Direct or incidental exposure of the nervous system to therapeutic irradiation carries the risk of symptomatic neurologic injury. Central nervous system toxicity from radiation includes focal cerebral necrosis, neurocognitive deficits, and less commonly cerebrovascular disease, myelopathy, or the occurrence of a radiation-induced neoplasm. Brachial or lumbosacral plexopathy are the most common syndromes of radiation toxicity affecting the peripheral nervous system. This article focuses on the clinical features, diagnosis, and management options for patients with radiation neurotoxicity.

Fistula formation after radical cystectomy is rare, often presenting early and involving the small bowel. Herein we describe a case of an appendiceal fistula in a patient who underwent chemoradiotherapy and pelvic exenteration over a decade prior to the presentation of their fistula. Initially suspected to be radiation-induced, final pathology revealed a mucinous neoplasm of the appendix, which had fistulized with the ileal conduit. This case emphasizes the importance of considering alternative causes, such as malignancy, in delayed fistulas, especially in patients with prior radiation. Clinic

For the modern practitioner of women's imaging, achieving a balance between the positive diagnostic benefits available from current medical imaging on the one hand, and the potentially deleterious effects of ionizing radiation exposure on the other, has become a central issue. Increased public and professional awareness of the side effects of radiation now require a comprehensive understanding of the facts involved, the various risks to which patients are exposed, and the measures that can be implemented to minimize these risks. The major challenges posed by pregnancy, radiosensitive breast ti

Optic neuropathy induced by radiation is an infrequent cause of delayed visual loss that may at times be difficult to differentiate from compression of the visual pathways by recurrent neoplasm. The authors describe six patients with this disorder who experienced loss of vision 6 to 36 months after neurological surgery and radiation therapy. Of the six patients in the series, two had a pituitary adenoma and one each had a metastatic melanoma, multiple myeloma, craniopharyngioma, and lymphoepithelioma. Visual acuity in the affected eyes ranged from 20/25 to no light perception. Magnetic resonan

Even though much is known about cancer risk associated with exposure to ionizing radiation, societal demands for detailed risk assessment go far beyond our ability to satisfy them according to customary standards of scientific accuracy. Society's requirement, however, is for the best information available, however good it may be, and not necessarily for the definitive solution to the problem posed. Bayesian methods may be useful for presenting incomplete and varied data and informed scientific opinion in a form suitable for use in societal decision making, while at the same time providing a di

Therapy-related myeloid neoplasms are unintended and unwanted complications of cytotoxic chemotherapy and radiation. Unlike other environmental toxin-induced malignancies, exposure to the inciting agent is required to eradicate a primary and life-threatening cancer. In this review, we will focus on the biochemical mechanisms that lead to therapy-induced myeloid malignancy. This includes discussion of known mechanisms by which cytotoxic chemotherapy and radiation induce genetic mutations and promote evolution and expansion of malignant hematopoietic clones. Mechanisms by which the hematopoietic

After external-beam radiation therapy, radiation-induced changes may be observed in abdominal and pelvic organs at imaging. In the liver, an area of low attenuation corresponding to the radiation port (or an area of hyperattenuation if the underlying liver tissue shows fatty change) can be seen at computed tomography (CT) performed within 3-6 months after therapy. Later, the liver may be fibrotic and contracted. In the stomach, small intestine, and colon, wall thickening and edema are early manifestations. Ulcers may also be observed. Long-term complications include strictures and fistulas. Af

Masson tumor (MT, papillary endothelial hyperplasia) is an exaggerated form of thrombus reorganization rarely occurring in the central nervous system (CNS), where it presents as a mass or hemorrhage in parenchyma, meninges, or venous sinuses. MT is subclassified as type 1 arising within a histologically normal vessel, type 2 associated with a ruptured vascular malformation, and extravascular. Limited reports of CNS MT after radiosurgery, or especially external radiation therapy, have emerged. We searched our databases for cases reported from 2008 to present. Nine cases were identified, 6 of wh

There are two types of radiation health effect; acute disorder and late on-set disorder. Acute disorder is a deterministic effect that the symptoms appear by exposure above a threshold. Tissues and cells that compose the human body have different radiation sensitivity respectively, and the symptoms appear in order, from highly radiosensitive tissues. The clinical symptoms of acute disorder begin with a decrease in lymphocytes, and then the symptoms appear such as alopecia, skin erythema, hematopoietic damage, gastrointestinal damage, central nervous system damage with increasing radiation dose

The thesis that medical radiation exposures to the general population in the United States are excessive and that urgent steps need to be taken to reduce them is examined. At the present time, no evidence is found that deleterious effects result from radiation exposures at the level of a few rads or less. Nevertheless it is prudent that we assume the possibility of harmful effects of radiation exposure. However, because medical radiology is clearly so beneficial, and the harmful effects are problematical, it is doubtful whether the public should be alarmed by the suggestion of any crisis. A ba

The application of ionizing radiation in medical diagnostics and treatments has been transformative in advancing healthcare for the benefit of patients. However, with these advancements comes the need to understand and mitigate the risks of ionizing radiation. While the role of radiation in medicine is undeniable, its potential to induce malignancies and genetic alterations requires careful application and understanding. This review provides an overview about the radiobiology of radiation risk, and then go through the evolution, challenges, and inherent uncertainties surrounding radiation risk

Exposure to ionizing radiation in a nonmilitary emergency is likely to alarm those persons exposed and their families. Not only is the outcome uncertain, but the risks are difficult to convey. This communication discusses uncertainties in short-term outcome and long-term effects, as well as means of conveying the magnitude of risks to patients and their families. It emphasizes that the perception of radiation hazards is contextual. It offers three ways of presenting the prognostic long-term risks of accidental radiation exposure by comparing them with the incidence of cancer and genetic abnorm

Prompted by increasing litigation claiming cancer induction from exposure to radiation from nuclear weapons tests, the United States Congress enacted an amendment to the Orphan Drug Act (Public Law 97-414, January 4, 1983), through which the National Institutes of Health was directed to and did produce Probability of Causation (PC) Tables. This review defines PC and discusses the data, limitations, and uses of the concept.

There is much experimental data which indicates that ionizing radiation is a very potent carcinogenic agent. Most types of carcinoma can be produced by radiation. Carcinoma is apparently induced through a single or a series of mutations in somatic cells. Radiologists have excess leukemia and other malignancy from external x-ray; uranium and other miners have excess lung cancer from internal alpha radiation; luminous dial painters have excess osteogenic sarcomas; and uranium mill workers appear to have excess lymphomas. A large number of persons are now exposed occupationally to radiation from

Radon is a naturally occurring radioactive material that is formed as the decay product of uranium and thorium, and is estimated to contribute to approximately half of the average annual natural background radiation. When inhaled, it damages the lungs during radioactive decay and affects the human body. Through many epidemiological studies regarding occupational exposure among miners and residential exposure among the general population, radon has been scientifically proven to cause lung cancer, and radon exposure is the second most common cause of lung cancer after cigarette smoking. However,

Exposure of children to ionizing radiation most commonly is from the environment, chiefly through cosmic rays and radon, or from medical technology. Medical radiation exposure occurs during diagnosis, therapy, and dental radiography. More is known about the biological effects of exposure to ionizing radiation than to nonionizing radiation from microwaves, radiowaves, and the electrical fields of other electrical appliances. This review applies only to sources of ionizing radiation and does not include the potential risks of indoor radon. The effects on children of ionizing radiation have been

BACKGROUND: During the Chernobyl accident in 1986, 237 individuals were identified as having the most severe exposure to ionizing radiation. In the period between 1998 and 2000, 99 long term survivors out of this group were reassessed for radiation-induced cutaneous lesions.
OBJECTIVE: To identify sequelae of accidental cutaneous irradiation.
METHODS: Detailed dermatologic examinations, including biopsies of suspicious cutaneous lesions for histopathologic examination and 20 MHz sonography, were performed in all patients.
RESULTS: Twenty-two of the 99 patients displayed radiation-induced cu

Recent advances in therapy for pediatric hematologic neoplasms have greatly improved the prognosis but have resulted in an increased incidence of associated complications and toxic effects. The main neuroimaging features in pediatric patients with leukemia or lymphoma treated with chemotherapy or radiation therapy were retrospectively reviewed. To simplify the approach and facilitate differential diagnosis, the neuroimaging features have been classified into three main categories: central nervous system manifestations of primary disease, side effects of therapeutic procedures (radiation therap

This review summarizes the results of experiments conducted in the Institute for Environmental Sciences for the past 21 years, focusing on the biological effects of long-term low dose-rate radiation exposure on mice. Mice were chronically exposed to gamma rays at dose-rates of 0.05, 1 or 20 mGy/day for 400 days to total doses of 20, 400 or 8000 mGy, respectively. The dose rate 0.05 mGy/day is comparable to the dose limit for radiation workers. The parameters examined were lifespan, neoplasm incidence, antineoplasm immunity, body weight, chromosome aberration(s), gene mutation(s), alterations i
