A 13-year-old Hispanic boy with no significant medical etymology presented with a chief complaint of widespread brown macules and patches. From January 1994 to October 1995, 5 patients with congenital giant pigmented nevi were treated using artificial dermis with a secondary skin graft from the scalp. The nevus was excised in full thickness and the open wound was grafted with artificial dermis. A 14-year-old girl presented with a new, rapidly growing, pigmented tumor on a previously existing yellowish, verrucous plaque on the scalp. The patient received complete surgical excision. Routine histology ruled out basal cell carcinoma (BCC) and the histological diagnosis was pigmented trichoblastoma arising in nevus sebaceous (NS). The histopathologic features of 120 cases of pigmented spindled nevus (PSCN) from the years 1973 through 1988 were reviewed from a consultative practice heavily weighted with difficult nevomelanocytic lesions. Becker's nevus (BN) clinically presents as brown patches with hypertrichosis. The risk of pigmentary changes still remains a concern when using lasers to treat Asian patients with BN.
