Nickel dermatitis. An update.

Several reports indicate that the prevalence of nickel sensitivity among females in the general population is around 10 per cent and probably is increasing, especially in schoolchildren. It has been claimed that there is such a high correlation between a positive history of nickel hypersensitivity and patch-test results that testing is unnecessary if there is a history of hypersensitivity. This is not true. Nickel hypersensitivity itself is not a serious health problem for the individual. However, when hand eczema also is present, the situation is quite different. Concerns about medical metal …

Embryotoxicity and genotoxicity of nickel.

Prenatal effects of nickel result from direct insults to the mammalian embryo as well as from indirect ones through maternal damage. Nickel may upset the hormonal balance of the mother and can impair the development of the preimplantation embryo. The metal can cross the feto-maternal barrier and enter the fetus. In addition to an increase in prenatal and neonatal mortality, nickel can produce different types of malformations in the surviving embryos but its teratogenic action seems to be delayed, probably as a result of retarded transfer via the placenta. No definite conclusions can be reached…

Systemic Nickel Allergy Syndrome.

Systemic nickel allergy syndrome (SNAS) is characterized by both cutaneous and extracutaneous symptoms following systemic exposure to nickel, most commonly through the ingestion of nickel-containing foods in individuals sensitized to nickel via skin contact. Patients with SNAS present with dermatitis in addition to gastrointestinal, neurologic, and other extracutaneous symptoms. The most widely implemented treatment is the low-nickel diet. The present authors saw a patient with a history of allergic contact dermatitis to nickel experience resolution of hand dermatitis, chronic pain (diagnosed …

Nickel allergy, how deep?

A 41-year-old woman was referred to our Cardiology Unit to evaluate the feasibility of a percutaneous closure of her 20-mm atrial septal defect. During the last few months she had increasing dyspnoea, and an echocardiographic study had disclosed an atrial septal defect with significant right ventricular enlargement. Her past medical history was silent, except for Nickel (Ni2+) allergy confirmed by a skin patch test. Ni2+ allergy is a problematic issue in patients scheduled for Ni(2+)-containing devices. The atrial septal occluder device that we used consists of a metallic alloy composed of 55%…

Nickel release from coins.

Nickel allergy is the most frequent contact allergy and is also one of the major background factors for hand eczema. The clinical significance of nickel release from coins was discussed when the composition of euro coins was decided. Current European coinage is dominated by cupro-nickel coins (Cu 75; Ni 25); other nickel-containing and non-nickel alloys are also used. Nickel release from used coinage from the UK, Sweden and France was determined. It was shown that nickel ions are readily available on the surface of used coins. After 2 min in artificial sweat, approximately 2 microg of nickel p…

Legionnaires' disease: nickel levels.

Nickel concentrations in lung tissues of nine deceased Legionnaire victims average nine times that of controls. There is a significant correlation between the nickel levels in the lungs of the Legionnaire victims and the weights of the lung specimens. No similar correlation is evident in the study of the victims' kidney tissues or tissues from the controls.

Nickel Toxicity in Plants.

Nickel (Ni) is a ubiquitous trace metal and an essential ultra micronutrient. It is a constituent of several enzymes and is currently known to be involved in a number of physiological processes in plants, including nitrogen metabolism, growth and seed germination. However, at elevated concentrations, Ni exerts multiple toxic effects on various physiological processes, which leads to impaired plant growth and morphogenesis, and reduced productivity. This review summarizes the current knowledge of the mechanisms of Ni toxicity, analyzing its effects on enzyme activity, membrane structure and fun…

Nickel transfer by fingers.

We investigated fingers as a potential source of nickel transfer to the face in patients with allergic contact dermatitis to nickel and a history of facial dermatitis. Samples were collected from the fingers and cheeks of volunteers using the stripping method with standard adhesive tape, and nickel levels were quantified using mass spectrometry. Fingers and cheeks of individuals who had handled coins were both positive for nickel, with levels ranging from 14.67 to 58.64 ppm and 1.28 to 8.52 ppm, respectively. The levels in a control group were considerably and significantly lower. Transfer of …

Nickel and human health.

This review focuses on the impact of nickel on human health. In particular, the dual nature of nickel as an essential as well as toxic element in nature is described, and the main forms of nickel that can come in contact with living systems from natural sources and anthropogenic activities are discussed. Concomitantly, the main routes of nickel uptake and transport in humans are covered, and the potential dangers that nickel exposure can represent for health are described. In particular, the insurgence of nickel-derived allergies, nickel-induced carcinogenesis as well as infectious diseases ca…

[Mechanisms of nickel allergy].

Nickel allergy constitutes a serious health problem of modern societies. Hypersensitivity to this metal is found in 13% adults and 8% children. Risk factors for nickel allergy are: female gender and early exposure to nickel, e.g. piercing. Various mechanisms of inducing nickel allergy are possible, which is also reflected in the different clinical pictures. Nickel can induce allergic reaction in 3 different ways: 1) it binds to carrier protein in the extracellular space and subsequently is processed and presented by antigen presenting cell (APC) in the context of MHC class II molecule, which a…

Breaking tolerance to nickel.

Nickel is a ubiquitous metal frequently responsible of allergic skin reactions. Development of undesired reaction to nickel has been positively correlated with the expansion of specific CD8+ T cells, that induce apoptosis of nickel-loaded keratinocytes through a perforin-dependent mechanism. In non allergic individuals, the lack of circulating CD8+ T cells reactive to nickel, as well as the absence of inflammatory responses at the site of skin exposure to the metal are the consequence of the expansion of specialized T cells with regulatory function. Among these, CD4(+)CD25+ T cells from periph…

Nickel allergy and orthodontics.

Nickel is the most common metal to cause contact dermatitis in orthodontics. Nickel-containing metal alloys, such as nickel-titanium and stainless steel, are widely used in orthodontic appliances. Nickel-titanium alloys may have nickel content in excess of 50 per cent and can thus potentially release enough nickel in the oral environment to elicit manifestations of an allergic reaction. Stainless steel has a lower nickel content (8 per cent). However, because the nickel is bound in a crystal lattice it is not available to react. Stainless steel orthodontic components are therefore very unlikel…

Nickel sensitivity in Singapore.

Nickel is the most common cause of allergic contact dermatitis in Singapore, resulting in positive reactions in patch tests between 7 and 14%. Women are more commonly affected and have an earlier mean age of onset than men (21 compared with 32). Wrist watches, earrings, buttons, and necklaces are the common source of nickel allergy. Twenty-five percent of patients also had some form of associated hand eczema. Thirty-six percent of patients had a history of atopy showing a higher incidence than the normal populations. Thirty-seven percent of patients had an associated positive reaction to cobal…

Nickel dermatitis in infants.

8 cases of allergic contact dermatitis to nickel in infants are reported. All showed a papular dermatitis matching the sites of contact. Patch testing was performed on 3 patients, 2 were tested to nickel sulfate in pet. at concentrations of 1.0%, 1.5%, 2.0%. 1 was tested to 2.5% alone. All developed ++ reactions at each concentration tested. We observed a strong association of nickel dermatitis with atopy; 7 of 8 patients had a family history of atopy and 5 of 8 had features of coexistent atopic dermatitis. The relationship between atopy and allergic contact dermatitis is briefly reviewed. Nic…

Human exposure to nickel.

In order of abundance in the earth's crust, nickel ranks as the 24th element and has been detected in different media in all parts of the biosphere. Thus, humans are constantly exposed to this ubiquitous element, though in variable amounts. The average natural nickel exposure from food in the past has probably been somewhat, but not much, below current levels. Nickel is a useful metal, particularly in various alloys, in batteries and in nickel-plating. Nickel compounds are used especially as catalysts and pigments. In nickel-producing or nickel-using industries, about 0.2% of the work force ma…
