Perioperative continuation or ultra-early resumption of antithrombotics in elective neurosurgical cranial procedures.

OBJECTIVE: Discontinuation of antithrombotics (AT) prior to elective cranial procedures is common practice, despite the higher risk of thromboembolic complications in these patients. The aim of this study was to investigate the risks and benefits of a new perioperative management protocol of continuation or ultra-early AT resumption in elective cranial procedures. METHODS: This study was an analysis of a prospectively collected cohort of patients undergoing elective cranial surgery with (AT group) and without (control group) AT. For extraaxial or shunt surgeries, acetylsalicylic acid (ASA) wa

A stereotactic near-infrared probe for localization during functional neurosurgical procedures: further experience.

OBJECT: The authors previously developed an optical stereotactic probe employing near-infrared (NIR) spectroscopy to provide intraoperative localization by distinguishing gray matter from white matter. In the current study they extend and further validate this technology. METHODS: Near-infrared probes were inserted 203 times during 138 procedures for movement disorders. Detailed validation with postoperative imaging was obtained for 121 of these procedures and with microelectrode recording (MER) for 30 procedures. Probes were constructed to interrogate tissue perpendicular to the probe path a

A comparison of frontal and occipital bispectral index values obtained during neurosurgical procedures.

UNLABELLED: We placed bispectral index (BIS) sensors on the frontal and occipital areas of neurosurgical patients and compared BIS values obtained from both areas during propofol/fentanyl anesthesia. BIS showed a strong correlation between frontal and occipital montages (r(2) = 0.96; P = 0.03). It may be valid to measure BIS with the sensor on the occipital area if required during frontal neurosurgical procedures. IMPLICATIONS: Bispectral values were positively correlated when recorded from frontal and occipital sensors in patients undergoing clipping of unruptured cerebral aneurysms while an…

Risk factors and morbidity associated with surgical site infection subtypes following adult neurosurgical procedures.

OBJECTIVE: Studies on surgical site infection (SSI) in adult neurosurgery have presented all subtypes of SSIs as the general 'SSI'. Given that SSIs constitute a broad range of infections, we hypothesized that clinical outcomes and management vary based on SSI subtype. METHODS: A retrospective analysis of all neurosurgical SSI from 2012-2019 was conducted at a tertiary care institution. SSI subtypes were categorized as deep and superficial incisional SSI, brain, dural or spinal abscesses, meningitis or ventriculitis, and osteomyelitis. RESULTS: 9620 craniotomy, shunt, and fusion procedures we

[Inflammatory Reactions to Hemostatic Agents in Neurosurgical Procedures: Two Cases Analysis and Clinical Implications].

Two patients, aged 72 years and 36 years, who underwent neurosurgical intervention using hemostatic materials intraoperatively, presented with clinical deterioration. Imaging demonstrated in both case collections were suspicious for abscess. In light of the clinical presentation and imaging findings, the patients underwent urgent repeat interventions in which sterile collections were found with surrounding brain edema, and in one case, the formation of granulomatous tissue at the surgical site. No macroscopic or laboratory evidence of an infectious process was found in either of the cases.

Andexanet Alfa in Emergency Cranial Neurosurgical Procedures: A Bicentric Analysis of Efficacy and Safety.

BACKGROUND: Factor Xa inhibitors are widely used anticoagulants but are associated with a known risk of bleeding complications. In 2018, andexanet alfa was approved as an antidote for the reversal of apixaban and rivaroxaban, demonstrating high efficacy in controlling intracranial hemorrhage. However, data on its use prior to neurosurgical procedures remain limited. Here, we present our experience with andexanet alfa in patients undergoing emergency cranial neurosurgical procedures. METHODS: Here, we present a case series based on a retrospective analysis conducted over a four-year period, id…

Combined ablative neurosurgical procedures in a patient with mixed spastic and dystonic cerebral palsy.

BACKGROUND: Harmful generalized spasticity is an obstacle in rehabilitation and caregiving. Neurosurgical intervention is a therapeutic option for patients with severe spasticity who do not respond to nonoperative management. Currently, intrathecal baclofen therapy (ITB) is a good alternative treatment for such patients. However, the ITB device is costly and the intrathecal drug is not available in Thailand. CASE DESCRIPTION: We report a combined use of ablative neurosurgical procedures in a patient with severe generalized spasticity and disabling cervical dystonia (CD). The assembled operati…

Isoflurane and cerebrospinal fluid pressure--a study in neurosurgical patients undergoing intracranial shunt procedures.

The effect of isoflurane 1.0% and 1.5% with nitrous oxide on ventricular cerebrospinal fluid pressure was studied in 17 patients who underwent intracranial shunt procedures. Isoflurane at both concentrations caused significant increases in cerebrospinal fluid pressure during normocapnic ventilation, but these could be prevented by simultaneous hyperventilation or by the prior induction of hypocapnia. Decreases in mean arterial pressure occurred also, and resulted in a significant reduction in cerebral perfusion pressure in normocapnic patients.

Remote cerebellar hemorrhage after a neurosurgical procedure: A report of three cases.

Remote cerebellar hemorrhage (RCH) is a rare complication of neurosurgical procedures and is characterized by a typical bleeding pattern defined as the "zebra sign." Only few cases of RCH have been reported in the English literature, and its pathophysiology remains unclear. In this report, we present the cases of three patients with RCH after three different procedures: burr-hole trephination and chronic subdural hematoma evacuation of bilateral cerebral convexity with subsequent subdural drain insertion, lumbar drainage for cerebrospinal fluid divergence for thoracic endovascular aortic repai…

The effect of different anesthetics on the incidence of AKI and AKD after neurosurgical procedures.

Acute kidney injury (AKI) incidence after neurosurgical operations has been reported as 10-14%. The literature regarding the incidence of nosocomial acute kidney disease (AKD) following neurosurgery is scarce. This retrospective, single-center, observational study aimed to assess the impact of different anaesthetics on development of postoperative AKI and persistent AKD in neurosurgical patients. We have categorized patients depending by the type of total intravenous anaestesia with propofol or sevoflurane. Most patients (74%) were on total intravenous anesthesia with propofol, while the rest

Is Pulse Co-Oximetry a Reliable Alternative to Invasive Hemoglobin Measurement in Pediatric Neurosurgical Procedures?

Background/Objectives: Pediatric neurosurgical procedures often involve significant blood loss and rapid hemodynamic shifts, necessitating accurate hemoglobin (Hb) monitoring. While continuous non-invasive Hb (SpHb) monitoring offers real-time trending, its accuracy in high-risk pediatric populations remains debated. We aimed to evaluate the diagnostic accuracy and clinical utility of SpHb compared to invasive arterial blood gas (ABG) analysis in pediatric patients undergoing cranial and spinal surgeries. Methods: This prospective, observational study enrolled 60 pediatric patients (aged 0-16 …

Analysis of Treatment Cost Variation Among Multiple Neurosurgical Procedures Using the Value-Driven Outcomes Database.

BACKGROUND: Health care costs comprise a substantial portion of total national expenditure. Although interest in cost-effectiveness analysis in neurosurgery has increased, there has been little cross-comparison of neurosurgical procedures. The aim of this study was to compare costs across elective neurosurgical procedures to understand whether drivers of cost differ. METHODS: The Value Driven Outcomes database was used to evaluate treatment costs for resection of vestibular schwannoma, intracranial meningioma, gliomas, and pituitary adenoma; anterior cervical discectomy and fusion and lumbar …

The role of endoscopic assisted microneurosurgery (image fusion technology) in the performance of neurosurgical procedures.

The use of endoscopic techniques has gained significant popularity over the past decade. The potential advantages of angled lenses to visualize around tight corridors without retracting have proven to be an attractive feature. The limitation of endoscopy has been the inability to obtain a true three-dimensional perspective for which microscopy is ideal. Therefore we postulated that for complex intradural lesions the merger of the two technologies would prove to be ideal. In this paper we report our experience with simultaneous microscopy and endoscopy, using picture-in-picture technology, for
