The prognostic value of myocardial perfusion imaging in patients with type 2 myocardial infarction.

OBJECTIVES: The aim of this retrospective study is to evaluate the prognostic role of myocardial perfusion imaging (MPI) in patients with type 2 myocardial infarction (T2MI). BACKGROUND: T2MI is an increasingly common diagnosis in clinical practice. The management of this condition is controversial and the prognostic value of MPI has not been established in this setting. METHODS: We retrospectively studied T2MI patients who underwent vasodilator gated MPI within 90 days of T2MI at a single tertiary care institution in 2013. Abnormal myocardial perfusion was defined as the perfusion defect in…

Regional heterogeneity of myocardial perfusion in healthy human myocardium: assessment with magnetic resonance perfusion imaging.

The knowledge of myocardial perfusion in healthy volunteers is fundamental for evaluation of patients with ischemic heart disease. The study was conducted to determine range, regional variability, and transmural gradient of myocardial perfusion in normal volunteers with Magnetic Resonance Perfusion Imaging (MRPI). Perfusion was assessed in 17 healthy volunteers (age: 20-47 yr, 11 males) at rest and adenosine-induced hyperemia using a 1.5 T MR scanner. Perfusion was quantified (mL/g/min) for the transmural myocardium and separately for the endo- and epimyocardium in the anterior, lateral, poste

Panic attack triggering myocardial ischemia documented by myocardial perfusion imaging study. A case report.

BACKGROUND: Chest pain, a key element in the investigation of coronary artery disease is often regarded as a benign prognosis when present in panic attacks. However, panic disorder has been suggested as an independent risk factor for long-term prognosis of cardiovascular diseases and a trigger of acute myocardial infarction. OBJECTIVE: Faced with the extreme importance in differentiate from ischemic to non-ischemic chest pain, we report a case of panic attack induced by inhalation of 35% carbon dioxide triggering myocardial ischemia, documented by myocardial perfusion imaging study. DISCUSSI…

Myocardial damages in systemic sclerosis detected by gated myocardial perfusion SPECT and sympathetic imaging.

BACKGROUND: Cardiac involvement is an important factor for the appropriate management of systemic sclerosis (SSc). The possibility for detecting early myocardial damage was investigated using (99m)Tc methoxyisobutylisonitrile (MIBI) gated perfusion single photon emission computed tomography (SPECT) and (123)I metaiodobenzylguanidine (MIBG) sympathetic imaging. METHODS AND RESULTS: Twenty-three patients with SSc and 14 control subjects were studied. The severity of SSc was defined by disease type and semi-quantitative skin thickness scores. A myocardial perfusion study was performed using (99m

SPECT Myocardial Perfusion Imaging in the Era of PET and Multimodality Imaging: Challenges and Opportunities.

Single photon emission computed tomography (SPECT) remains the most widely used modality for the assessment of coronary artery disease (CAD) owing to its diagnostic and prognostic value, cost-effectiveness, broad availability, and ability to be performed with exercise testing. However, major cardiology guidelines recommend positron emission tomography (PET) over SPECT when available, largely due to its superior accuracy and ability to provide absolute myocardial blood flow quantification. A key limitation of SPECT is its reliance on relative perfusion imaging, which may overlook diffuse flow r

Attenuation of adenosine-induced myocardial perfusion heterogeneity by atenolol and other cardioselective beta-adrenoceptor blockers: a crossover myocardial perfusion imaging study.

UNLABELLED: Little is known about the effect of chronic beta-blockade on adenosine actions. We sought to investigate the effect of oral beta-blockers on the presence, extent, and severity of myocardial perfusion abnormality induced by adenosine in patients with coronary artery disease. METHODS: In this crossover study, 45 male patients with coronary artery disease on beta-blocker therapy with atenolol, bisoprolol, or metoprolol underwent adenosine myocardial perfusion imaging both on and off beta-blockade in a random order on separate days. Myocardial perfusion was assessed both qualitatively…

Direct Risk Assessment From Myocardial Perfusion Imaging Using Explainable Deep Learning.

BACKGROUND: Myocardial perfusion imaging (MPI) is frequently used to provide risk stratification, but methods to improve the accuracy of these predictions are needed. OBJECTIVES: The authors developed an explainable deep learning (DL) model (HARD MACE [major adverse cardiac events]-DL) for the prediction of death or nonfatal myocardial infarction (MI) and validated its performance in large internal and external testing groups. METHODS: Patients undergoing single-photon emission computed tomography MPI were included, with 20,401 patients in the training and internal testing group (5 sites) an…

"Ebstein's Anomaly" a Potential Cause of Misinterpretation on Myocardial Perfusion Imaging.

Ebstein's anomaly is a rare congenital heart disease. We present the case of a 47-year-old woman with history of Ebstein's anomaly who was referred to nuclear medicine department for a myocardial perfusion single-photon emission computed tomography (SPECT) imaging during pre-anesthetic check-up. Myocardial SPECT perfusion images with (99mTc)-Sestamibi demonstrated medium sizes fixed perfusion defect in the mid and proximal infero-septal wall adjacent inferior wall. This is a cause of potential misinterpretation on myocardial SPECT perfusion imaging as this perfusion defect was due to fibrosis

Whole-heart spiral simultaneous multi-slice first-pass myocardial perfusion imaging.

PURPOSE: To develop and evaluate a simultaneous multislice (SMS) spiral perfusion pulse sequence with whole-heart coverage. METHODS: An orthogonal set of phase cycling angles following a Hadamard pattern was incorporated into a golden-angle (GA) variable density spiral perfusion sequence to perform SMS imaging at different multiband (MB) factors. Images were reconstructed using an SMS extension of L1-SPIRiT that we have termed SMS-L1-SPIRiT. The proposed sequence was evaluated in 40 subjects (10 each for MB factors of 1, 2, 3, and 4). Images were blindly graded by 2 cardiologists on a 5-point

Arm exercise myocardial perfusion imaging for prognostication of long-term outcome.

INTRODUCTION: Pharmacologic evaluations constitute ≥50% of imaging stress tests, but exercise reduces adverse effects, improves myocardial perfusion imaging (MPI) quality and diagnostic results, and provides powerful prognostic and clinically important information on exercise capacity and cardiovascular responses to the relevant physiologic stress of exercise. Thus, our purpose was to determine whether arm exercise and MPI variables predict long-term outcome in patients who cannot perform leg exercise. METHODS: We performed arm exercise MPI stress tests in 253 consecutive patients age 64.5 (1…

Myocardial perfusion imaging analysis in patients with regurgitant valvular heart disease.

BACKGROUND/AIM: We noticed that there was decreased correlation of the findings from myocardial perfusion imaging (MPI) and cardiac catheterization (CATH) in patients with mitral regurgitation (MR) and aortic regurgitation (AR) compared to patients without valve disease. METHODS: Through a systematic review of MPI records at the Harry S. Truman Memorial Veterans' Hospital between 1998 and 2008, we identified 58 patients with at least moderate MR or AR by echocardiogram who underwent cardiac catheterization within 6 months of the MPI study. A control group was formulated with 60 patients who d…

Incidental pathologic extracardiac uptake of 99mTc-tetrofosmin in myocardial perfusion imaging.

Technetium-99m-tetrofosmin ((99m)Tc-TF) myocardial perfusion studies have incidentally detected various extracardiac abnormalities. The interpretation of these findings may be essential for early diagnosis and treatment of important diseases. We present a rare case of a mediastinal thymoma incidently detected during myocardial perfusion imaging. A 60 year-old woman, with precardiac symptoms of possible myocardial ischemia, underwent a (99m)Tc-TF stress-rest single photon emission tomography test. Intense uptake of the radiotracer in the left paracardiac area, was observed. The computerized tom…

Intracoronary myocardial perfusion imaging: patterns in patients with coronary artery disease.

Myocardial imaging following the intracoronary injection of radiolabeled particles is used to identify transmural scars in patients being evaluated for coronary atherosclerosis. Selective imaging of the microcirculation derived from each major coronary vessel is accomplished using a dual radionuclide technique. This report illustrates the various normal and abnormal imaging patterns encountered in patients with coronary artery disease. The regional myocardial nomenclature proposed by the American Heart Association Council on Cardiovascular Surgery is used. Correlation of the nuclear study with

Deep Learning-Based Image Registration in Dynamic Myocardial Perfusion CT Imaging.

Registration of dynamic CT image sequences is a crucial preprocessing step for clinical evaluation of multiple physiological determinants in the heart such as global and regional myocardial perfusion. In this work, we present a deformable deep learning-based image registration method for quantitative myocardial perfusion CT examinations, which in contrast to previous approaches, takes into account some unique challenges such as low image quality with less accurate anatomical landmarks, dynamic changes of contrast agent concentration in the heart chambers and tissue, and misalignment caused by
