PURPOSE: To quantitatively assess pupillary functions after small pupil cataract surgery using the flexible iris retractor.
METHODS: Subjects were 11 patients (12 eyes) with small pupils who underwent phacoemulsification and intraocular lens implantation. Pupils were enlarged using the flexible iris retractor intraoperatively, and postoperative iriscorder data were compared with the data of 20 normal controls who underwent standard phacoemulsification and intraocular lens implantation.

The history, mechanism and aetiology of Horner'sy Syndrome is presented and the pharmacology of the pupil is discussed. The case reported is a rare combination of Horner's Syndrome in a patient who sustained bilateral fractures of the mandible and a chest injury. It is emphasised that the miotic changes in Horner's Syndrome, in combination with head injuries can lead to confusion in diagnosis and the potential anaesthetic hazards and their influence on the management of the facial injury are outlined.

Twenty-one patients had esotropia diagnosed before age 12 months. A group of 40 postoperative cataract patients had Miller-Nadler glare testing performed before and after midpupillary pharmacologic dilation. The results indicated that pupillary enlargement is associated with increased glare disability. For that reason it is essential to maintain normal pupil shape and function after cataract surgery, a difficult task in cases with miotic pupils.

Horner syndrome consists of unilateral ptosis, an ipsilateral miotic but normally reactive pupil, and in some cases, ipsilateral facial anhidrosis, all resulting from damage to the ipsilateral oculosympathetic pathway. Herein, we review the clinical signs and symptoms that can aid in the diagnosis and localization of a Horner syndrome as well as the causes of the condition. We emphasize that pharmacologic testing can confirm its presence and direct further testing and management.

A case of symptomatic herniation at the first thoracic disc level is reported. The patient presented with hand weakness, Horner's syndrome, and pain radiating along the medial aspect of the upper extremity. Myelography demonstrated a smooth lateral defect at T1-2. Three sequestrated epidural disc fragments were removed with postoperatvie relief of pain. A slightly miotic pupil remains.

BACKGROUND: A 69-year-old man presented with malignant glaucoma 10 days after an iridotomy for acute angle-closure glaucoma.

A modified filtering operation for neovascular glaucoma provided control of intraocular pressure after 12 to 23 months in ten of 13 eyes. After long-term follow-up, 23 to 54 months, seven of ten eyes still had controlled IOP. The effects of intravenous naloxone on several of the actions of intravenous morphine (mean dose 30 mg/70 kg) were studied in six volunteer subjects. Naloxone produced a well defined reversal of the respiratory depression, analgesia, and miotic and subjective effects of the morphine. The agonist action of morphine outlasted the antagonist action of a single dose of naloxone. The effect of repeated doses of naloxone was also short-lived, but continuous infusions were effective in maintaining reversal.

The Argyll Robertson pupil, a miotic pupil that fails to react to direct light, has been described for more than a century. Originally associated with tabes dorsalis, the sign has now been found in a number of conditions with lesions in the area of the nucleus of Edinger-Westphal. Magnetic resonance imaging studies have localized the lesion in patients with sarcoidosis and multiple sclerosis. With the declining incidence of neurosyphilis, the sign is increasingly likely to indicate another cause, although an assiduous search for lues should also be undertaken.

Low-luminance myopia of subjects with one normal eye and one aphakic eye was measured with a laser optometer. Low-luminance myopia was registered in the normal eyes, but no such myopia could be found in the aphakic eyes. The same subjects' aphakic eyes were further tested with miotic and dilated pupils. Miosis or mydriasis did not significantly alter the results as compared with measurements obtained with normal pupil size. These results would appear to be the first direct experimental evidence that accommodation is the primary cause of low-luminance myopia.
