the small amounts that are required, the vitamins are essential both for maintenance of health, growth, and treatment of disease. After reminding about the principal function of all the vitamins, their needs and the clinical consequences of their deficit, the text present some common clinical problems: the impact of inflammation on the assessment of status. The reasons and diseases which cause increased requirements are presented, wi

An update on essential micronutrients in critical illness.

PURPOSE OF REVIEW: Numerous micronutrients are involved in antioxidant and immune defence, while their blood concentrations are frequently low in critically ill patients: this has fuelled many supplementation trials. Numerous observational, randomized studies have been published, which are presented herein. RECENT FINDINGS: Micronutrient concentrations must be analysed considering the context of the inflammatory response in critical illness. Low levels do not always indicate a deficiency without objective micronutrients losses with biological fluids. Nevertheless, higher needs and deficiencie…
