The anatomy and pathophysiology of neck pain.

Neck pain should not, and must not, be confused with cervical radicular pain. Equating the two conditions, or confusing them, results in misdiagnosis, inappropriate investigations, and inappropriate treatment that is destined to fail. So critical is the difference that pedagogically it is unwise to include the two topics in the same book, let alone the same article. However, traditions and expectations are hard to break. In deference to habit, this article addresses both entities, but does so by underplaying cervical radicular pain so as to retain the emphasis on neck pain.

A man in the barrel with neck pain.

A 38-year-old man presented with an abrupt onset of occipital and neck pain, radiating to both shoulders. The pain was accompanied by inability to lift his arms against gravity (the "man-in-the-barrel" syndrome). These symptoms were associated with bilateral hand paresthesias, right-sided throbbing headache, vertigo, nausea, and vomiting. All symptoms resolved within 30 minutes, but arm weakness recurred. The differential diagnosis and historical origins of the "man-in-the-barrel" syndrome are reviewed.

The importance of diagnosing neck pain.

Diffuse idiopathic skeletal hyperostosis is a common but poorly recognised condition that may have important and occasionally life-threatening clinical outcomes. We report the case of a 71-year-old man with giant osteophytes in his cervical spine which caused dysphagia and silent aspiration, leading to pneumonia, septicaemia, aortic wall infection and septic arthritis. Early recognition of the cause of his neck pain may have averted the subsequent clinical course.

Neck and back pain in bicycling.

Back and neck problems in bicyclists should be managed by a combination of bicycle adjustment or modification, technique change, and medical treatment. The bicycle should be checked for proper fit. Often it is necessary to relieve the rider's extended position by using handlebars with less drop, using a stem with a shorter extension, raising the stem, or moving the seat forward. Changing hand positions on the handlebars frequently, riding with the elbows "unlocked," varying head position, using padded gloves and handlebars, and riding on wider tires all reduce the effects of road shock. Initia

Postanginal sepsis. A pain in the neck.

The primary care physician must be on the alert when a patient presents with a sore throat, signs of sepsis, and a rapidly deteriorating condition. Recognition of this distinct clinical presentation should lead to the diagnosis of postanginal sepsis. Once this diagnosis is suspected, therapy with an antibiotic that provides coverage against streptococci and anaerobes should be initiated promptly, and a drainable focus of infection should be excluded.

Control of pain in the head and neck.

The patient with head or neck pain should be managed primarily with medication that will adequately prevent, rather than relieve, pain. The terminal patient should receive medication to which he will not develop tolerance and that will permit him to function in his normal environment. Although nerve blocks are not a panacea for all pain syndromes, in carefully selected subjects, chemical interruption of nerve pathways can provide significant relief for chronic pain sufferers.

Neurostimulation for neck pain and headache.

Patients with medically refractory headache disorders are a rare and challenging-to-treat group. The introduction of peripheral neurostimulation (PNS) has offered a new avenue of treatment for patients who are appropriate surgical candidates. The utility of PNS for headache management is actively debated. Preliminary reports suggested that 60-80% of patients with chronic headache who have failed maximum medical therapy respond to PNS. However, complications rates for PNS are high. Recent publication of 2 large randomized clinical trials with conflicting results has underscored the need for fur

Not just another pain in the neck.

A 56-year-old man presents to the emergency department with complaints of neck pain with numbness to the right upper extremity. The complaints and symptoms initially appear to be cervical radiculopathy but further diagnostic testing during an inpatient hospitalization revealed an unusual diagnosis. The purpose of this article is to bring awareness to practitioners of the alternative and sometimes rare diagnosis of neck pain. A detailed history and physical examination accompanied with diagnostic testing and collaboration can ensure a definitive diagnosis and positive outcome.

The electrodiagnostic evaluation of neck pain.

Electrodiagnostic studies play an important role in the evaluation of radiculopathy. This article reviews the use of standard nerve conduction studies, late responses, evoked potentials, and needle electrode examination in the electrodiagnostic evaluation of neck pain.

Boerhaave's syndrome: a pain in the neck.

Boerhaave's syndrome, or post-emetic rupture of the oesophagus, classically presents with vomiting, chest pain and subcutaneous emphysema. Mortality in this condition is very high and increases dramatically with delayed diagnosis and intervention. The vast majority of patients have a tear in the left posterior-lateral wall of the lower third of the oesophagus and require urgent surgical intervention. Spontaneous rupture of the cervical oesophagus is very rare and may present differently to oesophageal perforations elsewhere. A case is presented following vomiting in a 70-year-old woman, which
