Significant improvement in symptoms of disabling cramps and muscle spasms was obtained in 9 patients with motor neuron diseases, tetany, and myotonic disorders who were treated with tocainide, a lidocaine analog. No significant side effects were observed except for light-headedness and fatigue in 1 patient, who also showed slight prolongation of intraventricular conduction time. Eleven patients with writer's cramp were studied during the performance of a series of writing and non-writing tasks. Their performance in the tasks and the intensity of contraction in the flexor and extensor muscles of the fingers and elbow (recorded electromyographically) were compared with the results from a group of matched normal controls. Reciprocal inhibition of H reflexes in the forearm flexor muscles was examined in a group of 16 patients with writer's and other occupational cramps. PURPOSE: Muscle cramping due to peripheral nerve hyperexcitability (PNH) is poorly characterized. This retrospective study examines the prevalence of PNH and response to treatment.
METHODS: The Duke EMG Database was queried to identify patients with muscle cramping tested for PNH from 2010 to 2015. Peripheral nerve hyperexcitability was defined by compound muscle action potential after-discharges on repetitive nerve stimulation. Positive muscle phenomena are due to muscle overactivity. Examples are cramp, myalgia, and stiffness. Avulsion of a muscular branch of a major artery without a history of major trauma has not been reported to our knowledge. Occasionally, blunt and even minor trauma can result in injuries that seem out of proportion to the level of injury. We report a case of an avulsed muscular branch of the superficial femoral artery in a patient with recent thigh cramping. This injury is likely related to the intense tetany the patient described having before he came to the hospital.

We report a case of thyrotoxicosis with prolonged post-treatment muscle cramp and hypocalcemia. A 36 year-old woman with hyperthyroidism was treated with Methimazole (MMI). As plasma levels of T4 and T3 were normalized, hypocalcemia was noted and severe cramp of skeletal muscle appeared so that the patient was unable to walk. We studied a woman who had evidence of myasthenia gravis at age 17. At age 19, she gradually developed alopecia and painful muscle spasms (Satoyoshi disease). Deposition of immune complexes (IgG, C3, and C9) was demonstrated at the motor endplates of limb muscle (biceps brachii); primary synaptic clefts were widened, and postsynaptic folds were simplified. This is the first case of myasthenia and Satoyoshi disease in the same patient.
