A cross-sectional study was conducted to evaluate the effect of a community-based health education intervention programme and to study the determinants of night blindness in Bangladesh. The intervention programme was implemented to reduce the morbidity of nutritional blindness (night blindness) in the northern part of Bangladesh (Ranjpure district) during 1986-1989. A 53-year-old woman developed night blindness 9 years after jejunoileal bypass surgery. Abnormal rod function was confirmed by dark-adapted electroretinography, and vitamin A deficiency was confirmed by low serum levels of vitamin A and carotene. Administration of oral vitamin A supplementation resulted in complete resolution of the patient's symptom and return of the electroretinogram to normal. Twelve cases of adult-onset blindness were identified in a flock of 130 polled Wiltshire sheep in New Zealand over a 3-year period. Affected sheep developed night blindness between 2 and 3 years of age, which progressed to complete blindness by 4 to 5 years of age. While there are over 100 distinct mutations in the rhodopsin gene that are found in patients with the degenerative disease autosomal dominant retinitis pigmentosa (ADRP), there are only four known mutations in the rhodopsin gene found in patients with the dysfunction congenital stationary night blindness (CSNB). The ERGs of 9 cases (18 eyes) of congenital stationary night blindness with normal fundi or myopia were tested. All eyes showed nonrecordable rod ERG and cone ERG with normal a-wave. Scotopic mixed ERGs were of the negative type in 7 eyes and of the subnormal type in 11 eyes. The b/a ratio was reduced in all eyes. The ERG characteristics are useful for the classification, estimation of the probable location of the lesion, and differential diagnosis of the disease.

Two sisters, aged 14 and 7 years, and their father had ophthalmoscopic and electrophysiologic findings consistent with fundus albipunctatus. Evaluation of family members revealed a history of severe juvenile night blindness which resolved by about age 20 in some affected individuals. This familial pattern is consistent with autosomal dominant fundus albipunctatus.
