Seven cases of non-supportive polyarthritis are reported from the Madang General Hospital over a 12 month period from June 1972. The distribution of diagnostic categories is similar to that reported by Maddocks in Port Moresby, with Reiter's disease featuring prominently. One case of Reiter's disease was associated with a haemolytic anaemia, considered to be auto-immune in origin.

BACKGROUND: Cholera is newly emergent in Papua New Guinea but may soon become endemic. Identifying the risk factors for cholera provides evidence for targeted prevention and control measures.
METHODS: We conducted a hospital-based case-control study to identify cholera risk factors. Using stool culture as the standard, we evaluated a cholera point of care test in the field.

We developed a guinea pig model of cryptococcal meningitis to evaluate antifungal agents. Immunosuppressed animals challenged intracranially with Cryptococcus neoformans responded to fluconazole and voriconazole. Disease was monitored by serial cerebrospinal fluid (CSF) cultures and quantitative organ cultures. Our model produces disseminating central nervous system disease and responds to antifungal therapy.

A study of Burkitt's lymphoma (BL) in Papua New Guinea for the years 1958-87 revealed four instances of familial BL. Incident cases occurred within 1 year of each other in the four families. Personal follow-up was possible for three of these families whose pedigrees showed that two or more siblings were affected. Research in Papua New Guinea and elsewhere in the developing world has indicted the dangers of bottle feeding infants. Following a failure to obtain the voluntary agreement of shopkeepers to restrict sales of infant-feeding bottles and teats, legislation was passed which placed these items on prescription. In order to obtain a prescription certain conditions have to be satisfied. A follow-up survey which was carried out in Port Moresby two years after the introduction of legislation indicated a significant decline in the extent of bottle feeding.

Human trichinellosis and teniasis (Taenia solium) are meat-borne helminthic infections with a wide distribution throughout the world. However, there is little information on the prevalence of these infections in Papua New Guinea. Until 1993 complex surgery for children in Papua New Guinea (PNG) was usually conducted by general surgeons, or by overseas paediatric surgeons during ad hoc visits. There had been little coordination of these occasional international visits and no involvement in the surgical teaching program in the University Department of Surgery. Cryptococcal meningitis is a difficult disease to treat and requires biochemical and haematological monitoring to detect the common adverse effects of treatment. Combination therapy with amphotericin B and flucytosine for at least 6 weeks is the best treatment so far evaluated. The clinical features of severe falciparum malaria and risk factors for mortality were studied in 489 children admitted with malaria to Madang Hospital, Papua New Guinea. The most common severe manifestations of malaria were severe anaemia (22%) and coma (16%). Huntington's disease (HD) was observed in a 45-year-old male Melanesian patient from the Eastern Highlands Province of Papua New Guinea. The patient had multiple peripheral nerve palsies as a result of tuberculoid leprosy and had been resident in the leprosy ward of Goroka Base Hospital for over 6 years. During this time his HD had remained undiagnosed. This is the first report of HD in the Highlands of New Guinea.

The discovery of Huntington's chorea in New Britain, Papua New Guinea in 1977 was followed by the establishment of a National Huntington's Disease Register. It is suggested that enteritis necroticans occurs in the people of the Highlands of Papua New Guinea because they have low levels of digestive proteases in the intestinal lumen. These low protease levels are the result of a low-protein diet and the presence of heat-stable trypsin inhibitors in sweet potatato, the dietary staple. Low proteolytic activity allows the B toxin of Clostridium welchii type C, which is extremely susceptible to proteolysis, to initiate the disease.

Adequate nutrition is a medical priority. It has been estimated that more than half the children in the Chimbu District of Papua New Guinea are malnourished. A practical scheme combat poor nutrition there was based on emphasizing the value of breast-feeding and of locally grown foods. The daily menu given was the same, with three meals each day. The scheme's underlying principles can be adapted for any developing country or for migrant and other minority groups in a developed country.
