Musical hallucinations in a musician.

The experience of music is difficult to study objectively. Here we describe a detailed analysis of musical hallucinations developing after a probable brainstem stroke in an 83 year old musician who was able to describe and notate the hallucinations. The hallucinations comprised simple, repetitive melodic and rhythmic motifs that were combined apparently randomly without definite tonality, large-scale structure, or timbre. This observation is consistent with the proposal that musical hallucinations represent abnormal spontaneous activity in auditory cortical areas beyond the primary auditory co…

Neuropsychology: music of the hemispheres.

Music may be the food of love but it is also good fodder for cognitive scientists. Here we highlight a recent study of a neuropsychological patient who has lost her ability to read music, but not text, in the absence of any other musical deficit.

A case of musical agraphia.

Damage to the left upper parietal lobule causes pure agraphia. However, we experienced a patient who exhibited musical agraphia following such a lesion after the agraphia improved. The patient was a 53-year-old female piano teacher. After surgery, she exhibited agraphia and musical agraphia. There was no expressive amusia, receptive amusia, aphasia, agnosia or apraxia. Fifteen months post-surgery, when her agraphia had resolved, her abilities to read, write, and copy music were evaluated. She could read and write single notes and musical signs, but her ability to write a melody was seriously i…

Musicality is Preserved in Neurodegeneration.

Certain musical abilities can endure even as language, memory, and behavior decline in dementia, yet the neural basis of this resilience remains poorly understood. We draw on behavioral and neuroimaging evidence to explain why musicality is selectively preserved or impacted across Alzheimer's disease (AD) and the frontotemporal dementias (FTDs). Adopting a network-based perspective, we describe how musicality arises from interactions across large-scale brain systems that support perception, emotion, and memory. Evidence from case studies and neuroimaging work suggests that music engages lower-…

Hearing and music in dementia.

Music is a complex acoustic signal that relies on a number of different brain and cognitive processes to create the sensation of hearing. Changes in hearing function are generally not a major focus of concern for persons with a majority of neurodegenerative diseases associated with dementia, such as Alzheimer disease (AD). However, changes in the processing of sounds may be an early, and possibly preclinical, feature of AD and other neurodegenerative diseases. The aim of this chapter is to review the current state of knowledge concerning hearing and music perception in persons who have a demen…

Neurology of musical performance.

Performing music at a professional level requires the integration of multimodal sensory and motor information and precise monitoring of the performance via auditory feedback. In the context of Western classical music, musicians are forced to reproduce highly controlled movements almost perfectly with a high reliability. These specialised sensorimotor skills are acquired during extensive training periods over many years. The superior skills of musicians are mirrored in functional and structural plastic adaptations of sensorimotor and auditory systems of the brain. Auditory-sensorimotor integrat…

Dyslexia and musical notation.

The authors examine the difficulties experienced by dyslexic musicians in the formalized study of music, in particular, musical notation. They describe case studies from the literature and from personal interviews they conducted with musicians about their educational histories, musical weaknesses and strengths, and successful compensatory strategies. The authors make instructional suggestions for educators and musicians with dyslexia on how to use multisensory approaches to teach musical notation.

Music Therapy in Patients Undergoing Pancreatic Surgery (MUSIC PUPS): A Mixed Methods Pilot Study.

BACKGROUND: Pancreatoduodenectomy (PD) and distal pancreatectomy (DP) are painful procedures often accompanied by psychological distress. Music therapy interventions such as music-assisted relaxation and imagery (MARI) have demonstrated efficacy for acute pain but have not been examined within PD/DP. Gene expression mechanisms by which MARI may affect pain also remain poorly understood. OBJECTIVE: This study assessed the feasibility and acceptability of administering MARI, collecting dried blood spots (DBS), and collecting mobile-device patient-reported outcomes (MDPRO) among adults undergoin…

Music Listening Habits and Music Perception Abilities of Prelingually Deafened Adolescent Cochlear Implant Recipients.

BACKGROUND: Music is a very important art form that can evoke strong emotions and involvement with it can have a considerable impact on cochlear implant (CI) recipients' quality of life. It is well known that music perception is limited in CI with fully electric hearing and CI recipients have significantly poorer music-appraisal ability when compared with normal hearing subjects. Considering the importance of music on the life of an adolescent and negative impact of hearing loss on mental health, understanding and improving music listening habits and perception abilities can have a positive im…

Music Is Quasi-Rhythmic Too: Comparing Bandwidth in Speech and Music Acoustic Modulation Spectra.

Speech and music both unfold over time. However, these dynamics are perceived quite differently: music as rhythmic, and speech as quasi-rhythmic. Here, we aim to identify whether this distinction between rhythm and quasi-rhythm is sourced from the raw acoustic waveform by analyzing the modulation spectrum of the acoustic envelope. We analyzed several large corpora on three scales: the coarsest scale containing recordings of each corpus, the intermediate scale of individual speakers/songs, and the finest scale of individual sentences or short musical segments. We confirm previous findings that …

The language network does not support music perception across a range of music experience.

UNLABELLED: Music and language share many similarities, yet music does not elicit a response in the language brain areas in adults with typical amounts of music experience, and patients with aphasia show preserved music ability. However, this dissociation might not generalize across development or expertise: it may not yet be present in children, who have limited experience with language and music, or it may be diminished in musicians with extensive music training. To evaluate these possibilities, we here used a precision fMRI approach to examine the responses in the language areas of children…

Digital music intervention and music therapy across the dementia care continuum: Randomized controlled trial.

INTRODUCTION: Music interventions show promise in dementia care and digital approaches may increase availability and accessibility. We investigated the efficacy of a digital music intervention (DMI) and conventional music therapy (CMT) compared to standard care (SC) across the dementia care continuum. METHODS: In an exploratory parallel-group randomized controlled trial, community-dwelling (HOME, n = 91) and care home-dwelling (CARE, n = 75) persons living with dementia (n = 166) were randomized to DMI, CMT, or SC. Group-based music interventions spanned 10 weeks (2 × 60-minute sessions/week)…

Music recharges people: Synchronized music during aerobic exercise leads to better self-regulation performance.

Previous studies have demonstrated that music has a positive effect on individuals during exercise and sports. We speculate that one of the mechanisms for this positive effect may be that music reduces the consumption of self-regulation strength. The primary objective of this study was to use a self-regulation strength model to explain the impact of music on individuals during aerobic exercises. Specifically, we examined the effects of synchronous music on college students' depletion of self-regulation during aerobic exercises. The participants underwent a pre-test in which they had to maintai…

Decoding selective auditory attention to musical elements in ecologically valid music listening.

Art has long played a profound role in shaping human emotion, cognition, and behavior. While visual arts such as painting and architecture have been studied through eye-tracking, revealing distinct gaze patterns between experts and novices, analogous methods for auditory art forms remain underdeveloped. Music, despite being a pervasive component of modern life and culture, still lacks objective tools to quantify listeners' attention and perceptual focus during natural listening experiences. To our knowledge, this is the first attempt to decode selective attention to musical elements using natu…

The Effect of Guided Imagery and Music with Sasando Music on Anxiety among Ischemic Stroke.

BACKGROUND: Guided Imagery and Music (GIM) interventions reduced the anxiety in many clinical settings. Traditional music pictures the emotion of specific culture. No interventional study was found in reducing anxiety among ischemic stroke patients using GIM with traditional music. The aim of the study is to determine the effect of GIM with sasando music on anxiety among ischemic stroke survivors. MATERIALS AND METHODS: The method was a quasi-experimental design with a pretest-post-test with a control group. Respondents are 50 ischemic stroke survivors divided into two groups and selected by …

Music therapy in addition to music listening for the prevention of delirium in mechanically ventilated patients.

INTRODUCTION: Delirium in mechanically ventilated patients has resulted in prolonged intensive care unit (ICU) and ventilator stay and higher mortality. Research has shown that a bundle containing sedation awakening and spontaneous breathing trials, appropriate choice of sedatives, early mobilization, and family engagement was associated with a reduced rate of delirium. Music listening and music therapy have also been studied, showing promising results for anxiety and pain control, but have not been studied in terms of preventing delirium. METHODS: As part of a quality improvement project, we…

Music Perception in Older Adults With Hearing Loss: Protective Effect of Musical Experience.

OBJECTIVES: The goal of this project was to investigate the impact of musical experience, hearing loss, and age on music perception in older adults. The authors hypothesized that older adults with a varying degree of musical experience would perform better at music perception tasks than their counterparts without musical experience while controlling for age and hearing loss. DESIGN: This study used a descriptive correlational cross-sectional design. Seventy-seven older adults aged 60 to 90 years were recruited and divided into two groups based on their lifetime musical experience: the group w…

Music Training and Education Slow the Deterioration of Music Perception Produced by Presbycusis in the Elderly.

The perception of music depends on the normal function of the peripheral and central auditory system. Aged subjects without hearing loss have altered music perception, including pitch and temporal features. Presbycusis or age-related hearing loss is a frequent condition in elderly people, produced by neurodegenerative processes that affect the cochlear receptor cells and brain circuits involved in auditory perception. Clinically, presbycusis patients have bilateral high-frequency hearing loss and deteriorated speech intelligibility. Music impairments in presbycusis subjects can be attributed t…
